Ms. Wockenfuss:
As we discussed recently, the subject property is owned by a Mr. Tony Strupulis. The septic system was
designed (Advantex Cat III system) by KND Engineering and permitted by the MOA in April of 2003.
The installation, and inspection of the installation, was performed in September 2003 by Alaska Water
and Wastewater Consultants (AKWWC), which is the former name of Gayness Engineering Group Ltd.
After completion of the installation in 2003, we requested and waited for the owner to provide us with the
well -log, pump -log, and as -built survey so we could complete the MOA well and septic "Inspection
Report". We requested this information several times (including a follow-up voice message on 10/4/06),
and after not receiving it for some time archived the file away (assuming someone would contact us when
the house was complete and they needed a "certificate of occupancy" (CO).
In the winter of 2013 the MOA Onsite staff began to clean-up old files and it was noted that no as -built
drawings and/or "Inspection Report" had been received for this well/septic installation. You had
indicated that you had contacted Mr. Stupulis regarding the septic system installation and the as -built
documentation. He indicated to you that the system was installed and that you should contact our office
regarding documentation of the installation. You then contacted our office to see what we knew about the
project. We pulled the file (quite frankly, we had completely forgot about the job) and confirmed that the
as -built documents had yet to be finalized because we needed an as -built survey. At that time it was
confirmed with Anchorage Tank and Welding that the system had yet to be started -up, because Mr.
Stupulis had indicated to them that he has yet to occupy the home. We conveyed that to you and took no
further action again, figuring that we would be contacted by Mr. Stupulis when he was close to
getting his CO.
We were contacted by you again in January of 2014 regarding the status of the as -built documentation.
We asked you to confirm if a CO had yet to be issued and you were able to confirm it had not. We then
made a call to Anchorage Tank and Welding and they confirmed that the Advantex system had yet to be
started -up and was not being monitored with their Vericomm system. I then contacted Tony Stipulus and
he indicated that he was still working on the house and the septic system was not being used yet. I told

him that the MOA was looking for documentation of the septic system installation and that we should try
to get the paperwork finalized. I conveyed to him that if he would get the as -built survey completed I
would finalize the paperwork. The as -built survey was completed by Lang and Associates during the
week of 2/8/2014 and forward to our office on 2/14/2014.
Attached are the as -built drawings of the septic system. The installation inspections were performed by
GEG employee Susan Oswalt. The following is a summary of pertinent points regarding the
installation/documentation:

1. Documentation of the Electrical Work: AKWWC was hired by Mr. Stupulis to install the septic
system and perform the installation inspections as required by the MOA Onsite Department. As part of
the installation AKWWC hired a licensed electrical contractor, Rising Son Electric, to perform the
electrical work. Because no home was present at the time the septic system was installed, we mounted
the Orenco Panel on a vertical 4" X 4" post immediately adjacent to the Advantex system. Rising Son did
all of the wiring for the Orenco components. Mr. Stupulis later provided power from the house to the
Orenco panel. In short, AKWWC is responsible for all electrical work from the Orenco Panel to the
Orenco components and Mr. Stupulis is responsible for the electrical power (and phone line) from the
house to the Orenco panel. Given the fact that the home is in the MOA "land use" area, electrical
inspections are not performed by the MOA; therefore, it is standard practice to obtain a letter from the
licensed electrician that did the work, stating that it was done in compliance with the applicable electrical
codes. Due to the passing of time, we are now unable to obtain a letter from the electrician who did the
work, because Rising Son Electric sold several years ago and has a new owner. When we discussed this
issue several weeks ago, you indicated that you would be willing to accept the invoice from Rising Son
Electric as documentation regarding the electrical work. I was able to find the invoice from Rising Son
Electric and documentation that we paid the invoice. A copy of both documents is attached.
2. Location of Test Hole Relative to Primary and Reserve Trenches: The design prepared by KND
and permitted by the MOA allowed for a large portion of each trench to be outside the 30 foot radius of
KND TH#02-2. The reason for this was probably due to the following facts:
• The percolation rate is 12.6 minutes per inch
• The allowable application rate for CAT III system effluent is 5.0 gpd/ft2
• The required absorption area for a four (4) bedroom drainfield is 120 ft
• The minimum required trench length for a 5 -wide trench with 6 inches of effective depth is 24
feet.
• The actual trench lengths (primary and reserve drainfields) are 40 feet each.
• In short, the majority of the trench that is outside the 30 foot radius of the test hole is
redundant/bonus trench length.
The KND showed the reserve trench slightly over the TH location. Given that the fact that the reserve
drainfield was installed at the same time as the primary trench, we located the reserve trench 5 feet away
from the test holes, so as to avoid placing the trench over un -compacted soil in the test hole The 120 ft2
of area required for each of the drainfields (primary and reserve) are encompassed within the 35 foot
radius of the test hole pipe.
3. Groundwater Readings and Depth of Installation: The KND test holes were excavated on
8/27/2002 and groundwater readings were taken a week later on 9/3/2002. Per historical rainfall records,
there was 0.92 inches of precipitation during the 5 days preceding the 9/03/2002 GWM reading by KND.

