Mr. Electric of Anchorage did a minimum mount of work on the lift station o~ lot 7
block 3 in the Mile Iqigh Subdivision. However I called Mr. John Atbright with Dalton
_Electric and he a~sured me the depth requirements were f-h~h~ff[i~-t~rs are up to
~ff~'~. Electric did test the system and it was working at that time of the test, June
16t" 1997. There has no customer complaints about the system. According to Mr.
Albrigbt, his portion of the system meets NEC code requirements and t wit~ concur
with his judgment, to the best of my knowledge the system meets NEC code
requirements. Also the inspection compliance was met. audible alarm inside the
garage.

Dear ladies aud Gentlemen:
Concerning The lift station at Mile High Lot, Note on invoice #3514 when the
technician installed the audio signal inside the garage it was noted that thc wiring had
been compromised. Currently, the wiring coming from the lift station is type LrF-B.
However thc wire terminated in thc control box is 1),pc NM-B. This would indicate
that thc wiring was spliced someplace in thc ground or thc conduit. ]~ither way, Type
NM-B is un-expectable for th/s application, This was brought to thc contractor
attention but there w~s no oct/on taken, to my knowl~g~, to make the proper repairs.
At this time I can only say that the system was checked and working, I can not say it is
to NEC code.

Dear Mr. Williams:
On September 9th, we submitted an application for a well and septic permit
for the referenced lot. That permit ended up being lost. On September 24th, !
faxed a copy to your office as a replacement and signed the faxed copies as that
appeared to be a concern to you. Within that application was a request for a
slope wavier of 20 feet for the primary field and "0" feet for the reserve field.
On September 26th, I received notice that waivers to slope would not be
granted for any less than 35 feet. On that same day I contacted your office and
requested a meeting in the field to discuss the situation. On September 30th,
both you and Mr. Cross met with me at the site and reached an agreement as
follows:
That I would modify the drawing to show the trenches being moved up
slope so that they would be a minimum of 35' away from the steep cut
bank. In turn you would issue the permit so that the owner could pursue
his land use permit. At the owner's option, he could either provide a
septic system that was in keeping with the 35 foot set back requirement
from slopes in excess of 25% or dig another testhole on the other side of
the lot, confirm acceptable soils with a percolation test and proceed with
moving (flip flopping) the well and septic.
On October 1st, I resubmitted the site plan with the requested change. On
October 9th you contacted me and indicated that what was shown was not
acceptable to you and that you now wanted me to provide changes to the site
plan modification that would support the site plan shift that you required. I
advised you that I can provide you a drawing that would illustrate a design
that you requested, but that I couldn't assume any responsibility for its
construction or design applicability to the lot. Regardless, you requested that I
provide you this information anyway. Within 30 minutes you called my
office back and stated that you were sending me a written list of discrepancies

that addressed your new concerns with the site plan submitted.
Based on the above and your October 9th fax, I am providing you the
following requested information. The responses correlate to the numbered
items on your fax.

The slopes in the area that we were requested to shift the trench to are
approximately 60%. This differs significantly with the original area slope
of 31% which required a waiver of 20 feet and could have been easily
regraded to meet the 25% slope requirement. As it is within the
department's purview to grant waivers, since you have visited the site at
least three separate times, and we met and discussed the modifications
on the site, I can only assume that you were anticipating granting the
necessary slope waivers to construct the system. I have provided a
conceptual profile that would illustrate one approach to developing a
system at this site.
We have provided a drainage arrow with estimated slopes that should
augment the existing surveyed topography.
No action required to respond to.
I have reviewed the requirements under code for site plans and have
been unable to locate any referenced to a requirement to show any
change in slope by grading or cutting or a profile section of the site as it
will be at the time the trench is excavated. If these are requirements then
please provide the specific section within code that it refers to. With
regards to the request to show how the effluent is equally divided
between the two trenches please refer to the submitted site plan and you
will notice that there is a splitter clearly shown.
I have reviewed the requirements under code for site plans and have
been unable to locate any reference to a requirement to specifically
address the burial depth of the septic tank. Please provide the specific
section within code that it refers to. At this time we do not anticipate any
excessive burial depths. If burial depths are required we will insure that
the tank is appropriately specified for the depth of bury.
So far the soils logs have been submitted three times to your department.
On the last submittal logs from another project got mixed up with the
logs for this site. Attached you will find the correct logs.
All known systems within 200 feet of the our proposed wastewater
system have been shown in accordance with code. A note to that effect is

During a recent Health Authority Approval on-site inspection
and test of the potable water supply well on Lot 7
Block ~ of ~[~ ~[ Subdivision, the well's
productivity was determined to be ~,S67 gallons per minute.
The minimum well productivity required by this Department
(AMC 15.55) for a ~ bedroom residence is o.~i gallons
per minute° Although the subject well currently exceeds this
minimum requirement, all parties concerned are advised that the
production capacity of the well may fluctuate. Restriction
of non-critical water uses such as washing cars and watering
lawns and gardens may be required.
This advisory must be attached to all copies of the subject
Health Authority Approval.

A short history is in order regarding this residence. The lift station in concern here was
originally installed and wired by another contractor earlier this year. During the process of the
installation of underground AC power to the residence utility meter, the type UF cable supplying
power and control to the lift station was damaged. The contractor, Newberry Alaska hired
Dalton Electric to perform the necessary repairs, an employee of Dalton Electric was dispatched
to perform these repairs. A period of time later, an inspection of the residence was performed for
the reason of closing on a deed of trust (I assume this was the reason). The inspection was
performed by Mr. Electric and his report noted that the repair work was substandard: type NMB
romex used for the repair in place of type UF. Upon questioning the mechanic dispatched to
perform the repair it was stated that this was not the case and he had in fact used type UF cable in
conjunction with UL listed underground splice kits. For further verification, I travelled to the
residence and performed a visual inspection of the cable type that was installed. The cable type
installed by our mechanic was indeed type UF and is clearly labeled on the jacketing of the cable.
Also, from all appearances and from the conversationwith my mechanic, the burial depths of the
cable are also NEC code compliant. My electrical administrator license is 603.
