LEGAL: MOUNTAIN PARK ESTATES BLOCK 5, LOT 4B
The owner has requested that we obtain a septic permit to upgrade the existing collapsed septic
tank (concrete lid) on the above referenced lot. Due to the collapse and public health / safety,
we request an expedited review. We propose to install a 1000-gallon HDPE tank to serve the
existing 3-bedroom residence. The lot and area are served by private wells. The design will not

Dear Sirs:
The purpose of this letter is to provide the required design narrative in support of our application for
a permit to construct wastewater disposal facilities on Lot 4B, Block 5, Mountain Park Estates, located
at 13201 Ridgeview Drive. Soils logs, perc test results, a site plan, design drawings and specifications
are enclosed for your review. This application is essentially for a renewal of permit # 910277 which was
never constructed at that time, however a sand filter has been added in an attempt to enhance the
longevity of the previously approved soil absorption bed. A lot line waiver was issued in 1991 allowing
the new system to be within 2 feet of the north lot line, and presumably that waiver is still valid.
The proposed system will be constructed in the vicinity of test hole # 2. As can be seen from the soil
log, the native material between 9' and 15.5' is a slightly silty sand with a measured perc rate of less than
1 minute per inch. Using the soil application rate of 0.7 gpd/sq, ft. specified in the wastewater ordinance
for filter sand, this 3 bedroom residence requires a total absorption area of(3 x 150)/0.7 = 643 square
feet. The proposed 20' x 33' bed design has a total absorption area of 660 square feet.
The topography of the lot slopes down towards the north at approximately 3%. The location of the
bed will encompass test hole #2, however that should not create any problem with settling since the test
hole was backfilled with sandy material which has had over 5 years to stabilize.
The proposed project will have no significant impact on present or future water supply and
wastewater disposal systems serving adjacent properties, nor will it have any significant impact on
reserved space-surface and subsurface, or on drainage.

Wastewater disposal system installation
Specifications
1.0 General:
1.1 The scope of the project consists of installation of a 1000 gallon septic tank which discharges
into a 20"wide x 33"long soil absorption bed.
1.2 Construction shall be as depicted on the approved site plan and design drawings. Minor
deviations from these drawings may be allowed or required by the engineer conducting the inspections.
All construction procedures and material specifications shall conform with Municipal and State
requirements. Ail separation distances shall be in conformance with Municipal requirements, unless
specifically waived.
1.3 The contractor shall be responsible to obtain any necessary utility locates, and to work around
any buried utilities.
1.4 The contractor shall provide adequate cover material and rough grading over all system
components to ensure that proper drainage is achieved after settlement and that there are no residual
depressions. Insofar as possible the contractor shall minimize damage to trees and existing lawn areas.
1.5 Unless specifically agreed otherwise, the homeowner shall be responsible for finish grading after
the soil is compacted, as well as placement of topsoil and reseeding all areas disturbed by the
construction.
1.6 Prior to the start of construction, the property owner shall arrange for a surveyor to locate the
northeast corner of the lot and to stake the lot lines in both directions at 25' and 50' from this corner.
2.0 Septic Tank:
2.1 The new 1000 gallon, 2 compartment septic tank shall be Municipally approved and shall be set
level on undisturbed soil. Each compartment shall be equipped with a watertight manhole cover and a 4"
cleanout. If the tank is buried less than 4 feet, it shall be insulated with 2 inches of approved burial type,
rigid insulation. The existing septic tank shall be properly abandoned.
2.3 Ail pipe connections to the tank shall be equipped with waterproof mechanical couplings. The
waste line from the residence to the septic tank shall have a minimum slope of 1/4'" per foot, and the
waste line between the tank and the soil absorption system shall have a minimum slope of 1/8" per foot.
A cleanout shall be installed within 5 feet of the building foundation, and a double cleanout shall be
installed within 5 feet downstream of the septic tank. A second double cleanout shall be installed near the
midpoint of the waste line between the septic tank and the soil absorption bed.
3.0 Soil absorption system:
3.1 The soil absorption system shall be constructed by excavating the surface fill and silt from the
entire 20' wide x 33 foot long bed area down to expose a fine sand stratum at a depth of approximately
9.5 feet below ground level in the vicinity of test hole #2.
3.2 The bottom of the excavation shall be level. Any compacted or smeared surfaces shall be raked
to allow proper infiltration. A total of 2 feet of filter sand shall be placed in the bottom of the excavation.
Sand serving as filter material shall have less than 4% passing the #100 sieve and shall comply with
Municipal filter sand specifications. "D.O.T. Road Sand" as manufactured by CPP meets these
specifications.
3.3 A total of 1.0 feet of approved sewer gravel shall be placed in the bottom of the excavation with
the perforated distribution pipes laid level such that the pipe inverts are no less than 0.5 feet above the
bottom of the sewer gravel. Sewer gravel shall be 0.5" - 2.5" washedand screened gravel (as supplied
by Anchorage Sand and Gravel), with less than 3% passing the #200 sieve.
3.4 Monitor tubes and cleanout pipes shall be of 4" diameter and installed in the locations shown on
the design drawings. The portion of the monitor tube extending through the sewer gravel shall be
perforated.
3.5 Approved filter fabric shall be placed over the entire top surface of the sewer gravel. A minimum
of 2 feet of soil cover is to be placed over the filter fabric.
3.6 The top surface of the cover material shall be raised a minimum of 12 inches higher than the
surrounding terrain to allow for subsequent settlement, and shall be graded to smooth contours. Fill
slopes shall be no steeper than 3:1.
3.7 Unless specifically agreed otherwise the homeowner shall be responsible for arranging to have
the site finish graded after the backfill material has stabilized, and for placement of adequate topsoil and
seed to promote rapid revegetation of all areas disturbed by the construction.
4.0 Inspections:
4.1 A total of 5 engineering inspections will be required during the course of the project: (1) initial
stakeout with the contractor to establish the location of the system and to discuss the plans, specifications
and construction procedures, (2) after the native material has been excavated to expose the infiltrative
surface to ensure that it is level and at the right elevation, and conforms with the soil test information, (3)
after the sand filter material has been placed, (4) after the sewer gravel is in place and the distribution
pipes have been laid and connected up to the septic tank, but prior to placement of insulation or filter
fabric, and (5) after rough backfill and grading is complete. The septic tank requires one inspection after
it is set level and the piping connected, but prior to backfill. This inspection may be incorporated with
any of the above inspections.
4.2 The installer shall coordinate the timing of the inspections with the engineer sufficiently far in
advance to ensure the availability of the engineer.

