General
1. From the water sample, total coliform was 0, and total nitrates was 7.11 mg/L
2. Any overflowing sewage would move toward the well head but be diverted by a ditch at the
road.
3. The location of the system is in a visible area and any surfacing effluent will be seen and
corrected.
4. Data from lot 6 well used because it is more consistant with wells in the area.

Ref: Waiver Request from Well to Absorption field on Neighboring Property: Mountain Park Estates
#2; Block 8, Lot 5
The existing 4 bedroom house is served by a private well and septic system. During our site investigation
work to obtain a COSA, we discovered that the well on the subject property is only 95 feet away from the
absorption field on Mountain Park Estates #2; block 1, lot 5. We request you grant a 95 foot separation
distance waiver from the well on the referenced property to the absorption field on Mountain Park Estates
42; Block 1, Lot 5. The following items are justification for the waiver:
• There is a road with ditch lines separating the well on the subject property and the neighboring septic
system. If the septic system was to overflow, it would have to travel through two road ditches and
cross a road surface, which would make it virtually impossible to reach the subject properties well —
head.
• The location of the neighboring septic system is in a very visible area so that if any effluent was to
subsurface, it would be noticed.
• We are asking for a variance of only 5%.
• The existing encroachment has existed for 25 years.
• Attached are well logs from surrounding properties. All of the logs indicate there is at least 3040 feet
of soil overlying bedrock.
Based upon the aforementioned facts, it appears that there is minimal risk associated with the 95 foot
separation distance waiver.
The well that serves the subject property was drilled pre October 4, 1971, and the septic system that
serves Lot 5, Block 1, Mountain Park Estates #2 was installed on 12/5/1983. It is our understanding that
the property owner of Lot 5, Block 1, Mountain Park Estates #2 is responsible for the waiver fee. We also
request that yoyr department issue a Certificate of On -Site Systems Approval.

Ref: Proposed Septic System Upgrade for Mountain Park; Lot 5, Block 8,
To whom it may concern:
The existing 4 bedroom house is served by a private well and septic system. The septic system is in a state
of failure and needs to be upgraded. We are proposing to install a 1250 gallon septic tank and dual deep
trench type drainfields. One test hole was excavated on the property. The drainfield will be designed
around the 30 foot radius of this test hole. Comments regarding the design are summarized as follows:
1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic system.
4. TOPOGRAPHY: As can be seen on the attached design drawing the average topography below the
drainfield is a 1-5 percent slope running approximately northeast to southwest.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems.

The gravel depth is the minimum depth of gravel between the outfall pipe and

installation inspections of any wells adjacent to this property and the number
of residences that the well will serve.

Backfilling of any system without final inspection and approval by this department
will be subject to prosecution.
Minimum distance between a well and any on-site sewage disposal system is 100 feet
for a private well or 150 to 200 feet from a public well depending upon the type
of public well. Minimum distance from a private well to a private sewer line
is 25 feet and to a community sewer line is 75 feet. Well logs are required
and must be returned to this department within 30 days of the well completion.
Other requirements may apply. Specifications and construction diagrams are
available to insure proper installation.

In conducting this evaluation, GEG provided an engineering evaluation of the well and/or septic system
in accordance with the guidelines and regulations established by the Municipality of Anchorage and
industry practices. The reported results describe the condition of the system/s on the date/s of the
evaluation. Separation distances were measured to readily identifiable features. Hidden defects or
encroachments may exist that were not identified during the evaluation. The operational life of all wells
and septic systems depend upon a variety of variables, including but not limited to, soil conditions
groundwater levels (that may fluctuate during the year), quality of construction (materials ane
workmanship), and the water usage of the family utilizing the system/s. These conditions can vary, and
are outside the control of GEG. Satisfactory test results do not guarantee future performance of the
system/s; therefore, GEG makes no warranty (express or implied) regarding the future performance of
the well or septic system. GEG makes no representation whether an alternative well or septic system
can be installed on the property in the event either of the current systems fail to perform adequately in

