Capstone Electric has installed and completed the electrical portion of the work
located at 22607 Northwoods Dr. Chu2iak. AK 99567. All wiring and installation of electrical
components were installed in accordance with the latest electrical codes and were found to be in
proper working order after final testing. All alarms are functioning and installed in the proper
locations. Because this site is outside the MUNI inspection area please accept this letter as final
inspection for the work that has been done.

Tom and Sam,
According to Mike w/ -A+, the on/off float is set at 28" below the top of the basket and the high water alarm is set at 23"
below the top of the basket. According to you, the basket is 64" tall. This would make the floats at 36" from the bottom
of the basket/tank for the On/Off float and 41" from the bottom of the basket/tank for the, High Water float. Assuming a
2" swing up and down for the float, the "Off' level would be 34", the "On" level would be 38", and the "High Level"
would be 43". We need to know what the tank liquid capacity would be with the floats set at these levels or if they need
to be adjusted to make sure the minimum tank liquid capacity Is met for this installation.

Pressure Ratings:
All pressure ratings are a maximum PSI @ 73.4°F.
If temperatures exceed 80°F, contact Charter Plastics for a working pressure de -rating.
Joining:
Charter Plastics Black SIDR pipe is made to ASTM D2239 Standards for inside diameter controlled pipe. It should be
joined with barbed insert fittings and clamps. Double clamping is recommended on 1-1/2" and 2" pipe in all pressures
and on all PC 200 or greater pipes.
You may also use special OD Compression fittings that are designed for ASTM D2239 pipe sizes. Charter Plastics
recommends roughing the end of the pipe with sandpaper prior to sliding on the fitting.
Never use any lubricant on the pipe. Do not expose the pipe to direct flame.
Installing:
All Charter Plastics SIDR Black Potable Water Pipe can be used for down the well applications. For underground
installations, the pipe can be direct buried, plowed or pulled. Buried pipe must be supported by proper embedment
material like sand or gravel. Refer to PPIs "Handbook of Polyethylene Pipe" and follow as local, state or federal
guidelines.
To safely handle and store polyethylene, refer to PPI's Material Handling Guide.
*This pipe is not designed for use inside the building or for hot water applications.
Disinfection:
New water mains and service lines should be disinfected according to AWWA C651.The disinfection should take place
after the initial flushing and pressure testing. Prolonged exposure or concentrated levels of disinfection chemicals
may cause damage to the inside diameter of the pipe.The disinfection chemicals should never contain more than 12%
active chlorine. Charter Plastics recommends the test duration not exceed 24 hours and that upon completion, the
system be thoroughly flushed with fresh water.

All pipe should be hydrostatically tested after installation.
Pneumatic testing is not recommended. Polyethylene. Pipe. Partners. O

FOR MUNICIPAL AND INDUSTRIAL WATER AND SEWER PIPING SYSTEMS
RICH HISTORY
Thanks to the support of our loyal customers, and the
dedication and hard work from our JM Eagle'" family mem-
bers, JM Eagle7' continues to be the largest manufacturer
of PVC pipe in the United States and in the world.
JM Eagle- was formed in 1982 following the purchase of
eight PVC pipe manufacturing facilities from the Johns -
Manville Corporation. Over the course of the years, we
have expanded our operations through seventeen addi-
tional plants in Wharton, TX; Fontana, CA; Batchelor, LA;
Meadville, PA; Adel, GA; Buchannon, WV, Columbia, MO,
Cameron Park, CA, Hastings, NE, Perris, CA, Sunnyside,
WA, Tacoma, WA, Visalia, CA, and now with the addition
of three Polyethylene (PE) facilities in Kingman, AZ,
Magnolia, AR and Tulsa, OK.
There are only a few pipe manufacturers in the world that
produce both PVC and PE pipe, and we are proud to be
one of them. With our modern PE pipe production facilities
in North America, we are able to fulfill all of our customers'
plastic pipe needs.
DESCRIPTION
JM Eagle- manufactures High -Density Polyethylene
(HDPE) water pressure pipe for municipal and industrial
water transmission systems. This pipe is available for sizes
Y2"-63" in diameter. Our HDPE pipe is recognized in the
industry for its zero leak rate, high performance, and long

life expectancy. They may be manufactured with color
striping to identify their application, such as blue stripe for
potable water, green stripe for sewer application, and
purple stripe for reclaimed water.
LONG LAYING LENGTHS
The standard laying length of HDPE pressure water pipe is
40/50 feet. Smaller size pipe 6" and under can be extrud-
ed at continuous coil lengths by request. Longer lengths
provide convenience in installation and allow for significant
cost savings in labor and equipment.
APPLICABLE SPECIFICATIONS
JM Eagle- HDPE water pressure pipe is manufactured
in accordance with AWWA C901/C906, ASTM D2239,
ASTM D2737, ASTM D3035, ASTM F714 and •ANSI/NSF
14/61 listed.
QUALITY CONTROL
JM Eagle"' takes great pride in the quality and workman-
ship of all of our products. JM Eaglee' quality control pro-
grams encompass three critical aspects of the manufac-
turing process: the incoming raw material, pipe produc-
tion, and finished goods. Incoming material is visually
inspected and tested to ensure the material meets all
applicable requirements before its release for production.
During production, the pipe will be visually examined for
any cosmetic defect and pipe samples will be collected for
physical verification and testing for compliance. The fin-
ished product is subjected to further visual inspection to
ensure it has met all the appropriate specifications and
packaging requirements. Without exception, JM Eagle'"
pipe is constantly monitored throughout the entire manu-
facturing process to ensure that it is produced in accor-
dance with all applicable specifications. The JM Eagle -
Quality Management System is ISO 9001:2000 registered.