This is a fairly intensive amount of rainfall during such a short period of time. The KND readings on
9/03/2002 indicated groundwater was 3.3 feet below grade in TH#2. During the drainfield installation's
on 9/04/2003, Susan Oswalt documented groundwater at 5.33 feet (64 inches) below grade in TH #2.
Groundwater monitoring readings taken from monitoring tubes placed vertically in an excavated hole
(backfilled with un -compacted soil) are subject to artificially high readings until the soil has had some
time to re -compact. The loose/disturbed soil and lack of vegetative mat around the test hole provides
little impedance to surface runoff running into/around the test hole. The loose soil around the GWM pipe
and the intense rainfall preceding the groundwater readings on 9/03/2002 likely resulted in erroneous, but
conservative, groundwater readings by KND.
Due to the passage of time, the conversations that took place between the MOA, myself, and Susan
Oswalt during the installation can only be speculated, but it is our assertion that we obtained MOA verbal
authorization to modify the system depth. Based upon Oswalt's readings, the design was revised to place
the top of the sand in the lower drainfield at 0.29 feet above OG (highest OG on uphill side of trench),
rather than 1 foot above OG as proposed in the KND design.
Based upon GEG's groundwater readings, the following separations have been provided between the
bottom of the drainrock (or top of filter sand) in each trench and groundwater:
• Upper Trench GEG groundwater reading on 9/04/2003: 6.3+ feet
• Lower Trench - GEG groundwater reading on 9/04/2003: 5.6 feet to 6.7 feet
The adjacent property (Potter Heights, B2, L10), has a dual trench system on it. There were two test holes
excavated for the design. On 9/20/1995 groundwater was encountered at 4.5 feet below grade in one test
hole and 5.5 feet below grade in the other test hole. These groundwater readings are consistent with the
readings taken by GEG in September of 2003.
It is important to recognize that this is a CAT III advanced treatment system. Based upon Alaska test
data, the effluent BOD and TSS numbers are consistently below ADEC secondary standards (30 mg/L)
and the un -disinfected effluent has fecal Coliform reductions of 95% or more. In short, the effluent from
a CAT III system is of such quality that a reduced separation distance to groundwater is fully justified if
deemed necessary by your department.
4. Size of Orifices in pressure lines: During the installation, the decision was made to use pressure
distribution rather than 4 inch perforated pipe in the trenches. This will allow for better distribution of the
effluent along the length of the trenches and avoid creating any concentrated hydraulic loading along the
trench length reducing the potential for daylighting out of the mound. Susan Oswalt did not note
the orifice spacing or size on her September 2003 field notes. The construction superintendents field
notes are unavailable. Neither person has worked for GEG for a number of years. It is reasonable to
assume that during the installation I was probably contacted by Susan Oswalt or the construction
superintendent and asked what the orifice size/spacing should be. I probably told them to use '''A" holes
and space them at 4 feet on center. There is a separate pressure line coming out the Orenco pump vault
for each trench. There are also isolation valves inside the pump vault so flow can be diverted to either
trench or both trenches at the same time.

