The existin9 septic tank and seepage system wiii be obandoned in place.
The four bedroom house wi!! produce 600 9pd ¢ was4ewater. At an application rate ¢ 1.0
~ud/~u, this wiii require a see¢~age bed area of 600 ~2. A seepage bed 24 feet wide by 25 feet
(~00 ~2) is proposed. The seepage bed wiii have four I ~" diameter laterals spaced at 6 feet
installed with pedoration down a~ached to a i ~A" manifold. Based on the pump proposed to be

The subject permit, issued November 18, 1993 to the previous owner of this property by this
office for a single family well and/or on-site wastewater system, has expired as of November 18,
1994.
A new permit must be obtained from this office for a well and/or on-site wastewater system NOT
installed by the expiration date.
If the well was ch'illed, a well log must be sent to this office for documentation of the installation
and to close the permit.
If the ou-site wastewater system has been completed and a licensed Professional Engineer has
inspected the installation of the on-site wastewater system, the original as-built iuspection report
must be sent to this office for review, approval and documentation. All inspection reports must
be submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $320.00 for an on-site wastewater permit; $120.00
for a well permit aud $440.00 for a combined on-site wastewater and well permit.

We request you issue a permit to upgrade the septic system serving the
four bedroom house on the referenced property.
An adequacy test performed on the existing system for Health Authority Approval
purposes found the absorption capacity of the existing system to be inadequate.
A test hole was excavated and a percolation test performed in the area of the
proposed upgrade. The approximate location of the test hole is located on the
attached site plan. At the time of excavation water was encountered at 11 feet
and after seven day water monitoring water was found at 9 feet. Attached it the
proposed upgrade design.
We do not anticipate any adverse effects on neighboring properties by the
installation of the proposed septic system.

GENERAL:

The scope of this project includes the installation of a 1500 gallon septic
tank effluent pumping (S. T.E.P.) system and an absorption bed to serve the
four bedroom residence located on the referenced property. The existing
leachfield, 1250 gallon septic tank, and lift station are to be abandoned in
place.
Construction shall be in accordance with the approved site plan and design
drawings; Municipal permit with any special provisions or conditions; and all
applicable State and Municipal Wastewater Disposal Regulations.
The contractor shaft be responsible for obtaining any necessary
underground utility locates.
Unless specifically agreed otherwise, the property owner shaft be
responsible for final grading areas subsequently depressed from soft
settling. On all leachfield mound systems, the property owner shaft be
responsible for ensuring a satisfactory vegetation growth over the mounded
area.
Contractors installing wastewater disposal systems must be certified by the
Municipal Health Department for system installations. Owners installing
their own systems must also receive prior approval from the Municipal
Health Department.
SEPTIC TANK INSTALLATION:
A septic tank is to be constructed by a certified septic tank manufacturer.
Construction shall include two 4" cleanouts for pumping access.
The septic tank shaft be sufficiently bedded to prevent settling or shifting of
the tank.

Aft standpipes on the septic tank shall extend a minimum of 12 inches
above final grade.
Septic tanks installed with less than 4 ft. of cover shall be insulated.
A foundation cleanout shaft be installed one to four feet from the building
foundation. In the line between the tank and the leachfield there shaft be
two adjacent cleanouts (unless an effluent pumping system exists within the
septic tank). These cleanouts shaft be located on undisturbed soil not more
than 10 ft. from the tank. The first cleanout, in line, shaft be to clean toward
the leachfield. The second cleanout shall be to clean toward the septic
tank.
Final grading over the septic tank shaft be such that a positive slope exists
away from the septic tank.
LEACHFIELD BED INSTALLATION:
Excavate the proposed Bed Area to the depth shown on the design. The
bottom of the excavation shaft be within 2 inches of level. If the bottom of
the excavation becomes smeared, it must be raked or scratched (ruffed-up)
before gravel or sand placement.
If a sand layer is required, place sand over entire excavation to the required
depth shown on the design. The top of the sand layer must be within 2
inches of level.

Sewer rock shaft be placed uniformly throughout the entire bed. Perforated
distribution pipe must be installed level with perforations down. Gravel
depth below the perforated pipe shaft be a minimum of six (6) inches.
Gravel depth above the perforated pipe shaft be a minimum of two (2)
inches. The total gravel depth throughout the entire bed shaft be a
minimum of twelve (12) inches.
The perforated distribution pipes must be no more than six feet apart. The
distance between the outermost perforated distribution pipes and the
sidewall of the absorption bed must be no more than three feet.

Silt barrier material must be installed between the final gravel layer and the
native soil backfill. Ensure the silt barrier covers the entire gravel surface
before placing backfill.
Monitor tubes shaft be of four (4) inch diameter and installed at the
locations shown on the design. The portion of the monitor tube extending
through the gravel depth shaft be perforated six (6) inches below the bottom
of the horizontal distribution lines.
Backfill over the final gravel layer must not be less than twenty-four (24)
inches. Insulation must be installed when the backfill depth is less than
thirty-six (36) inches. The finished grade over the bed must be mounded
to prevent the formation of a depression after settling.

Any septic tank proposed for installation must be constructed by a
Municipally approved septic tank manufacturer.

