The scope of this project includes the installation of a 2500 gallon Holding Tank to serve the four bedroom dwelling located on the referenced property. The existing 1250 gallon septic tank is to be excavated, pumped, crushed and abandoned in place. The existing trench is to be abandoned in place. 2. Construction shall be in accordance with the approved site plan and design drawings, Municipal permit with any special provisions or conditions, and all applicable State and Municipal Wastewater Disposal Regulations. 3. The contractor shall be responsible for obtaining any necessary underground utility locates. 4. Unless specifically agreed otherwise, the property owner shall be responsible for final grading areas subsequently depressed from soil settling. 5. Contractors installing wastewater disposal systems must be certified by the Municipal Health Department for system installations. Owners installing their own systems must also receive prior approval from the Municipal Health Department. HOLDING TANKINSTALLATION: 1. A holding tank is to be constructed by a certified wastewater holding tank manufacturer. Construction shall include a 6" cleanout for pumping access and watertight manholes. 2. The holding tank shall be sufficiently bedded to prevent settling or shifting of the tank. 3. All standpipes on the holding tank shall extend a minimum of 12 inches above final grade. 4. Holding tanks installed with less than 4' of cover shall be insulated. Page Two Mountain Park Estates Subdivision 92; Lot 19; Block 3 May 13, 1994 5. The holding tank shall be equipped with a high water alarm which registers both visually and audibly inside the dwelling. The alarm shall be positioned to allow at least 450 gallons of storage after the alarm has been activated. 6. Final grading over the holding tank shall be such that a positive slope exists away from the holding tank. MINIMUM MA'£F~c[AL SPECIFICATIONS: 1. Any holding tank proposed for installation must be constructed by a Municipally approved holding tank manufacturer. 2. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical Company Styrofoam HI or equal). 4. Holding tank inlet shall be fitted with watertight couplings (Caulder, Fernco, or equal). INSPECTIONS: Typically there will be a minimum of two (2) inspections required during the installation of the wastewater disposal system. These inspections will occur as follows: 1. The first inspection must be conducted after the tank has been set; lines, cleanouts, standpipes, and insulation are in place and prior to backfilling. Often there will be more than these 2 inspections required, especially with the installation of multiple trenches, sand filters, pressurized distribution systems, etc. Thus, the inspecting engineer is to be contacted at least 24 hours prior to the start of construction. If necessary, a pre-construction meeting will take place on-site. The inspecting engineer will not coordinate, direct or control in any way the contractor's activities. The owner shall contract with the contractor to perform the work outlined in these specifications and plans and in accordance with the attached M.O.A. permit. There will be no contractual arrangement existing between the contractor and S & S Engineering. S & S Engineering shall be the owner's representative and will inspect the work as stated above to document the contractor's activities. Final acceptance of the contractor's work rests with the owner and the M.O.A. S & S Engineering shall have no liability to the owner or to others for acts or omissions of the contractor or any other persons performing work on this project or the failure of the contractor to carry out the work in accordance with these construction documents. S & S Engineering's inspecting engineer will not be responsible for the construction means, methods, techniques, sequence, procedures or the safety precautions incident to this project. Grass, sod, utilities and a fence exist between the leachfield trench and referenced well, therefore, effluent which could daylight from the leachfield would spread out over the ground and would be easily detected before reaching the well. The property owner is relocating to another state and a relocation company will be assuming the property upon his departure. However, a H.A.A. will be required before the relocation company is able to assume the property, and a waiver must be obtained from your department before a H.A.A. can be issued. A waiver of the separation distance down to 93 feet should be granted for the following reasons: Water samples obtained and tested on April 13, 1994, showed nitrates of 0.81 mg/L. This would indicate the well has been not been influenced by the existing encroachments over the past sixteen years. A well flow test on April 18, 1994, found the current static water level at 304 feet below ground level Therefore, surface water or septic effluent must filter through approximately 186 feet of gravel with 30% clay and 118 feet of bedrock before reaching the aquifer. We have reviewed this design with Rolf Strickland of the Municipal Health Department and he has found it acceptible. Following discussions with you and a review of the on-site sewer permit and inspection report and the soils investigation performed by R&M Consultants, Inc., we have concluded that there are at least three possible causes for the failure of the existing absorption field: 1. The existing system may have been overloaded due to a continuous running of water from the Morales residence, 2. Construction of the existing System may not have been in accordance with the design provided by the Municipality. The design of the existing system may have been in error due to an improper correlation between percolation test results and absorption area requirements for design. Without further investigation it is not possible to determine the exact cause or causes of the failure of the existing system. The following procedure was utilized to design the replacement absorption system: 1. "Manual of Septic Tank Practice", Public Health Service Publication No. 526, 1969. 2. Ordinances and regulations of the Municipality of Anchorage for design and installation of on-site sewer systems. At your request, I have evaluated the subsurface condi- tions in the relatively small area available for the additional leach line required to suppIement the failing present system. In evaluating the new absorption field proposed by Mr. Howard Holtan of Quadra Engineering, it was determined that the soils in the area of the proposed system probably would not accept waste water suffi- ciently rapidly for the system as designed. This evaluation was based on the Soils Log and Percolation Test i (see sheet 5 of 8) and the performance of a test pit in the drain field location. The soil conditions were different than upon which the design was based in that apparently there was no or insufficient sand layering of the dense sandy silts typical of the soils. At your request, additional exploration and testing was performed and resulted in only one test hole out of the three additional borings encountering soils sufficient- ly permeable and layered with sand to accept a drainfield. I de- signed a system based on this test hole. However, you are advised there is a substantial risk that the soil conditions may change within the area designated for the proposed system and I may not be able to observe these changes during excavation and that these changes may adversely affect the performance of the system. You have also been informed that in order to minimize the disturbance of the area a test procedure not included within the "Manual of Septic Tank Practice" was used requiring a ~non-standard design procedure. This procedure was submitted for approval to the Municipality of Anchorage Department of Environmental ©~ality upon application for the permit. It has also been discussed with your engineer Mr. Howard Holtan. These factors have been discussed with you on October 23, 1978, and realizing the risks and the alternative involved you have directed me to obtain the required permits if there were no objections to the design procedure, and coordinate ~th Mr. Duane Gorder to install the system. Be also advised that in order to prolong the life of this system, the septic tank should be pumped once a year. I trust the foregoing is comprehensive of our conversations.
