Water and Wastewater Upgrade Specifications
HOLDING TANK:
t. Expose the top of tank at both ends to allow the engineer to verify the
size and integrity of the tank, watertight manholes, and calder couplings. Repair or
replace as necessary.
2. Install an approved float in the tank set to trigger an audible and visual
alarm mounted inside the house when the fluid level reaches the point where 500
gallons of capacity remains.
3. Extend tl~e cleanout pipes as needed and provide 4.5 feet of soil cover
over the top of the tank for insulation and to prevent flotation.
4. Expose a portion of the sewer line between 'the foundation cleanout and
the holding tank to allow the engineer to verify that the previously existing septic
tank is no longer in place,

WELL:
1. Develop tile existing well to produce a minimum of 2 gallons pet' minute
or drill a new well elsewhere on the lot that is capable of producing 2 gallons per
minute. Care shall be taken to ensure that the location is both within the lot lines
and greater than 75 feet from the holding tank.
2. If tile existing well is deepened, first remove the existing pitless adapter
and weld the hole shut, then install a new pitless adapter and connect to the
existing water line leading to the house after the deepening is completed.
3. If a new well is drilled, install a pitless adapter after the well is
developed and connect into the existing waterline leading into the house. In either
situation, test the entire water system to be sure that all components are working
properly.
4. If a new well is drilled, properly abandon the old well by filling it with
bentonite or concrete grout, and cut it oiT below grade with a steel plate

Yesterday I investigated a sewer complaint on the above
property. The complaint alleges that last fall a new holding
tank was installed on this property. According to records in
this department an approved on-site sewer system was installed
in 1977. Since that time there have been no permits for any
type of alteration to the system. However, information received
by this office verifies the holding tank installation.
AMC 15.65.120A prohibits any person from installing or modifying
an on-site wastewater disposal system without first obtaining a
permit. Requirements for a permit are spelled out in section
.120B.
During the inspection I also noted that although the well wires
are disconnected, the well.casing is not sealed as required by
Anchorage Municipal Code 15.55.060G and K.1. Since the well is
a direct conduit to a potable water aquifer, the lack of a seal
may pose a serious health risk to users of this water.
Within fourteen days the following must be accomplished:
Install an approved sanitary seal on the well casing.

Prior to granting a permit for a holding tank, the
engineer's report must show that it is not possible to
construct a conventional sewer system. Special
conditions for a holding tank installation will be
specified on the permit.
Provide an inspection report by the engineer on the
system allowed by the above permit.
This office will not grant a health approval on the dwelling
until the above discrepancies have been corrected. In addition,
failure to comply with the above will result in lega% action.

Dear .Gloria:
This letter ~onstitutes our submission for a progress payment on the
remodel of the above referenced property. The following list of
items have been completed to date. This list will correspond with
the highlighted items on the attached copy of our proposal.
A natural gas forced air heating system and hot water heater have
been installed.. ]'he well shed has been removed, .wood~stove flue has
been extended, side railings have been added to the front deck and
the rear fa-fret tails have been cut back and a fascia installed.
Tile and vanity cabinet have been removed from the upper bedroom and
1/2 bath, the back of the ~loset has been framed in and one window
has been replaced with an egress size unit. The lower 'bath vanity
and floor covering have been removed. The burlap and trim have been
removed from the lower bedroo~m a~d the f'~ue add a closet framed in.
The plywood on the ceiling and the corner shelves have been removed
from the dining room, the plumbing pipes in the corner have been
framed over and the ceiling furred for sheetrock. The wood lights,
cabinets, shelves and wall trim have been removed from the kitchen
and the ceiling has been furred for sheetrock. The burlap has been
removed from the rear entry and a step %~ the kitchen installed- The
ceiling tile and wo~d trim have been removed from the living room,
the stairs to the second floor has bemn replaced and a step to the
kitchen installed- The electrical system has been reworked and
corrected and thm mlectric heat removed. The sheetrock has been
installed, taped and textured.
We are asking ~or a payment of $18,104.32 to cover this work.
Painting is in progress and the remainder of the work should be
completed within a month.

The department has recently investigated a complaint of an
illegal on-site sewer installation on the above lot. We have
also been given copies of documents, signed by you, which verify
the allegation.
I am aware that over the past several years you have obtained
various on-site permits, so I am confident that you recognize
the need for a permit prior to installation. .Installation
without benefit of a permit is in violation of AMC15.65.120A.
Since you were responsible for this installation, I am notifying
you of the violation. At this time the department is
considering what legal action to pursue.
I believe that we may be able to amicably resblve this complaint
if you will contact my office as soon as possible. Should you
fail to do this, I will continue to process this complaint to
completion.

RE: Test Hole and Soil Log Report for Sanitary System
Lot 22, Block 5, Rabbit Creek l]eights
Dear Mr. Tadda:
We are submitting herewith the test boring results and our comments
regarding soil conditions encountered at the sL~ject site. This
investigation was performed in accordance with your request of
June 27, 1976 and those procedures outlined in a letter dated
July 15, ].975, by Mr. Rolf Strickland of the Municipality of Anchorage
Department of Environmental Quality.
A single test hole was put down within Lot 22 Block 5 for the purpose
of defining genera] subsurface soil conditions for the proposed sani-
tary syste. Excavation was accomplished with an auger' type dri] ling
rig and the test hole was extended to a total depth of ]9 feet below