CORROSION RESISTANCE
HDPE pipe has excellent corrosion resistance against
electrolytic or galvanic corrosion or any known corrosive
soil or water condition. You don't have to worry about
tuberculation, or the need for costly lining, wrapping, coat-
ing or cathodic protection.
FLOW CAPACITY
JM Eagle- polyethylene pipe has a smooth interior provid-
ing less drag and a lower tendency for turbulence at high
flow. Its superior flow characteristic eliminates scaling and
pitting to preserve its form for consistent flow throughout
the pipe's entire service life. The C Factor is 150 and the
Manning's flow coefficient is .009.
REDUCED INSTALLATION COST
HDPE is lightweight, flexible, and comes in long lengths,
allowing for easier and convenient installation. Because of
its flexible nature, it reduces the use of fittings. HDPE also
allows for non -intrusive installation by offering technology
such as directional boring, pipe bursting and slip lining.

To whom it may concern:
The referenced property is served by public water and a private septic system. We have been retained
by CRW Engineering Group (contract engineers on this project for the Alaska Native Tribal Health
Consortium), to evaluate the septic system on this property and prepare a design package for
necessary upgrades/repairs. The existing septic system consists of a 1,250-gallon steel STEP tank
and dual 5-wide pressurized trench type drainfields that were installed in 1994. We recently performed
a septic adequacy test and determined the absorption capacity to exceed 450 gpd.
Due to the age of the STEP tank (almost 25 years old) and our field observation that the tank looked
badly corroded, we are proposing to replace the tank with a new one. Attached is a design drawing
showing the proposed location. We are proposing that the old STEP tank be completely
decommissioned per UPC and two new Infiltrator tanks be installed, an IM-1060 2-compartment
Infiltrator tank and IM-540 Infiltrator pump tank. Based upon our site evaluation, the soil cover over the
new Infiltrator tank/s will be very close to four (4) feet deep. We are proposing the contractor expose
the sewer line where it exits the house to determine if proposed Infiltrator plastic tanks can be installed
without exceeding 4 feet of soil cover. If it is determined that the soil cover will exceed 4 feet, then the
tanks shall be installed in accordance with the attached detail drawing "Infiltrator IM-Tank Installation
Requirements for Increased Backfill Depth—6 FT Max. —Alaska Only".
We believe that the pressurized line from the existing STEP is compromised to some extent; therefore,
we are proposing to install a new pressure line from the STEP tank to the existing trenches. We are
also calling for flushing valves to be installed at the end of each pressurized line (in the trenches), so
that the lines can be properly flushed/cleaned on an as needed basis.
We are unaware any adverse impacts this installation would have on adjacent wells or septic

REFERENCE: Lot 36; Block 3; Northwoods Subdivision
Request you issue a permit to replace the septic system serving the
three bedroomhouseonthe referenced property.
A test hole was excavated and a percolation test performed. The
approximate location of the test hole is located on the attached site
plan. The monitoring tube within the test hole has been checked and
found to be dry. Attached is the proposed upgrade design.
This property is served by a Con~uunity Water System.
We do not anticipate any adverse effects on neighboring properties by
the installation of the proposed septic system.
There are no protective well radii which encroach upon the property.
During an inspection of the existing septic system, effluent was found
to be surfacing in the area.

REFERENCE: Lot 36: Block 3; Northwoods Subdivision
GENERAL:
1. The scope of this project includes the installation
of two five foot wide drainfields and a 1250 gallon
septic tank effluent pumping (S.T.E.P.) system to
serve the existing three bedroom residence on the
referenced property. The existing pressure
distribution bed is to be abandoned in place. The
existing 1000 gallon septic tank and lift station
are to be excavated, pumped, crushed and abandoned
in place.
2. Construction shall be in accordance with the
approved site plan and design drawings, Municipal
permit with any special provisions or conditions,
and all applicable State and Municipal Wastewater
Disposal Regulations.
3. The contractor shall be responsible for obtaining
any necessary underground utility locates.
4. Unless specifically agreed otherwise, the property
owner shall be responsible for final grading areas
subsequently depressed from soil settling. On all
leachfield mound systems, the property owner shall
be responsible for ensuring a satisfactory
vegetation growth over the mounded area.
5. Contractors installing wastewater disposal systems
must be certified by the Municipal Health Department
for system installations. Owners installing their
own systems must also receive prior approval from
the Municipal Health Department.
SEPTIC TANK INSTALLATION:
1. A septic tank is to be constructed by a certified
septic tank manufacturer. Construction shall
include two 4" cleanouts for pumping access.
2. The septic tank shall be sufficiently bedded to
prevent settling or shifting of the tank.

