Reference: Septic System Upgrade for Northwoods S/D; Block 3, Lot 15
To whom it may concern:
The 3 bedroom house on the referenced property is served by a AWWU water and a private
septic system. Per MOA records, the septic system serving the subject property was installed in
1981 and consists of a 1000 gallon septic tank and a deep trench type drainfield (72 feet long x
3 feet wide with 6 feet of effective). The drainfield is currently in a state of failure and needs to
be upgraded. A test hole has been excavated on the property. We are proposing that a new
1060 gallon Infiltrator 2-compartment plastic septic tank and new dual 5-wide type drainfields be
installed. The proposed drainfield has been design around the 30 foot radius of this test hole.
Comments regarding the septic design are summarized as follows:
1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring,
and the percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: No surface waters were observed within 100 feet of the proposed
septic system.
4. TOPOGRAPHY: The general topography in this area is slope generally running from South
to North. Attached is a copy of the MOA Argis topography map of the area for your review.
There are no steep slopes, cutbanks, ditchlines, etc., within 50 feet of the proposed drainfield. In
short, there are no slope concerns.

investigation, based on procedures outlined in the Certificate of On-Site Systems Approval Guidelines for this application
shows that the on-site water supply and/or wastewater disposal system is(are) safe, functional and adequate