5. Side Slopes on the Mound: The MOA code requires 3:1 side slopes or less on mounded systems.
The final slope grades were not noted in our 2003 field notes. An inspection was performed recently and
final grade shots were taken. The entire mound is now vegetated and has slopes of roughly 2.5 to 1. In
short, the slopes are slightly steeper than allowed by the code. Given the effluent quality (CAT III
system) and the presence of MOA sand below the drainfield, the effluent will rapidly migrate downward
to an elevation that is at least 4.5 feet below the top of the mound. From this elevation, the effluent would
have to travel laterally approximately 25 feet in order to daylight out of the mound. Given the porosity of
the insitu soil below the drainfields, significant lateral migration of the effluent is unlikely. Given these
facts, we are requesting that the MOA approve the installation with the existing side slopes. Once in use,
the CAT III system will be inspected regularly as required by MOA code. If daylighting is observed,
additional fill can be imported so as to create more gradual side -slopes on the downhill side of the mound.
6. Closing: The subject installation was completed in 2003 and the system has been available for
beneficial use by the owner since that time; however, it is my understanding that he has yet to occupy the
home. Following completion of our work in 2003, we have not performed any additional engineering or
construction work on this system, except some minor work in August of 2005 to seal a leaking manhole
riser.
This letter, the "on-site wastewater inspection report", as -built drawings, and application of my
professional engineering stamp is to document the construction and engineering work completed in 2003.

ON-SITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION PRACTICES AND MATERIAL SPECIFICATIONS
LEGAL DESCRIPTION: POTTER HEIGHTS, BLOCK 2, LOT 9
GENERAL:
1. The scope of this project includes the installation of a septic tank and/or drainfield to serve a
single-family residence on the referenced property as delineated on the attached Municipality of
Anchorage (MOA) permit and site plan.
2. Construction shall be in accordance with the approved site plan and design drawings; MOA
permit with any special provisions or conditions, and all applicable State and Municipal
Wastewater Disposal Regulations.
3. KND Engineering, Inc (KND) assumes no responsibility of the accuracy of the as -built survey
provided by the owner or owner's representative.
4. The owner or contractor is responsible to notify the engineer of any discrepancies noted on the
design plans or drawings prior to commencing work. These discrepancies could be, but not
limited to, possible encroachments, surface water, varying soils, unplatted easements, types or
use of dwellings on or adjacent to property, nearby public wells, etc.
5. The contractor shall be responsible for locating any necessary underground utilities, easements,
property lines and surface water. The owner and/or contractor shall ensure, if deemed
appropriate, that a registered land surveyor prior to construction flags all setbacks.
6. The contractor shall be responsible for verifying all pertinent separation distances and
component layout.
7. Unless specifically agreed otherwise, the property owner shall be responsible for final grading
areas subsequently depressed from soil settling. On all leachfield mound systems, the property
owner shall be responsible for ensuring a satisfactory vegetation growth over the mounded
area.
8. The MOA Development Services Department On -Site Water & Wastewater Program must
certify contractors installing wastewater disposal systems. Owners installing their own systems
must also receive prior approval from said MOA Department.
SEPTIC TANK / HOLDING TANK / LIFT STATION INSTALLATION:
1. A septic tank is to be constructed by a certified septic tank manufacturer. Construction shall
include two 4" cleanouts for pumping access.
2. The septic tank shall be sufficiently bedded with proper material and compacting to prevent
damaging, settling or shifting of the tank.
3. All standpipes on the septic tank shall extend a minimum of 12 inches above the final grade.
4. Septic tanks installed with less than 4 foot of cover shall be insulated. The contractor will
ensure that the fill over the tank does not exceed the burial depth rating.
5. A foundation cleanout shall be installed one to four feet from the building foundation. In the
line between the tank and the leachfield there shall be two adjacent cleanouts (unless an effluent
pumping system exists within the septic tank). These cleanouts shall be located on undisturbed
soil not more than 10 feet from the tank. The second cleanout, in line, shall be to clean toward
the septic tank.
6. Final grading over the septic tank shall be such that a positive slope exists away from the septic
tank.

