A new permit must be obtained from this office for a well and/or on-site wastewater system not installed by the expiration date. If you have drilled the well, a well log needs to be sent to this office for documentation of the installation and to close the permit. If a private engineer inspected the installation of the on-site wastewater system, the original as-built inspection report (three-part form) must be sent to this office for review, approval and documentation. All inspection reports must be submitted within 30 days of construction completion. When applying for a new permit, the fees are: $90.00 for an on-site wastewater permit; $50.00 for a well permit; $140.00 for a combined on-site wastewater and well permit. Due to the presence of a shallow water table overlain by gravel, a two foot thick layer of medium sand is to be placed under the sewer gravel to slow down the percolation rate and achieve a soil rating of 125 sq. ft./bdrm. The size of the soil absorption bed is 125 x 3 × 1.5 = 562.5 sq. ft., which is achieved by a 23' x 25' bed, 2. The project is to be constructed as shc)wn on the site plan and design drawings, except that minor modifications may be allowed or required by the engineer conducting the inspections. 3. All construction practices and material specifications shall conform with M.O.A. requirements. 4. The waste line from the residence to the septic tank shall be 4" dia non-perf, pipe (ASTM D-3034) and shall llave a slope of 0.25 in./ft. 5. The septic tank shall have 1250 gallon capacity, and have a Municipally approved "Orenco" or equal lift station installed in the second compartment. An insulated, watertight manhole riser shall extend above ground surface giving access to the lift static)n components for servicing. 6. The discharge line from the lift station to the soil absorption bed and the distributic)n manifold shall be 2" dia. schedule 40 PVC. 7. The soil absorption bed shall be constructed by excavating the required 23' x 25' bed area to a depth of 6 feet lower than the ground surface elevation in the vicinity of test hole t) i. Two feet of medium sand shall then be placed in the e~cavation to bring the grade back up to a level surface 4 feet below the original ground elevation. 0,75 feet of approved sewer gravel shall then be placed on top of the sand, with the manifold and distribution laterals buried so that their inverts are no less than 0,5' above the top of the sand, Filter fabric shall be placed over tile top of the sewer gravel, prior to final backfill with no less than 3' of soil cover material, Monitor tubes shall be installed in the locations shown, and the final surface graded to drain away from the soil absorption bed. 8, The distribution pipe hole size and spacing is based on t.he lift station pump output of 34 gpm at 20 feet of head. Since each 3/16" hole will pass 0.6 gpm at 2 feet of residual head and 0.9 gpm at 5 feet, the holes are to be spaced at. 30" on center and oriented downwards to achieve an even, low pressure distribution, The ends of the distribut, ion laterals are to be caDDed, and all PVC pi~e joints are to be glued. 9. A total of five inspections will be required during construction: t 1 ) initial stakeout, ~2) ~fter the bed is e~caYated, but before placement of sand, (3~ after tl}e sand Js placed, but before placement of the sewer gravel, (4} after the septic tank is installed, the seWer gravel is placed, the distribution piping is laid and connected up to the lift station, but before final backfill, and (5) after final backfill. .~lattop Technfca!