Nitrate is a negatively charged compound of nitrogen and oxygen, which is very soluble in water. Nitrate
is not readily filtered or otherwise removed in the soil and can pass rapidly into ground water wells.
SOURCE: Nitrate is a major component of fertilizer and wastewater. Often the nitrate is in the form of
ammonia or protein first, which through contact with oxygen and certain bacteria, converts to the
oxidized form known as nitrate. Sources of nitrate from wastewater include urea, ammonia cleaners,
food solids, and bacterial cells. It may also result from the breakdown of organic matter buried in the soil.
TOXICITY: Nitrate is generally not toxic to adults or children over the age of two or three years, but is
associated with a potentially fatal infant disease called methemoglobinemia. In the digestive system of
young children, nitrate converts to nitrite, which can pass through the intestinal wall into the blood
stream. There it combines with the hemoglobin and interferes with the ability of the blood to carry
oxygen. For this reason, methemoglobinemia is referred to as "blue baby" disease. The EPA limits the
concentration of nitrate in public drinking water supplies to 10 mg/L. The standard has been lowered
from a previous level of 45 mg/L set by the US Public Health Service and the World Health Organization.
TREATMENT: due to its solubility in water and negative ionic charge, filtration and other common home
water treatment systems such as softening or iron filtration does not readily remove nitrate. The best
method for limiting nitrate in well water is source control. This can include avoiding overdosing of
fertilizer near the well and maintaining good separation distances between septic tank leach fields and
the well. A special anion exchange filter that contains a media with a strong affinity for negatively charged
ions in water, or by a reverse osmosis treatment system or distillation can remove nitrate.
TESTING: Nitrate analysis is usually done by one of the several "wet chemical" methods using a
spectrophotometer to read the final color endpoint. Specific ion electrodes also can be used to detect
the activity of nitrate in water. This laboratory uses several different wet chemical methods approved
under the public water supply laboratory certification program. They also have test kits available, which
the laboratory uses to perform an inexpensive "screening test", and with which the homeowner can
monitor the change in nitrate levels from their well. They recommend comparing the test kit results
against a certified analysis from the lab occasionally to verify the accuracy of the kit. We recommend
using a specially prepared bottle that has been rinsed in hydrochloric acid for collecting samples.

The Right of Way Division has reviewed a request for a letter of nonobjection to an existing well casing,
which encroaches 3.0 inches into the 10.0 foot Utility Easement. On the as -built survey dated June 17,
2008, submitted with the request, the petitioner has shown the well casing.
This letter of nonobjection is issued with stipulations, and by using it the petitioner is agreeing to the
following:

1. Easement. Chugach is the grantee or user of an casement for the construction and
maintenance of electrical facilitics, as follows:
Dedicated by the plat of the subdivision known as Mountain Park Estates Subdivision No.
2, according to Plat Number P-561, recordcd on the 31"day of October 1960, on file in the
office of the District Recorder, Anchorage Recording District, Seward Meridian, Alaska,
Described in that certain written instrument recorded on the 200' day of January 1953, at
Volume 84, Page 298, on file in the office of the District Recorder, Anchorage Recording
District, Seward Meridian, Alaska,
herein "Ememcul") which pertains to the following described real property:
The South Ten Feet (S 10') of Lot Five (5), Block Eight (8), Mountain Park Estates
Subdivision No. 2, according to Plat Number P-561, recorded on the 31" day of October
1960, on file in the office of the District Recorder, Anchorage Recording District, Seward
Meridian, Alaska.
Permittee acknowledges the validity of the Easement. Permittee warrants and represents that
Permittee is the owner of the fee simple interest in the land subject to the Easement and the
following described real property to which the Easement is adjacent:
Lot Five (5), Block Eight (8), Mountain Park Estates Subdivision No. 2, according to Plat
Number P-561, recorded on the 31" day of October 1960 on file in the office of the District
Recorder, Anchorage Recording District, Seward Meridian, Alaska,

