The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On -Site Systems
Approval (COSA) based only upon the representations given In paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.
4. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my
investigation, based on procedures outlined in the Certificate of On -Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) sale, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the
information obtained from the Municipality ofAnchorage files and from my investigation and inspection, the
on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal
and State codes, ordinances, and regulations in effect at the time of installation.

Engineer's Comments:
In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines 8 Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily Identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to moot the
operational requirements of the ADEC or MOA DSD. The content of this report Is for
the solo bonefit of the owner listed above. Any reliance upon or use of this report by any
other person or party Is not authorized, nor will it confer any legal right whatsoever.

Your request for a waiver of the required 100 feet horizontal separation from the
absorption field to the private well has been approved. The approved separation distance
is 90 foot horizontal separation from well on Mountain Park S/D #2 Block 3 Lot 3 to
septic crib on lot 2. This waiver approval also applies for a 0 foot lot line from septic crib
to west property line
Any future upgrade to the on-site wastewater disposal system will require all separation
distances be met or another approval from this department.

Septic Crib Data
The Crib was installed in 1972. This crib has encroached the well for
more than 20 years. The bottom crib elevation was assumed to be
10 feet and was most likely installed in a sandy gravel layer.
Well Data
The well was installed after the Crib on Lot 3. The well is
approximately 225 feet deep. There are detectable nitrates in the
surrounding area. The water samples for Lot 3 are with -in Municipal
Standards. According to Garness Engineering Nitrates are elevated
in the area and are not directly related to the 90 foot encroachment.

Ref: Waiver Request and COSA for Mountain Park Subdivision ti2; Lot 3, Block 2.
The existing 3 bedroom house is served by a private well and septic system. On 4/21/06 a site visit was
done in order to obtain a COSA. During the site visit the separation distance was measured from the crib
pipe to the well on Lot 3, Block 3, and was found to be 104 feet. The exact location of the perimeter of
the crib is unknown. Based upon conversations with your department a waiver is required. We request
you grant a 90 feet separation distance waiver from the crib on the referenced property to the well on Lot
3, Block 3; The septic system on the referenced property was installed on 9/21/1972 and the well and Lot
3, Block 3, was drilled on 8/4/1975; therefore the owner of Lot 3, Block 2, is not responsible for the
waiver fee. The following items are justification for the waivers:
• The ground slopes from west to east; however there is a ditch alongside the road, so that if the septic
system was to overflow, it appears that the effluent would not travel toward the well head.
• The location of the septic system is in a very visible area so that if any effluent was to surface, it
would be noticed and the problem corrected.
The other path of contamination is subsurface migration of wastewater. As can be seen on the attached
well logs, the static water levels in rearby wells ranges from 40-270 feet, and bedrock ranges from 30-
290 feet. There are several layers of silty sand, gravel and clay that have served to inhibit the migration
of untreated wastewater into the aquifer Recent water sample results indicated nitrate levels to be 5.70
mg/L and coliform bacteria results to be undetected. Based upon our experience, nitrate levels are
elevated throughout the area and are not directely related to this encroachment. This encroachment has
been in existence since 1975. Based upon the aforementioned facts, it appears that there is minimal risk
associated with the 90 feet separation distance waiver.
We also request a 0' lot line waiver from the existing seepage pit to the west lot line. We are unaware of
any adverse impacts that are associated with the granting of this waiver.