Use of a type of pipe other than listed above must be approved by the
inspecting engineer.
Insulation shaft be at least 2" thick extruded direct burial polystyrene (Dow
Chemical Company Styrofoam HI or equal).
Septic tank inlets and outlets shaft be fitted with watertight couplings

A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N, or equal) must
be installed between the final leachfield gravel layer and the native soil
backfill.
All leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel with
less than 3% passing the #200 sieve.
When sand is being used as a filter material, it's gradation specifications
must conform to current M.O.A. or D.E.C. requirements.
INSPECTIONS:
Typically there will be a minimum of three (3) inspections required during the
installation of the wastewater disposal system. These inspections will occur as
follows:
The first inspection must be conducted after the excavation of
ditches, pits, trenches, or beds and before the installation of any
gravel. A septic tank may be set in place, but may not be backfilled
before this inspection.
The second inspection must be conducted after the placement of the
silt barrier, gravel, distribution lines, standpipes, cleanouts, and
insulation, but before the placement of any other backfill.
3. The final inspection is to occur upon final grading of the property.
Often there will be more than these 3 inspections required. Especially with the
installation of multiple trenches, sand filters, pressurized distribution systems, etc.
Thus, the inspecting engineer is to be contacted at least24 hours prior to the start
of construction. If necessary, a pre-construction meeting will take place on-site.

is 25 feet and to a community sewer line is 75 feet. Well logs are required
and must be returned to this department within 30 days of the well completion.

As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures
outlined in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or
wastewater disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated
herein. I further verify that based on the information obtained from the Municipality of Anchorage files and from my investigation and
inspection, the on-site water supply and/or wastewater disposal system is (are) in compliance with all applicable Municipal and State
codes, ordinances, and regulations in effect at the time of installation.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MOA
COSA guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time
of the test, and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on
the local soil condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not
guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot
provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole

Nitrate Advisory

A water sample revealed a nitrate concentration of 8.79 milligrams per liter (mg/L).
The Environmental Protection Agency (EPA) has established a maximum
contaminant level (MCL) of 10.0 mg/L for public drinking water systems. While
private wells are not subject to this regulation, EPA standards are based on existing
health information and can therefore be used to gauge the relative quality of water
from private wells. Since nitrates are known to slowly increase, we recommend
you monitor the water quality. Please see the attached “Nitrate Fact Sheet” for
important information regarding nitrate.

ENCROACSMENT PERMIT

administrators and assigns, hereinafter called the "PERMITTEE", and the Municipality of
Anchorage, a municipal corporation organized and existing under its Charter and the laws

WHEREAS, "PERMITTEE" is the owner and/or administrator of the following
described real property:

thereof, on file in the office of the District Recorder, Seward
Meridian, Alaska,

WHEREAS, "PERMITTER" owns and/or maintains the real property more
particularly described as follows:
THE SIXTY FOOT (60) RIGHT OF WAY OF LUPINE ROAD
immediately to the West of the "PERMITTEE'S" property, and
THE TEN FOOT (10) UTILITY EASEMENT comprising the
East ten feet of the "PERMI r=S" property,
and;
WHEREAS, "PERMITTEE" has caused to be placed a lamp pole and a well
which encroach upon the "PERMITTER'S" Lupine Road Right of Way, and a chain link
fence which encroaches upon the "PERMITTER'S" ten foot utility easement;
NOW, THEREFORE, it is mutually agreed between the parties hereto that:
i . The "PERMITTER", acting through the Director of Public
Works, hereby grants to the "PERMITTEE" the privilege of allowing their presently
placed lamp pole and well to encroach upon the "PERMITTER'S Lupine Road Right of
Way distances of two and seven -tenths feet (2.7') and one and two-tenths feet (1.29

2. The "PERMITTEE" agrees forever to indemnify, defend, save
and hold harmless, the Municipality of Anchorage, its officers and employees, from any i
and all lawuits, claims or actions brought by any person for or on account of damage to
property or injury, disease, illness or death of persons, including all costs and expenses
incident thereto, arising wholly or in part from or in connection with the existence of,
alterations, maintenance, repair, renewal, rwwfitructl0n, operation, use or removal of the
encroaching lamp pole and/or well as situated upon the " MMrIER'S" Lupine Road
Right of Way, and/or the encroaclmW chain link fence as situated upon the
"PERMITTER'S" ten foot utility easement as described ribe d herein.
3. The "PERMITTEE" shall not assign or transfer any of the
rights granted herein to another individual or company without first notifying and securing
the approval of the Director of Public Works.
4. This agreement and permit grants the "PERMMEE" no
interest in the "PERI IT R'S" real property whatsoever, except only the encroachment
rights described herein.
5. The "PERMITTER" reserves the right to revoke this permit
upon twenty (20) days written notice to the "PERNUTTEE". The "PERMITTEE" agrees
upon said notice of revocation to move immediately any or all said encroachments from
the Lupine Road Right of Way and/or ten foot utility easement in which the
"PERMITTEES" lamp pole, well, and chain link hence are placed. Should the
"PERMITTEE" refuse or fail to comply with said written notice, the "PERNuriw may,
without fiuther notice to the "PERMITTEE", remove, or cause to be removed, the
encroachment, and the "PERMITTEE" hereby agrees to reimburse the "PERM=R"
for any and all costs incidental to the removal thereof.
In addition to the mutual promises heretofore made, the "PERMITTEE" hereby
promises to pay the "PERMITTER" a one-time permit fee of one hundred dollars
($100.00).
IN WITNESS THEREOF, the parties hereto have hereunto set their hands and
