Approval Certificates based only upon the representations given in paragraph 5 above by an independent
professional engineer registered in the State of Alaska. The DHHS does this as a courtesyto purchasers of homes
and their lending institutions in order to satisfy certain federal and state requirements. Employees of DHHS do not
conduct inspections or analyze data before a certificate is issued. The Municipality of Anchorage is not
responsible for errors or omissions in the professional engineer's work.

Dear Mr. Cross:
This letter serves as a request to waive the 100-foot separation requirement for the above referenced
property. The actual separation distance between the well and the private well is 98.5 feet, approximately
1.5-feet less than the 100-foot separation required by the Municipality. The separation distance was
discovered in conjunction with an adequacy test that was performed on the well and septic system on May
27, 1997.
The absorption system was designed for a fern'-bedroom residence, although the house is currently listed
as a three-bedroom house. The septic system was installed by Enoch Adams on September 14, 1976, and
has a 1250 gallon Sunset plastic septic tank. The absorption system is a 39 foot deep trench with seven
feet of gravel and 11 feet of depth over all. The absorption area on the septic and well inspection report is
listed as 540 square feet. There is a six inch sump at the end of the trench and a four inch clean-out at the
beginning of the trench.
The 98.5-foot separation distance between the well and the septic tank clean-out was approved by a
Municipality of Anchorage official (John Kennedy) on September 14, 1976, with a note that the well was
not installed at the time of inspection. The well log states that the well was installed November 4, 1973.
In researching adjacent properties for a well to septic tank waiver, it was noted that other lots in both
blocks five, six, and seven also have septic tanks encroaching within the 100 foot radius of the well.
Support for waiving the separation distance includes the fact that the well is located up-gradient from the
septic system. Nitrate levels measure in water samples from the private well were 7.6 mg/l which is below
the maximum level of 10.0 mg/1. The actual separation distance is only 1.5-feet short of the required 100-
foot minimum. It is unlikely that this short distance would have a significant adverse effect on the potable
water taken from the well.

RECEIVED
A sewer system adequacy test was performed on the system located
on the referenced property, as you requested. The septic tank was
pumped and verified to have a capacity of 1250 gallons. The absorption
trench was tested by a continuous flow of 874 gallons over a period
of 24 hours without any measurable rise in the sump at,the end of the
trench.
It can be concluded from this test that the waste water disposal
system serving the four bedroom residence located 6n this property is
currently functioning adequately. However, the system cannot be
guaranteed against subsequent failures.