3. Ail standpipes on the septic tank shall extend a minimum of
12 inches above final grade.
4. Septic tanks installed with less than 4' of cover shall be
insulated.
5. Final grading over the septic tank shall be such that a
positive slope exists away from the septic tank.
ABSORPTION TRENCH/DRAINFIELD INSTALLATION:
i. Excavate the proposed trench to the dimensions shown on the
design. The bottom of the excavation shall be within 2
inches of level. If the sidewalls of the excavation become
smeared, they must be raked or scratched (roughed-up)
before gravel (sewer rock) placement.
2. Once the gravel is installed, the distribution pipe is to
be installed level with the perforations faced downward.
Gravel is then to be placed over the distribution pipe to
provide a minimum of 2 inches of cover over the pipe.
3. A silt barrier must be installed between the final gravel
layer and the native soil backfill. Ensure the silt
barrier covers the entire gravel surface before placing
backfill.
4. Monitor tubes shall be of four (4) inch diameter and
installed approximately in the locations shown on the
design. The portion of the monitoring tube extending
through the gravel shall be perforated from the bottom of
the trench to the invert of the distribution pipe. This is
equivalent to the effective depth of the gravel as noted on
the design.

5. Backfill over the final gravel layer must not be less than
twenty-four (24) inches. Insulation must be installed when
the backfill depth is less than thirty-six (36) inches.
The finish grade over the trench must be mounded to prevent
the formation of a depression after settling.
MINIMUM MATERIAL SPECIFICATIONS:
1. Any septic tank proposed for installation must be
constructed by a Municipally approved septic tank
manufacturer.

3. Insulation shall be at least 2" thick extruded direct
burial polystyrene (Dow Chemical Company Styrofoam HI or
equal).
4. Septic tank inlets and outlets shall be fitted with
watertight couplings (Caulder, Fernco, or equal).
5. A permeable nontoxic silt barrier (Typar 3401,
140/N, or equal) must be installed between the
leachfield gravel layer and the native soil backfill.

6. Ail leachfield gravel (sewer rock) shall be 0.5"-2.5"
screened gravel with less than 3% passing the #200 sieve.
7. When sand is being used as a filter material, its gradation
specifications must conform to current M.O.A. or D.E.C.
requirements.

INSPECTIONS:
Typically there will be a minimum of three (3) inspections
required during the installation of the wastewater disposal
system. These inspections will occur as follows:
1. The first inspection must be conducted after the excavation
of ditches, pits, trenches, or beds and before the
installation of any gravel. A septic tank may be set in
place, but may not be backfilled before this inspection.
2. The second inspection must be conducted after the placement
of the silt barrier, gravel, distribution lines,
standpipes, cleanouts, and insulation, but before the
placement of any other backfill.
3. The final inspection is to occur upon final grading of the
property.
Often there will be more than these 3 inspections required,
especially with the installation of multiple trenches, sand
filters, pressurized distribution systems, etc. Thus, the
inspecting engineer is to be contacted at least 24 hours prior to
the start of construction. If necessary, a pre-construction
meeting will take place on-site. The inspecting engineer will not
coordinate, direct or control in any way the contractor's
activities.
The owner shall contract with the contractor to perform the work
outlined in these specifications and plans and in accordance with
the attached M.O.A. permit. There will be no contractual
arrangement existing between the contractor and S & S Engineering.
S & S Engineering shall be the owner's representative and will
inspect the work as stated above to document the contractor's
activities. Final acceptance of the contractor's work rests with
the owner and the M.O.A.

S & S Engineering shall have no liability to the owner or to
others for acts or omissions of the contractor or any other
persons performing work on this project or the failure of the
contractor to carry out the work in accordance with these
construction documents. S & S Engineering's inspecting engineer
will not be responsible for the construction means, methods,
techniques, sequence, procedures or the safety precautions
incident to this project.

has flooded with subsurface ground water. This condition will
cause back up and is not an approved dry absorption field.
The absorption system will have to be rebuilt in an elevated
mound manner designed by a professional engineer.
The residence may be occupied in the interium of the septic
ta~k is capped and a pumping routine guaranteed if you so
desire.

The length dimension is the length(in feet) of the trench or drainfield. The
depth of a trench or pit is the distance between the surface of the ground and
the bottom of the excavation(in feet), There is no set width for trenches.
The gravel depth is the minim~ depth of gravel between the outfall pipe and
the bottom of the excavation(in feet).
Permit applicant has the responsibility to inform this department during the
installation inspections of any wells adjacent to this property and the number
of residences that the well will serve,

Backfillin9 of any system without final inspection and approval by this department
will be subject to prosecution.
Minimum distance between a well and any on-site sewage disposal system is 100 feet
for a private well or 150 to 200 feet from a public well depending upon the type
of public well. Minimum distance from a private well to a private sewer line
is 25 feet and to a community sewer line is 75 feet. Well logs are required
and must be returned to this department within 30 days of the well completion.
Other requirements may apply. Specifications and construction diagrams are
available to insure proper installation.