2. Encroachment. Permittee has constructed or purchased the following improvements
on or within the real property subject to said Easement:
A water well that encroaches approximately one Foot (l') into said Easement for a
distance of approximately One Foot (1'),
(herein "Encroachment").
3. Permit. Chugach hereby authorizes Permittee to continue the existence of the
Encroachment in its existing location subject to the covenants set forth in this permit.
4. Indemnification. Permittee shall indemnify and hold Chugach harmless from any
claims for personal injury, property damage or other loss arising in anyway from the continuation of
the Encroachment.
S. No ExpanQion or Increase. Permittee shall not in any way expand or increase the
extent to which the Encroachment occupies any of the real property encumbered by the Easement
without the prior written consent of Chugach. Chugach has no obligation to provide such consent.
6. No r t in Real Estate Acquired. Permittee hereby acknowledges that the
Encroachment and any additions thereto, even if in violation of this permit, has not and shall not in
the future give rise to any interest or estate in the real property subject to the Easement or any other
real property. Without in any way limiting the foregoing, Permittee acknowledges that the
Encroachment is not adverse to any interest of Chugach and its continuation is entirely with the
permission of Chugach for purposes of the common law doctrine of adverse possession. This permit
creates no interest in the real property subject to the easement.
7 Modif'rattnn of Ujility Facilities. If the electric utility facilities located within the
Easement are upgraded, added to, replaced or reconstructed, Permittee shall pay that portion of
Chugach's total costs for such modification which is reasonably attributable to accommodating or
preserving the Encroachment. If such payment is not made within thirty (30) days of written demand
for same, Chugach may terminate this permit in accordance with paragraph 9, below.
8. Covenants Running with the Land• The obligations of the Permittee arising fiornthis
permit shal be covenants running with the land which shall burden the Parcel and the Permittee's
interest in the land subject to the Easement and shall benefit the Easement.
9. JgEninaf oar . Chugach may terminate this permit after the giving of thirty (30) days
written notice of such termination if.
a. The Encroachment is enlarged, increased or extended within the real property
affected by the Easement.

b. The Encroachment is damaged by any means to an extent of more than thirty percent
(30%) of its replacement cost at the time of destruction.

10. I=, This permit shall, if not sooner terminated by Chugach, expire forty (40) years
from the date hereof.
11. Entire Aerecment. This written permit constitutes the entire agreement between the
parties with respect to the subject matter hereof and supersedes all other prior or contemporaneous
agreements, oral or written, between the parties. No modifications, amendments, deletions, additions
or alterations of the permit shall be effective unless in writing and signed by all of the parties hereto.

Dear Russell R Barbara,
Subject to your agreement to indemnify the company as set forth below, GCI Cable Inc.
of Alaska has no objection to the well casing encroaching into the easement by 3" of 6" at
Lot 5 Block 8 Mountain Park Estates 42 (known as 12720 Saunders Rd) located in City
Grid 2839.
This letter of non -objection in no way precludes GCI Cable from full use and enjoyment
of any rights it may have within any portion of the utility easement and or the right-of-
way, including unlimited access for servicing its facilities. Also any additional and
extraordinary costs incurred during any future required construction, repair or
reconstruction of GCI's facilities to accommodate any or all of the encroachments shall
be paid by the property owner.
By signing below, you agree to indemnify and hold GCI Cable harmless, now and
forever, for any damage, costs, expense (including reasonable attorney's fees), liabilities
and injury to any person or property occurring as a result of the encroachment.

ACS has no objection to the V encroachment of a well system into the platted easement along the
south property line of Lot 5, Block 8, in the Mountain Park Estates Number 2 Subdivision also
known as 12720 Saunders Road as depicted on the as -built drawings submitted.
Acceptance and use of this letter of non -objection by yourself, your heirs, your assigns, or your
successors, will constitute agreement to the following stipulations:
1. ACS will be held harmless, now and forever, for any damages or injury to any
person or property as a result of this encroachment.
2. Any ACS facility damaged or destroyed as a result of this encroachment will be
repaired at no cost to ACS.
3, Any costs incurred by ACS for special construction necessitated by this
encroachment will be bome by the property owner.
4. All applicable safety code regulations will be observed and maintained.
5. This letter of non -objection will in no way preclude ACS from full use and
enjoyment of its rights within any portion of Its right-of-way.
