The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On -Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an Independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also Issues COSAs upon request to homeowners. Certificates of On -Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

Investigation, based on procedures outlined In the Certificate of On -Site Systems Approval Guidelines for this application,

information obtained from the Municipality of Anchorage riles and from my investigation and inspection, the

Arsenic Advisory
Certificate of On -Site Systems Approval # 070264
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block 5, Lot 6 of
Poggas Subdivision. This inspection revealed an arsenic concentration of
10.6 micrograms per liter (ug/L) for the property's well water sample. The
Environmental Protection Agency (EPA) has established a maximum
contaminant level (MCL) of 10.0 ug/L for public drinking water systems.
While private wells are not subject to this regulation, EPA standards are
based on existing health information and can therefore be used to gauge the
relative quality of water from private wells. Information on arsenic is
available from the On -Site Water and Wastewater Program website
(www.muni.or /og nsite) or at 343-7904.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

Subject: COSA for Lot 5, Block 6, Poggas Subdivision
To whom it may concern:
Recently we inspected and then tested a private well on the subject property. The house
was built in 1954 and we assume the well was drilled about the same time. The well is
located in the basement of the dwelling, near the east wall. The wellhead is surrounded
by a poured concrete basement floor. The static water level is 33 feet.
There is no well log available. The lots around this one arc on public sewer and water.
The nearest well we have information on is on Lot 19, Block 1, Thomas W. Sperstad.
During a COSA application the engineer indicated the well casing was extended to 118'
in 1996. North of this lot, on Lot 18, a well was drilled by Alpine Drilling in 1998. The
driller's log shows casing beyond 40 feet (verified by telephone to Dave Harper).
A new variable speed pump, well seal, and pressure tank were installed by Arctic Pump.
The pump that was installed is only used for wells that are less than 100 feet deep. A
well flow test produced 1000 gallons in 208 minutes. Allowing for 100 feet of casing at
1.48 gallons/feet of casing, this well is able to produce over 4 gallons per minute.
Increasing the height of the well casing above the concrete floor will not provide
substantially greater protection for the wellhead, and will be difficult to achieve.
Inasmuch as this well was drilled years before the requirement for 40 feet of casing, and
given the conditions found at the wellhead, we are requesting that no further work be
done. Photographs of the wellhead and piping are attached.

Your file reflects that in 1977 the owner felt he was connected to the public sewer system
but AWWU was unable to perform a dye test due to an address problem. On July 12,
2007, AWWU confirmed by dye test that the dwelling and a separate shop building are
connected to the public sewer system.
