3. The wastewater code requires inspections during the installation.The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing
Special Provisions: Prior to Inspection Report approval, protection for the well casing in the driveway is to be
provided.

Subject: Mountain Air Estates, Block 1, Lot 2 —6900 Fernwood
Septic System Design
Dear On-Site Services Engineer:
The existing septic system on the subject lot has failed and a new system must be placed to serve
the four-bedroom home. The existing septic tank will be decommissioned in accordance with
Municipal Code. We are submitting this permit application to construct the new septic system.
The attached site plan identifies the location of the home and the existing well and septic system
along with the proposed septic location. No conflicts exist between this proposed system and any
other well or septic system on this lot or adjacent lots.
The ground surface on the lot in the area of the proposed absorption system slopes to the
northwest at 15%. There are no slopes greater than 25%within 50 feet downslope of the primary
site. Topographic contours are shown on the site plan indicating the grade and direction of flow.
Stormwater drainage will not impact this septic system. The new trenches will be constructed
parallel to the slope as much as possible.
Wells on this and adjacent lots are shown. The new system will be a minimum of 100' from all
wells and surface water, and more than 5' away from the septic tank.
Please refer to the attached test hole log, and plan and profile sheets for the septic design. If this
design is followed, there will be no adverse impacts to adjacent properties.

We request you issue a permit to upgrade the septic system serving the
four bedroom house on the referenced property.
An adequacy test performed on the existing system for Health Authority Approval
purposes found the absorption capacity of the existing system to be inadequate.
A test hole was excavated and a percolation test performed in the area of the
proposed upgrade. The approximate location of the test hole is located on the
attached site plan. At the time of excavation water was encountered at 10 feet
and after seven day water monitoring water was found at 9 feet. Attached is the
proposed upgrade design.
We do not anticipate any adverse effects on neighboring properties by the
installation of the proposed septic system.

During a recent Health Authority Approval on-site inspection and
test of the potable water supply well on Lot ~ Block . /
of ~T~I~ ~/~ ~Z~£~ Subdivision, the well's productivity
was determined to be .~2 gallons per minute. The minimum well
produgtivity required by this department (AMC 15.55) for
a ~ bedroom residence is .4~ gallons per minute.
Although the subject well currently exceeds this minimum
requirement, all parties concerned are advised that the
production capacity of the well may fluctuate. Restriction of
noncritical water uses such as washing ca~s and water.ing lawns
and gardens may be required.
This advisory must be attached to all copies of the subject

DATE OF TEST: OCTOBER 2, 6, AND 9 1986.
TEST PROCEDURE: SYSTEM WAS INSPECTED AND MEASURED. TANK WAS FOUND
WITH TWO FEET OF COVER AND 49 INCHES OF LIQUID.
TWO SUMPS WERE FOUND FOR THE TRENCH. ONE SUMP WAS 9 FEET DEEP AND
DRY, THE OTHER WAS 11.5 FEET DEEP AND WITH 12 INCHES OF LIQUID.
ON OCTOBER 2, 460 GALLONS WAS ADDED TO THE TRENCH. THE
WATER LEVELS ROSE TO 16 AND 42 INCHES.AFTER SIX HOURS THE LEVELS
HAD DROPPED TO 14 AND 41.
ON OCTOBER 6 900 GALLONS WERE ADDED. THE LEVELS ROSE
TO 46 AND 67. AFTER SIX HOURS THE LEVELS WERE 35 AND 57 AFTER 21
HOURS THEY WERE 29 AND 51. '
ON OCTOBER 9, 1500 GALLONS WERE ADDED. THE WATER LEVEL
IN THE SHALLOW SUMP ROSE TO 62 INCHES. THE LEVEL IN THE TANK
REMAINED AT 48.5 INCHES. AFTER SIX HOURS THE LEVEL IN THE SUMP
WAS 49 INCHES. AFTER 21 HOURS THE LEVEL WAS 43.
TEST RESULT: THIS SYSTEM MEETS THE CODE REQUIREMENTS OF
THE MUNICIPALITY OF ANCHORAGE.
THE RESIDENCE HAS BEEN VACANT FOR SEVERAL MONTHS. THE TRENCH IS
LEACHING VERY SLOWLY BELOW THE 40 INCH WATER LEVEL. ABOVE THE 40
INCH WATER LEVEL THE TRENCH LEACHES SUFFICIENTLY FOR A 4 BEDROOM
HOUSE. RISING THE WATER LEVEL TO 62 INCHES RESULTED IN AN
INFILTRATION RATE IN EXCESS OF 900 GALLONS PER DAY.
The operational life of all septic systems depends on the local
soil conditions, groundwater levels that may fluctuate during the
year, and the water usage of the family being served by the
system. These conditions are outside the control of the evaluator
of this septic system. We can therefore not give any estimate of
how long the system will continue to meet the operational requi-
rements of the Municipality and State.

TEST PROCEDURE: WELL WAS PUMPED AT A CONSTANT RATE OF 5
GALLONS PER MINUTE WHILE THE DRAWDOWN WAS
MONITORED WITH AN ACOUSTIC PROBE. THE WELL
WAS PUMPED DRY. 150 GALLONS OF WATER WAS OBTAINED IN A PERIOD OF
27 MINUTES.THE STATIC WATER LEVEL WAS FOUND AT 64 FEET BELOW TOP
OF CASING. TOTAL DEPTH OF WELL WAS 150 FEET.
TEST FOR COLIFORMS: WATER WAS TESTED FOR COLIFORM BACTERIA ON
OCTOBER 3, 1986. TEST WAS NEGATIVE.
TEST RESULT: THIS WELL MEETS THE REQUIREMENTS OF THE
MUNICIPALITY OF ANCHORAGE.
The Municipal requirement for well flow is 150 gallons of water
per bedroom per 24 hours.This well surpasses this requirement.
The assessment of the condition of this well applies only to the
conditions as of this date. The flow rate of the well may change
due to subsurface conditions that may not be observed from the
surface, and changes in land use and other factors that may
impact the conditions of the aquifer feeding the well.

Prior to pumping there was approximately four inches of
liquid in the cleanout between the tank and the trench.
The liquid depth in the four inch s~mp at the end of the
trench was 52 inches. After pumping the cleanout was dry,
the depth in the sunup was unchanged at 52.
On April 21 370 gal was discharged into the sump over the
course of the day. Water was from the ~11. The following