7. All I-I.D.P.E. tanks shall have a vertical length of 4 -inch perforated drainpipe besides the tank
extending to the bottom of the tank to above grade to monitor ground water.
8. All tanks shall be anchored when there is not enough adequate soil cover to resist buoyancy
forces in high -groundwater areas.
9. All holding tanks and lift stations shall have an audible and visual alarm inside the residence.
10. All lift stations shall be wired in compliance with the Uniform Electrical Code and all MOA
guidelines.
11. The contractor shall remove the old tank, or abandon it in compliance with the Uniform
Plumbing Code. The contractor is solely responsible for the abandonment procedure.
ABSORPTION TRENCH/DRAINFIELD INSTALLATION:
1. Excavate the proposed trench to the dimensions shown on the design. The bottom of the
excavation shall be within 2 inches of level. If the sidewalls of the excavaction become smeared,
they must be raked or scratched (roughed -up) before gravel (sewer rock) placement.
2. Once the sewer rock is installed, the distribution pipe is installed level with the perforations
facing downward. Gravel is then to be placed over the distribution pipe to provide a minimum
of 2 inches of cover over the pipe.
3. A silt barrier must be installed between the final gravel layer and the native soil backfill. Ensure
the silt barrier covers the entire gravel surface before placing backfill.
4. Monitoring tubes shall be four- (4) inch diameter, installed approximately in the locations
shown on the design, and extend a minimum of 12 inches above final grade. The portion of the
monitoring tube extending through the gravel shall be perforated from the bottom of the trench
to the invert of the distribution pipe. This is equivalent to the effective depth of the gravel as
noted on the design.
5. Backfill over the final gravel layer must not be less than twenty-four inches. Insulation must be
installed when the backfill depth is less than thirty-six (36) inches. The finish grade over the
trench must be mounded to prevent the formation of a depression after settling.
6. All advanced technology waste water disposal systems (Advantex, Biocycle, Intermittent
Dosing Sand Filter, etc...) shall be constructed, installed, maintained and monitored per
manufacturer's specifications and MOA requirements.
MINIMUM MATERIAL SPECIFICATIONS:
1. A MOA approved septic tank manufacturer must construct any septic tank proposed for
installation.
2. The following pipe materials are MOA approved for use in septic system installations:

3. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical Company
Styrofoam HI or equal).
4. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Femco, or
equal).
5. A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N, or equal) must be installed between
the final leachfield gravel layer and the native soil backfill.
6. All leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel with less than 3% passing
the #200 sieve. If deemed necessary, a sieve analysis should be performed.
7. When sand is being used as a filter material, its gradation specification must conform to current
MOA or DEC requirements.

INSPECTIONS:
Typically there will be three (3) inspections required during the installation of the wastewater
disposal system. These inspections will occur as follows:
1. The first inspection must be conducted after the excavation of ditches, pits, trenches, or beds
and before the installation of any gravel. A septic tank may be set in place, but may not be
backfilled before this inspection.
2. The second inspection must be conducted after the placement of the sewer rock, distribution
lines, standpipes, and cleanouts, but before the placement of the silt barrier, insulation, or any
other backfill.
3. The final inspection is to occur upon final grading of the property.
Often there will be more than these 3 inspections required, especially with the installation of multiple
trenches, sand filters, pressurized distribution systems, etc. The inspecting engineer is to be contacted
at least 24 hours prior to the start of construction. If necessary, a pre -construction meeting will take
place on-site. The inspecting engineer will not coordinate, direct or control in any way the contractor's
activities.
The owner shall contract with the contractor to perform the work outlined in these specifications and
attached plans and in accordance with the attached MOA permit. All materials, components, and
construction practices shall comply with the MOA's and the State of Alaska's latest ordinances,
regulations, or policies regarding wells and septic systems. The failure of the inspecting engineer to
notice a material or construction non-compliance does not relieve the contractor of his/her
responsibility to correct such deficiency. There will be no contractual arrangement existing between
the contractor and KND Engineering, Inc. KND shall be the owner's representative and will inspect the
work as stated above to document the contractor's activities. Final acceptance of the contractor's work
rests with the owner and the MOA.
KND shall have no liability to the owner or to others for acts or omissions of the contractor or any other
persons performing work on this project or the failure of the contractor to carry out the work in
accordance with these construction documents. KND's inspecting engineer will not be responsible for
the construction means, methods, and techniques, sequence, procedures or the safety precautions
incident to this project.
BY PROCEEDING FORWARD WITH THE INSTALLATION OF THE WATER AND/OR
WASTEWATER SYSTEM (S), THE PROPERTY OWNER, CONTRACTOR, AND WELL DRILLER
ACKNOWLEDGES HAVING READ AND ACCEPTED THESE SPECIFICATIONS.
