Park Estates #2; Block 8, Lot 2, To whom it may concern: The existing 3 bedroom house is served by a private well and septic system. The drainfield is in a state of failure and needs to be upgraded. We are proposing to rebuild a portion of the existing drainfield (12' x 23') and install a CAT II Quanics Aerocell treatment system on this property which includes the installation of a new 1500 gallon STEP tank. 1. SOILS & GROUNDWATER LEVELS: See the 1984 soil log (attached) which shows the soil classifications, groundwater monitoring, and the MOA issued permit (attached) discussing the percolation results. We are proposing to remove the contaminated sand below the existing bed and import MOA approved sand, so as to place the top of sand 2' higher in elevation than the bottom of the existing drainrock. We request that your department waive the request for an additional percolation test and 7 -day groundwater monitoring based upon the following justifications: • The drainfield has served the home for almost 30 years. • GEG does not believe that an additional percolation test in the old saturated drainfield will be beneficial. • The existing drainfield was installed 4 feet above groundwater (monitiored in July 1984). We are proposing to install the re -built drainfield an additional 2 feet higher, so as to provide a separation distance of 6 feet to groundwater. • We are proposing to install a CATII Quanics Aerocell system with 4 feet of MOA approved sand. 2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications. 3. SURFACE WATERS: The proposed re -built drainfield will be installed greater then 100' from any surface waters. 4. TOPOGRAPHY: The average topography around the drainfield is relatively flat. In short, there are no slope concerns. Recently a permit was issued for this lot, in which the septic system was to be built as a bed 18 feet by 38 feet, 9 feet deep, with 2 feet of NFS sand placed from 7 to 9 feet (to keep sewer rock 4 feet above the water table at 11 feet). Unfortunately, as you might have guessed, the well drillers had already been on the lot and drilled a well which made an 18 x 38 bed impossible to fit in. We carefully flagged 100 feet radius from the well and made sure the bed was also 100 feet or more from neighboring wells, and proceeded to build the system. As a result, it is a rather unusual shape, and several drawings are attached to illustrate the situation. (See sheets 1 thru 2). More NFS sand than was called for was used, because the natural sand strata dipped down to the west and north (see sheets 3 and 4). The septic tank was placed 5 feet from the north lot line. There was nowhere else on the lot the tank could have been placed that was 5 feet from the bed and 100 feet from the well, which is illustrated on sheet 1. We are therefore requesting a waiver of the septic tank to north lot line to be 5 feet. If you have any further questions please call our office. In consideration of the mutual covenants contained herein, the parties to this Maintenance and Repair Agreement agree as follows: 1. Advanced Wastewater Treatment Systems. Municipality grants permission to Owner to utilize and operate an Advanced Wastewater Treatment System (A W WTS), described as Quanics AerocelI system located at (legal description). Mountain Park Estates #2 L2 B8 2. Definitions. Alteration. Any change to the design or function of an AWWTS that includes the installation or removal of any parts, components or pieces not included in the original construction permit and design. Certificate of On -Site Systems Approval (COSA). An approval by the Municipality of existing water and wastewater disposal systems given at the time of property sale and title transfer in accordance with Anchorage Municipal Code (hereinafter, "AMC") 15.65. These approvals certify that the systems are adequate for the homes that they support and meet the codes that were in place at the time of system construction. Damage. Any man-made or natural change in a system that would inhibit the system from performing as designed. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AWWTS that were included in the original design which would allow the AWWTS to continue to perform as designed. Permit. An On -Site Wastewater Disposal Permit as required by AMC 15.65 to construct and operate an AWWTS. 3. Term. The term of this Maintenance and Repair Agreement shall begin on the date of approval by the Municipality to operate the installed system or issuance of a COSA, and shall continue while the AWWTS is in use or is operational or until the property is sold or title is transferred by the owner and a new COSA is issued to the new owner or transferee of the property. 4. Alterations. Installation and Removal of Additional Equipment. Prior to performing any alterations to an AWWTS, the owner agrees to obtain an On-site Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65. 5. Maintenance and Repairs. A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall maintain their AWWTS in a satisfactory condition capable of producing treated septic effluent in accordance with the equipment's approval for operation in the Municipality. The owner shall enter into a service agreement with an AWWTS service and maintenance provider approved by the municipality and the manufacturer of the AWWTS for the entire term of the AWWTS. In addition, it shall be the responsibility of the Owner during the term of this Maintenance and Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs. B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules and orders for the AWWTS. C. Upon request by the Municipality, the owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system. When a record of maintenance is documented and maintained by the system vendor, the owner agrees to allow the Municipality access to this information. D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be assessed in accordance with AMC 14.60 for improper discharge. E. Owner agrees that only maintenance and repair personnel approved by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. F. Owner agrees to grant the Municipality reasonable access to test and inspect the AWWTS upon 24 hours written notice. G. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On -Site Systems Approval. H. Owner agrees that the AWWTS installation and maintenance requirements as provided by the AWWTS vendor/installer and approved by the Municipality are the governing guidelines for the construction, maintenance and repair of the Owner's AWWTS. 6. Nonwaiver. The failure of either party at any time to enforce a provision of this Maintenance and Repair Agreement shall in no way constitute a waiver of the provisions, nor in any way affect the validity of the Maintenance and Repair Agreement or any part hereof or the right of such party thereafter to enforce each and every provision hereof. 