Health & Human 8ervic~,s SUBJECT: Lot 4, Block 1, Rabbit Creek Subdivision No. 2 Holding Tank Construction and As Built On July 9, 1997, I discussed the status of the septic system on Lot 4, Block 1, Rabbit Creek Subdivision No. 2 which was permitted for construction in November of 1993 with the owner of the property, David Piaskowski. He informed me that the high level alarm required in the holding tank had not yet been installed. He did say, however, that he was using the holding tank on an irregular basis as he only lives in the house infrequently. He was surprised to hear that a complaint had been filed against the use of his septic system. He stated power was available to the house and there was no real reason why the high level alarm was not connected. He is unsure as to how to install the alarm and is unwilling or unable to hire the work clone. The well serving the house is in operation and was in place on the lot when the holding tank was installed. No information is available on the well. Mr. Piaskowski stated the tank is not leaking nor is there m~y septic effluent on the ground. The tank has never overflowed and is apparently functioning as designed. The certified as built has uot been submitted because the high level alarm has not been installed. A copy of the site plan showing the location of the holding tank is attached for your review. A new permit must be obtained from this office fol' a well and/or on-site wastewater system NOT installed by the expiration date. If you have drilled the well, a well log must be sent to tiffs office for documentation of the installation and to close the permit. If the on-site wastewater system has been completed and a licensed Professional Ellgineer has inspected the installatiou of the on-site wastewater system, the original as-built inspection report must be sent to this office fol' review, approval and documentation. All iuspection reports must be submitted within 30 (lays of construction completion. No. 2 Holding Tank Permit Dear Dan: Attached is a letter from Isaac's Pumping Service indicating he will service the holding tank scheduled for installation on the subject lot. The tank will be placed adjacent to the driveway/parking area in front of the house on the lot. The pumper truck will have access to the tank on a year round basis as the area surrounding the tank is flat and well graded. I have also included an area map showing the well and septic systems on the lots surrounding the new tank installation. No wells are located within 100' of the new tank. No. 2 Holding Tank Permit Dear Onsite Services Engineer: The subject lot is bisected by Rabbit Creek near its midpoint. The 100' stream setback leaves very little area for placement of an onsite septic system if an alternate site is also required. The previous occupant of the property utilized a cesspool for disposal of septic waste without benefit of a septic tank. We are, therefore, unable to consider the placement of a new system an upgrade to an existing system. The only solution at this time appears to be placement of a holding tank until the owner of the property can obtain a vacation of a portion of the right of way for Our Own Lane. The attached site plan illustrates the setback problems. It also shows the area identified for placement of the holding tank. The testhole placed near the site for the holding tank revealed groundwater at a depth approximating 4'. No. 2 Holding Tank Installation Scope of Work o Install new M.O.A. approved 2,000 gallon holding tank with alarm system. Piping shall be ASTM D3034 P.V.C. or ASTM D2661 A.B.S. Assure 100' separation distance from Rabbit Creek and onsite water well to new holding tank. Tank burial must be a minimum of 4";'. Shallbwer 'bury? will require styrofoam high density direct burial insulation, minimum 2" thick. Tank hold downs shall be per tank manufacturer's recommendations. ..~ . . ._.. ,. All materials and workmanship shall be in accordance with A.M.C. Chapter 15.65 Wastewater Disposal Regulations and in particular Section 15.65.070, Holding Tanks. Electrical fittings and connections for wiring of the high liquid level alarm shall meet NEMA requirements for a water tight service.
