Request you issue a permit to install a 2000 gallon
wastewater holding tank to serve the residence located on
the referenced property.
In April/May, 1992 we tested the well and septic on this
property for Health Authority Approval purposes. We found
the septic system in a state of failure due to groundwater
intrusion (refer to attached letter to Becky Powell-Simpson
Company Realtors dated May 5, 1992). For the reasons listed
in the referenced letter, we feel the best wastewater
disposal option at this time for this property is a holding
tank.

GENERAL:
1.
The scope of this project includes the installation of a
2000 gallon wastewater holding tank system to serve the
three bedroom residence located on the referenced
property. The existing septic tank is to be excavated,
pumped, crushed, and abandoned in place.
Construction shall be in accordance with the approved
site plan and design drawings; Municipal permit with any
special provisions or conditions; and all applicable
State and Municipal Wast·water Disposal Regulations.
The contractor shall be responsible for obtaining any
necessary underground utility locates.
Unless specifically agreed otherwise, the property owner
shall be responsible for final grading areas subsequently
depressed from soil settling. On all leachfield mound
systems, the property owner shall be responsible for
ensuring a satisfactory vegetation growth over the
mounded area.
Contractors installing wastewater disposal systems must
be certified by the Municipal Health Department for
system installations. Owners installing their own
systems must also receive prior approval from the
Municipal Health Department.
HOLDING TANK INSTALLATION:
A holding tank is to be constructed by a certified
wastewater holding tank manufacturer. Construction shall
include a 6" cleanout for pumping access and watertight
manholes.
The holding tank shall be sufficiently bedded to prevent
settling or shifting of the tank.
Ail standpipes on the holding tank shall extend a minimum
of 12 inches above final grade.

Holding tanks installed with less than 4 ft. of cover
shall be insulated.
The holding tank shall be equipped with a high water
alarm which registers both visually and audibly inside
the dwelling. The alarm shall be positioned to allow at
least 450 gallons of storage after the alarm has been
activated.
Final grading over the holding tank shall be such that a
positive slope exists away from the holding tank.
INSPECTIONS:
Typically there will be a minimum of three (3) inspections
required during the installation of the wastewater disposal
system. These inspections will occur as follows:

The first inspection must be conducted after the
excavation of ditches, pits, trenches, or beds and
before the installation of any gravel. A septic
tank may be set in place, but may not be backfilled
before this inspection.
The second inspection must be conducted after the
placement of the silt barrier, gravel, distribution
lines, standpipes, cleanouts, and insulation, but
before the placement of any other backfill.
The final inspection is to occur upon final grading
of the property.
Often there will be more than these 3 inspections required.
Especially with the installation of multiple trenches, sand
filters, pressurized distribution systems, etc. Thus, the
inspecting engineer is to be contacted at least 24 hours prior
to the start of construction. If necessary, a pre-
construction meeting will take place on-site.

Insulation shall be at least 2" thick extruded direct
burial polystyrene (Dow Chemical Company Styrofoam HI or
equal).
Septic tank inlets and outlets shall be fitted with
watertight couplings (Caulder, Fernco, or equal).
A permeable nontoxic silt barrier (Typar 3401, Mirafi
140N, or equal) must be installed between the final
leachfield gravel layer and the native soil backfill.
Ail leachfield gravel (sewer rock) shall be 0.5"-2.5"
screened gravel with less than 3% passing the #200 sieve.
When sand is being used as a filter material, it's
gradation specifications must conform to AMC 15.65.060D.

At your request we have completed a water and s~wer assessment on the
referenced property for the purposes of obtaining a H~alth Authority
Approval.
A flow test was p~rformed on the w~ll serving the referenced property
on May I, 1992. The static water level w~s m~asured in the w~ll at 166
ft. A meter was connected to the water system and the flow turned on
fu~l. The water lev~ was drawn down to the pump at 246 ft. The flow
was atlowed to continue for the duration of the 4 hour test where the
flow rate stabilized at 3.5 gallons per minute (GPM). This flow rate
is not guaranteed to r~ain constant, subsequent variations can occur.
Water samples were taken on April 8, 1992 and tested for coliform
bacteria and nitrates. The resu~s were satisfactory.
Also at your request an inspection w~ performed on the septic system
on the referenced property. On April 9, 1992 the septic tank was
pumped by A+ Home Services. On April 29, 1992 the monitoring tub~s
were located and liquid levels in the septic tank and l~achfi~ld were
measured. The liquid lev~ in the septic tank was measured at 64". and
liquid levels in the l~achfi~ld were m~asured to be in excess of 10 ft.
After the tank was pumped, groundwater saturated the septic syst~ to a
point where water backed into the home through the garage floor drain.
These high liquid levels indicate the existing wast~water d~posal
system is b~ing surcharged by groundwater and thus not ~ligible for a
Health Authority Approval (HAA) Certificate unt~ the system h~ been
upgraded.
Unfortunately, a conventional system cannot be installed on the
referenced property for the following r~asons:
I) The existing system has a history of failure, has been
upgraded twice and there is no available ar~a for another
conventional upgrade.