A new permit must be obtained from this office for a well
and/or on-site wastewater system NOT installed by the
expiration date.
If you have drilled the well, a well log must be sent to
this office for documentation of the installation and to
close the permit.
If a licensed Professional Engineer has inspected the
installation of the on-site wastewater system, the original
as-built inspection report must be sent to this office for
review, approval and documentation. All inspection reports
must be submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $200.00 for an
on-site wastewater permit; $75.00 for a well permit and
$275.00 for a combined on-site wastewater and well permit.

Dear Sirs:
The purpose of this letter is to provide the required design narrative in support of our application for a
permit to construct wastewater disposal facilities on Lot 4B, Block 5, Mountain Park Estates 4/1 S/D,
located at 13201 Ridgeview Drive. Soils logs, perc test results, a soil sample, a site plan, design drawings
and specifications are enclosed for your review.
The existing soil absorption system is surcharged, and in need of replacement. The proposed system
will be constructed in the vicinity of test hole # 2. As can be seen from the soil log, the native material
between 9' and 15.5' is slightly silty sand with a measured perc rate of less than 1 minute per inch. The
enclosed sample of material taken form the perc hole demonstrates that it is equivalent to "filter sand" as
described in the ordinance. Using the soil application rate of 0.7 gpd/sq, ft. for filter sand, this 3 bedroom
residence requires a total absorption area of (3 x 150)/0.7 = 643 square feet. The proposed 20' by 33' bed
design has a total absorption area of 660 square feet.
The location of the proposed bed will encompass test hole #2, however this should not create any
problem with settling because the test hole only goes 6 feet deeper than the bed bottom and was backfilled
with sandy material.
The topography of the lot in the vicinity of the proposed soil absorption bed slopes towards the north
at approximately 3%.
The proposed project will have no impact on present or futm'e water supply and wastewater disposal
systems serving adjacent properties, nor will it have any impact on reserved space/surface and substu'face,
or on drainage.

