Field Orientation Waiver Request
Ladies and Gentlemen:
I am requesting a waiver for the minimum distance to a break in the terrain where the slope is greater
than 25 percent. There is a retaining wall approximately 18' away where the top of system is at
approximately the same elevation as the top of wall and the bottom of the system is at approximately the
same elevation as the toe of the wall. This system operated for over 30 years in this configuration without
any effluent daylighting, and has been operating in failure for the past 2 to 3 years without any effluent
daylighting. Granting this waiver is not likely to affect the health and safety of this lot or the surrounding
lots now or in the future.

Septic System Permit Upgrade Request
Ladies and Gentlemen:
I am writing to request a permit to construct an upgrade system be issued for this property. The proposed
systems will serve an existing three (3) bedroom house. Currently the lot is developed. The existing drain
field is in failure and will be abandoned by removal. The existing system was installed in this location in
1979 and operated for over 30 years in that location, it is now in failure and no daylighting of effluent to
the surface has occurred. Due to space available the new drain field will be built in the same location; any
part of the existing field within 10' of the new drain field will be removed. The lot is served by a private
well that is located over 100' from the new system. The surrounding lots are also served private wells that
are located over 100' from the new system.
1. Soils. One test hole was excavated by Pannone Engineering Services in July, 2012, and the ground
water was monitored for seven days. Ground water was monitored at a depth of 9.5' below grade.
Bedrock was encountered and 10.0' below grade. It is my opinion, based on the results of the percolation
tests and overall soils appearance; an application rate of 0.8 gallons/day/square feet should be used for a
conventional wastewater system.
2. Soil Absorption System Design.
a. See Sheet 2 of the design package
3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain
field. The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage
ditches.
4. Topography: The drain field will be installed in the existing mound in the same location as the
existing field. The ground slopes to the left and the right from the center of this mound.

5. Lot Line Waiver: I request a lot line waiver from the drain field to the west property line due to
the small area available for the construction of the field.
6. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification
that the review is complete and that there are no further comments is received from MoA On -Site
Department, the note will be removed and "Issued for Construction" drawings will be issued.
The proposed installation will not affect the future development of the surrounding or existing lots. There
are no wells or septic systems within 100 feet of the proposed septic location.

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of On -Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

It is requested that you issue a waiver from the well to the septic tank at 97'. Also
request a waiver from the leachfield trench to the west property line at 5'. It appears
that the leachfield trench is less than 5' from the septic tank.
Since the tank is fiberglass, this does not cause concern over the possibility of the exterior
tank surface being wet. Potential surfacing effluent or tank leakage would not flow
towards the well. The well is uphill from the septic tank. Septic tanks are not considered a
continuous source of contamination, as they are designed to be water tight. The fiberglass
septic tank will not corrode eliminating concern over leakage from tank wall deterioration
(corrosion).
We do not anticipate any adverse effects on neighboring wells, septic systems, reserve
areas or drainage patterns by the granting of these waivers.