Municipal files on thc referenced property show the existing
wastewater disposal system had been upgraded twice since it's initial
installation in 1974. The system initially consisted of a 1000 gallon
septic tank and a seepage pit. The first upgrade in 1975 added a 50'
trench to the existing seepage pit. A second upgrade in 1980 added
two more trenches (one at 47' and one at 28') running para~lel to the
existing 50' trench. Due to the relatively small lot size and
previou~ upgrades there is no available area for anoth~ upgrade.

A curtain drain located on the Southeast corner of this lot indicates
a surface water problem, presumably from roadway drainage running
along D~Armon Road.
Therefore, we recommend installing a 2000 gabon holding tank for the purpose
of on-site wnstewater disposal on referenced property.
During our site visit it was noted that a CMP (Corrugated metal pipe) is
located over the septic tank with a plywood lid. We recommend that this be
abandoned or sec~ed close, as it is now presents a safety hazard.
Also, we found the well casing extended o~ly 10" above the ground surface and
in a depressed area. The casing will have to be extended and fi~will have to
be placed around the well head to provide positive drainage away from the well.

for the purpose of exposing the crib for joint inspection by the
contractor and GAAB inspectors.

whatever corrective measures are required to put the system and
surrounding area into a condition as prescribed by the GAAB. It is

This case has an interesting history, A sewer permit was issued, the system
was installed and original approval was on 5-23-74. In April, 1975, we received
complaints from the owner that the system was failing and had to be pumped regularly.
Reinspeetion revealed that the system was originally installed improperly and in a
manner not normally covered by our inspection procedures. In June, 1975,. a new
sewer Permit was issued, the system was upgraded and reinspeeted. We have no
knowledge of problems since that time.
The latest lean approval request was dated 8-3-77, and resulted in the sewer and
water systems being tested for the following reasdns:
The bacterial quality of the water must.be determined.
(2) The sewer system had a history of failure.
(3) The sewer system was over two years old, Our survey of three
hundred sewer systems in 1975 showed 50% failure after only two
years of use.
(4) The soils in this subdivision and on this lot are very poor.for the
continued satisfactory operation of an on-site sewage disposal system.
The results of both sewer and well tests showed both systems were satisfactory and
they were both approved.
The main reason the sewer system ~is tested and our authority for this test is:

The bank, real estate company, etc., asks if the system is still
working and complies with ali codes and;
Municipal Code, Title 15, Ch. 15.65, Section 15.65. 020 A. states
that sewage shall be disposed of only in properly designed,
constructed and maintained sewerage systems, septic tanks or
by other methods approved by the department.
Our test insures that the sewerage system has been adequately maintained and
will still dispose of all wastes generated and as required by ordinance.
~The same reason applies for the bacterial test of the well water. It is not
possible to determine the adequacy of either sewerage or water systems without
these tests and thereby give approval to them.
Our legal authority to charge a fee for this service is covered in .Anchorage
Municipal Code Title 15, Chapter 15.65, Sec. 15.65.120 D.

Subject: Flow Test on Existing Septic System, Lot 13, Block 4, Mountain
Park Estates, Anchorage, Alaska
Dear Mr. Baird:
On August 15, at your request, we conducted a percolation test of the
septic system on the above described lot.
For the test, 200.2 gallons of water were ~dded to the system over a 51-
minute period. The following table represents the level of liquid in
the septic tank.
Ail measurements were taken from the top of the standpipe.

The meter used during the test was a Rockwell 5/8" standard water meter
which had previously been calibrated by R & M Consultants, Inc.
If it is assumed that the existing three bedroom residence on the lot
will house six people, the average load on the disposal system can be.
expected to be 450 gallons per day or .31 gallons per minute.

The system in question accepted 200.2 gallons, or nearly one-half the
daily load, in 51 minutes. During that time the~e was no significant
rise in water level in the septic tank. This indicates an acceptance
rake of at least 3.9 gallons per minute.
Since the house on the property was occupied, we assume that the leach
field was at its normal degree of saturation, and conclude that the
system is performing in an acceptable manner for a three bedroom res-
idence. ~