Specifications
1.0 General:
1.1 -The scope of the project consists of verifying the integrity of an existing septic tank and installing
a new 20' wide x 33' long soil absorption bed.
1.2 Construction shall be as depicted on the approved site plan and design drawings. Minor
deviations from these drawings may be allowed or required by the engineer conducting the inspections.
All construction procedures and material specifications shall conform with Municipal and State
requirements.
1.3 All separation distances shall be in conformance with Municipal requirements, unless specifically
waived.
1.4 The contractor shall be responsible to obtain any necessary utility locates, and to work around any
buffed utilities.
1.5 Prior to the start of construction, the property owner shall re'range for a surveyor to locate the
northeast corner of the lot and to stake the lot lines in both directions at 25' and 50' from this comer.
2.0 Septic Tank:
2.1 The existing septic tank may be retained in service only if it's structural integrity has been verified,
and if it has functional cleanout pipes with airtight caps, allowing pumping access to each compartment.
2.2 If after the contractor exposes the existing tank, it is found to not be in satisfactory condition, the
tank shall be properly abandoned and replaced with a new 1000 gallon Municipally approved tank with a
minimum of two compartments. The new tank shall be set level on undisturbed soil. Each compartment
shall be equipped with a watertight manhole cover and a 4" cleanout. If the tank is buried less than 4 feet,
it shall be insulated with 2 inches of approved burial type, rigid insulation.
2.3 All pipe connections to the tank shall be equipped with waterproof mechanical couplings. The
waste line from the residence to the septic tank shall have a minimum slope of 1/4"' per foot, and the waste
line between the tank and the soil absorption system shall have a minimum slope of 1/8" per foot. A
cleanout shall be installed within 5 feet of the building foundation, and a double cleanout shall be installed
within 5 feet downstream of the septic tank. A second set of double cleanouts shall be installed midway
on the line between the tank and the new soil absorption bed.
3.0 Soil absorption system:
3.1 The soil absorption system shall be constructed by excavating the surface fill and silt from the
entire bed area down to a expose a fine sand stratum at a depth of approximately 9.5 feet below ground
level in the vicinity of test hole #2.
3.2 The bottom of the excavation shall be level. Any sand used for leveling shall comply with
Municipal specifications.
3.3 A total of 1.0 feet of approved sewer gravel shall be placed in the bottom of the excavation with
the perforated distribution pipes laid level such that the pipe inverts are no less than 6 inches above the
bottom of the sewer gravel. Sewer gravel shall be 0.5" - 2.5" screened gravel, with less thm~ 3% passing
the #200 sieve.
3.4 Monitor tubes and cleanout pipes shall be of 4" diameter and installed in the locations shown on
the design drawings. The portion of the monitor tube extending through the sewer gravel shall be
perforated.
3.5 Approved filter fabric shall be placed over the entire top surface of the sewer gravel before
backfill. The top surface of the cover materiai shall be raised a minimum of 12 inches higher than the
surrounding terrain to allow for subsequent settlement, and shall be graded to smooth contours. The top 4
inches of cover material shall be topsoil. Fill slopes shall be no steeper than 3:1. The owner shall be
responsible for reseeding the disturbed areas.
4.0 Inspections:
4.1 A total of 4 engineering inspections will be required during the course of the project: (1) initial
stakeout with the contractor to establish the location of the system and to discuss the plans, specifications
and construction procedures, (2) after the native material has been excavated to expose the infiltrative
surface to ensure that it is level and at the fight elevation, and conforms with the soil test information, (3)
after the sewer gravel is in place and the distribution pipes have been laid and connected up to the septic
tank, but prior to placement of insulation or filter fabric, and (4) after final backfill and grading is
complete. The existing septic tank requires one inspection, and if a new tank is installed a second
inspection is required after it is set level and the piping connected, but prior to backfill. These inspections
may be incorporated with any of the above inspections.
5.2 The installer shall coordinate the timing of the inspections with the engineer sufficiently far in
advance to ensure the availability of the engineer.

Dear Mr. Bolles:
As we discussed this A.M., in submitting our application dated 8/29/91 for a sewer upgrade permit for
Lot 4B, Block 5, Mountain Park Estates #1, we neglected to request a waiver allowing the proposed soil
absorption bed to be within 2' of the north property line abutting Crestview Drive. By means of this
waiver request, we wish to correct that oversight.
The proposed design was selected to optimize the proximity of the system to the best soils which were
encountered in test hole#2, while staying away from the 100 foot radius from the private well serving the
lot. This forces the proposed bed to be within approximately 5 feet of the lot line, and the waiver to 2 feet
is to compensate for possible survey errors.
Since the requested waiver is for the distance to a lot line abutting a dedicated right of way, granting
the waiver will have no impact on any existing or possible future wells or wastewater disposal systems. It
will also have no impact on any possible future expansion of Crestview Drive.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering arralysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.

Nitrate Advisory
Certificate of On-Site Systems Approval # 101240
A Certificate of On-Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block 5, Lot 4B
of Mountain Park Estates subdivision. This inspection revealed a nitrate
concentration of 7.66 milligrams per liter (mg/L) was reported for the
property's well water sample. The Environmental Protection Agency (EPA)
h~s ~-stablished a mammum contaminant level (MCL) of 10.0 mg/i~ f~-r
public drinking water systems. While private wells are not subject to this
regulation, EPA standards are based on existing health information and can
therefore be used to gauge the relative quality of water from private wells.
Please see the attached "Nitrate Fact Sheet" for important information
regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On-
Site Systems Approval.
