Well and Septic System Permit Renewal Request
Ladies and Gentlemen:
I am writing to request that the permit (Permit SW010447, PID 020,121-11) to install the previously designed
wastewater system and water supply system be reissued for this lot. A site visit has been performed and it has been
verified that the soil conditions at the present time have stayed the same as when the permit was issued in 2001. We
performed water monitoring this date and found the test hole to be dry. The drain field was installed in September
of 2002 under the original permit. Inspections for this drainfield were performed by PES. These inspections were
called into your office and noted on the original permit. The Owner would like to install the septic tank in preparation
to sell the property. There are no excessive slopes or cut banks within 50 feet down hill of the proposed drainfield.
The required separation distances can and will be metfor all systems.
The proposed installation will not affect the future development of the surrounding or existing lots. There are no
wells or septic systems within 200 feet of the proposed septic location.

Well & Septic System Permit Request
Gentlemen:
My firm was contacted to investigate the possibility of installing a new septic system
serving this lot. Two test holes were excavated on August 22, 2001. The soils report
and a percolation test result is attached. Ground water was monitored for seven days.
No bedrock was encountered in the test hole. Ground water stabilized at a depth of 11
feet, just above a silty layer. A test hole was excavated in 1976 on the extreme western
portion of the lot and approximately 30 feet lower than the recent test holes. Ground
water stabilized at a depth of 1.5 feet below the surface.
The lot is approximately 1.5 acres in size. Lot 2 slopes to the west at approximately 10
percent in the area of the test holes, then flattens to approximately 2 percent on the
western portion of the lot. The proposed installation will be located in the eastern
portion of the lot. The proposed well will be located in the western portion of the lot,
greater than 100 feet from the proposed system. The proposed location is greater than
100 feet away from existing neighboring wells and 25 feet from the water service lines.
The surrounding wells are located greater than 100 feet from the proposed installation.
The proposed installation will not affect the future development of the surrounding or
existing lots. See the attached design.

Dear Mr. Kortie:
We are submitting herewith the test results and our comments regarding
soil conditions encountered at the subject site. This investigation
was performed in accordance with your request of May 12, 1976 and
those procedures outlined in a letter dated July 15, 1975, by Mr. Rolf
Strickland of the Municipality of Anchorage, Department of Environmental
Quality.
A single test hole was put down within the Lot 2 area for the purpose
of defining general subsurface soil conditions for the proposed sanitary
system. Excavation was accomplished with a truck -mounted auger type
drilling rig and the test hole was extended to a total depth of 17 feet
below ground surface. The final log prepared for the test hole has been
included in Drawing A-01.
Ground water was encountered in the test hole at a depth of 1.5' below
the surface which precluded an attempt to run a percolation test. It
was observed that the soil became some what dryer with depth.

5. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures outlined
in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or wastewater
disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated herein. I further verify that
based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply
and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes, ordinances, and regulations in effect at
the time of installation.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MOA COSA
guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test,
and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil
condition, ground water levels that may Fluctuate during the year, and the water usage of the family being served by the system. These conditions
are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future
performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provide any warranty
for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed
above.
