1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing
Special Provisions:
A test hole is required prior to construction of the absorption field, in the location shown on the site plan, to
confirm minimum separation to impermeable soil and seasonal high groundwater, as well as percolation rate.
Construction may proceed at your own risk prior to 7-day groundwater monitoring. If results require a design
change, construction shall stop pending approval of a change order. Please submit results with the inspection
report (or change order, if required).

Subject: Rabbit Creek V&H Block 11H Lot 10A — Paine Road
Septic system design
Dear On -Site Services Engineer:
The owner of the above lot needs to replace her trench for a home sale, so we are submitting this
permit application for the construction of a new septic system. The attached site plan identifies the
location of the home and existing well, as well as the new septic location. No conflicts exist
between this proposed system and any other well or septic system, whether on this lot or adjacent
lots.
The ground surface on the lot slopes fairly uniformly from the front of the lot toward the back, at
between 20-25% slopes. Near the back where the septic system will be located is slightly flatter,
under 25%. There are no slopes greater than 25% within 50 feet downslope of either the primary
or alternate site. Contours are shown on the site plan showing the grade and direction of flow.
Stormwater drainage will not impact this septic system. The new trench will be constructed parallel
to the slope as much as possible. Wells on this and adjacent lots are shown. The new system will
be a minimum of 100' from all wells and surface water, and more than 5' away from the septic
tank.
Due to the nature of the proposed home sale, we have designed the system using the test hole
information on this lot and adjacent lots, including across the street. We have then specified that a
new test hole will be dug to confirm consistency of the soils and groundwater levels. We have
noted the test holes that we found in the following table. The most conservative perk rate that we
found was 30 min/in, and the highest groundwater was 5.5' in October. Our design is based on
those criteria.

It has recently come to our attention that your single family dwelling has been
remodeled into a duplex. Your on -site septic and well permit, #810130, from the
Municipality is approved for only a single family, 3-bedroom dwelling. You need
to contact Steven Eng at the State Department of Environmental Conservation
within five working days of receipt of this letter and apply for a permit to
operate a Class C well and septic system.
You also are required to cease and desist furnishing water to the adjacent pro-
perty of Richard Wiley Dickins at L13 B11 Rabbit Creek Heights Subdivision imme-
diately. The Dickins has no proper sewage disposal system for the water.

Dear Mr. Wagner:
As per your request, I have inspected the well and the septic system
on the above referenced property, and find the well to be installed
in accordance with Municipal Standards, at least 100' from the septic
system. The septic system has a total of 5-4" standpipe cleanouts
installed and capped with air tight caps.