7. Amendment. A. This Maintenance and Repair Agreement shall only be amended, modified or changed by a writing, executed by authorized representatives of the parties, with the same formality that this Maintenance and Repair Agreement was executed with, and such writing shall be attached to this Maintenance and Repair Agreement as an amendment. B. For the purposes of any amendment modification or change to the terms and conditions of this agreement, the only authorized representatives of the parties are: a. Any attempt to amend, modify, or change this contract by either an unauthorized representative or unauthorized means shall be void. 8. Jurisdiction: Choice of Law. The laws of the State of Alaska shall govern the rights and obligations of the parties under this Maintenance and Repair Agreement. 9. Severability. ANCHORAGE, ALASKA 99516 This letter is provided to explain the procedures used in my 9/11/98 adequacy test of the water supply and wastewater disposal systems serving the 3 -bedroom residence on Lot 2, Block 8, Mountain Park Estates, Unit #2, located at 12620 Saunders Road. I discussed these findings with Dan Roth on 9/14/98 and agreed to provide a narrative description in support of the attached HAA request. According to the driller's log, the well was drilled in June of 1985 and has a total depth of 410 feet, of which all but the top 28 feet is in bedrock. The driller's log reports the yield to be 1 '/z gpm. On September 11, 1998 the static water level stood at 45 feet below the top of the casing. Steady pumping of 577 gallons of water from the well at the maximum pump output caused the water level to be gradually drawn down to 396 feet below the top of the casing, at which time the water was shut off. I then monitored the recovery of the water level inside the bore hole as it rose to 265 feet over the next 54 minutes. Based on these measurements, I determined that the yield of the well is 2.4 gpm, and it appears that the borehole diameter is 5 inches in at least the bottom 150 feet of the well. This measured yield is more than adequate to comply with M.O.A. criteria for approval of a 3 -bedroom residence. Water samples collected on September .11,.1998 were satisfactory, showing 0 coliform or other bacteria and 1.34 mg/l of nitrate -N. At the time the septic system was installed (before the house was constructed) the inspecting engineer reported the distance from the well to the septic tank to be 101 feet, and the distance from the well to the soil absorption bed to be 100 feet exactly. I measured the distance from the well to a cast iron standpipe at the end of the soil absorption system across the street on Lot 2, Block 1 to be 107 feet. According to the as -built inspection report, the wastewater disposal system was installed in June of 1985 and consists of a 1000 -gallon septic tank that discharges effluent into an irregularly shaped soil absorption bed with 1007 square feet of effective absorption area. The bed was constructed by placing additional sand on top of a naturally occurring sand stratum to create a level surface four feet above the measured water table. The system was tested by S & S Engineering in March of 1994 and determined to be adequate, and a HAA certificate was issued at that time. S & S's field notes from their test indicate that one of the two monitor tubes was plugged with dirt, and that the depth of the other may exceed the bottom of the bed. At S & S's request, D.E.C. issued a waiver on 5/2/94 allowing the soil absorption system to be 128 feet from the Class "C" well on Lot 3. In February of 1998 S & S retested the system and determined that it was in failure. This determination was based on the fact that 237 gallons of water added to the system caused fluid to back up 7 inches in the cleanout upstream of the bed. On June 10, 1998 the owners arranged with Alaska Drainfield Restoration, Inc. to "Terra -lift" the soil absorption bed. My adequacy test (conducted on September 11, 1998) provided the required follow- up test of the system after 90 days. As I discussed with Dan Roth, the more southerly of the two monitor tubes in the bed was plugged with dirt at 50 inches below ground level, whereas I was able to push a probe 86 inches below ground level in the more northerly monitor tube. There was no free liquid in the tube, however the bottom 18 inches consisted of gray, viscous mud. This mud appeared to be predominantly mineral in nature, rather than organic septic slime. Thus, I would have to agree with S & S's hypothesis that the depth of this monitor tube may exceed the bottom of the bed. The cleanout preceding the bed is located nearby, and it was dry. Based on these observations I concluded that the bed was essentially dry at the start of the test. According to the owner, the residence had been occupied continuously since the "Terra -lift" treatment was done. To assess the adequacy of the system I ran a total of 577 gallons of water into it through the second compartment of the septic tank, while monitoring fluid levels in the septic tank, in the cleanout, and in the soil absorption bed monitor tube. The addition of this amount of water (which is in excess of the total daily design demand) caused no measurable change in the fluid levels in any of the cleanouts or monitor tubes. The cleanout immediately upstream of the bed remained dry and no fluid built up in the monitor tube. The appearance and consistency of the 18 inches of mud in the bottom of the monitor tube remained unchanged throughout the duration of the test. Based on this demonstrated surge capacity while the house was in an occupied condition (and after discussing the performance with Dan Roth in your office) I concluded that the system soil absorption bed is now able to absorb water at an adequate rate for a three-bedroom residence. In conducting an adequacy test I attempt to provide a thorough, conscientious analysis of the system. In reporting the results I emphasize that they describe the performance of the systems under the conditions encountered at the time of the test, and that separation distances are measured to readily identifiable features. I point out that all systems eventually fail and that satisfactory test results do not guarantee future performance of the system under different conditions, nor do they guarantee that there are no hidden defects or encroachments. Furthermore, the long term efficacy of drainfield restoration techniques, such as "Terra -lift" is unknown. Please feel free to give me a call if you have any questions on this report. Thus, it appears that there is existing evidence in support of the requested waiver. Therefore, in accordance with the provisions of 18 AAC. 80.030, the following separation distance have been granted: Between the existing Class C Public Water System Source Well and Soil Absorption System, from 150 feet to approximately 128 feet. This waiver will remain valid until the soil absorption system on Lot 2, Block 8 Mountain Park Estates #2 requires upgrading or is modified. However, based on the increasing levels of contamination being found in the ground water, either the installation of additional treatment or a holding tank may be the only alternatives the Department will consider if the existing soil absorption system fails.
