While technically this includes the entire septic system, the drainfield portion of the review is very minimal and as such we are requesting a septic tank upgrade permit be issued for the following scope of work: 1. Remove the existing septic tank in accordance with the code. 2. Install a new Infiltrator IM-1530 as a single-chamber tank (without partition). a. The entire tank will serve as the first septic tank chamber. 3. Install a new Infiltrator IM-1060 as a single-chamber tank (without partition). a. The entire tank will serve as the second septic tank chamber. 4. Install a RetroFAST septic treatment system. 5. Connect the two existing 4-bedroom drainfields together by installing a splitter valve after the post tank double cleanouts. a. The drainfield currently in use is showing signs of excessive bio-matting and will be rehabilitated in-place using a RetroFAST treatment system. b. The other drainfield is not currently hooked into the septic system. A preliminary adequacy test shows it is capable of handling at least the 4-bedroom flow it was designed for. Design for the alternate 6-bedroom drainfield is shown on the attached drawing. There are no probable adverse impacts to adjacent properties if installed in accordance with the design. The rates were measured to be 1.2 minutes per inch and 14.5 minutes per inch. I have attached the results on the revised soils log sheet. Further I have revised the design accordingly. It is attached. I believe this is the final comment concerning this upgrade permit request. We are prepared to install this system within one week of issuance of the permit. My response will be in the same order as they are presented. et Comment: On the design site plan please show the effluent entering the middle of the field. Response: Design modified. V- Comment: Regarding Cleaning/Thawing Accessibility Response: My comments indicated that the line could be cleaned through the double clean-out at the tank and through the ball type diverter valve. Additionally, I indicated that based on field experience, thawing companies (JR's Pumping, Chugach Sewer & Drain, Alaska Sewer & Drain, Dayton Sewer & Drain) are able to turn a jetter head up a clean-out that is sweeping the opposite direction. Thus allowing the line to be cleaned from two directions. The design has been modified to have a double clean-out installed at the connection to the field to facilitate cleaning. Comment: Regarding the percolation test. Response: Thank you for your clarification. I would be more than happy to perform an additional percolation test at the time of installation when the test can be performed under laboratory conditions. Currently there is over four feet of frost in the ground at this site. That is why the test was performed the way that it was. Please also find attached three quotes from Certified Excavators to install this system. Page 2 of 2 If you have any questions or concerns, please contact me at 272-8218. Sincerely, • Steven R. Comment: The percolation test procedure does not follow the adopted Table 3-8 of the EPA Design Manual. In the future, percolation tests not following this procedure will not be accepted by this department. Response: I disagree with this comment. Please provide written discrepancies that you feel my test procedure did not follow. Comment: The field was not designed parallel to the contours. Response; The ground surface has a slope of approximately 2 percent. The orientation of the field was not parallel to the slope, but still met the six feet vertical separation to bedrock, and ground water as verified in test hole 1. The ground will rise approximately one foot over the length of this field. The reason the code wants the field to be parallel to the contours is to maintain the separation to bedrock and ground water. The original orientation met that requirement. ek Comment: The subsurface disposal field may not be located less than 14 feet from any existing field. ' Response: Field relocated to maintain 14 feet from the existing system. Page 2 of 2 Comment: How will the length of pipe from the diverter valve to the field be accessed for cleaning or thawing purposes? Response: A double clean-out is specified to be installed after the septic tank and before the ball -type • diverter valve. Additionally, a sweep -tee clean-out will be installed at the beginning of the field. A ball -type diverter valve is a horizontal valve, which allows a snake to pass through the valve for cleaning and thawing. if you have any questions or concerns, please contact me at 272-8218. Bragaw Street P. O. Box 196650 Anchorage, Alaska 99519 Subject: Lot 1, Block 7 Preuss, No. 4 Conditional Certificate of Onsite Approval Request Septic System Permit Request Ladies and Gentlemen: I am writing to request a permit to construct a new septic system be issued for this property and a conditional Certificate of Onsite Approval (COSA) be issued. The proposed systems will serve an existing four-bedroom house. Currently the lot is developed. The lot is served by AWWU community water supply. The existing septic system shall be reused. A diverter valve will be installed between the new tank and the existing field. The surrounding developed lots are served by private water systems and AWWU. The private wells are located over 100 feet from the proposed soil absorption system. 1. Conditional COSA. The Owner is in the process of selling this property. The existing system was located and inspected. The system was test for operation and was found that it could not accept the required 150 gallons per bedroom per day (600 gpd). The system is able to absorb approximately 250 gallons per day. There will be no health risk in approving this Conditional COSA as we will be installing the new system in June. 2. Soils. Two test holes were excavated. See the attached soils logs. Ground water was monitored for 30 days. No ground water was monitored to a depth of 20 feet below ground level after the monitoring period. Bedrock was not encountered in the test hole. It is my opinion, based on the results of the percolation tests and overall soils appearance; an application rate of 0.8 gallons/day/square feet should be used, using a conventional treatment system. 3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and the existing drain field. The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage ditches. 5. Topography: The average topography in the area of the proposed septic system is approximately 2 percent based on the survey information. The proposed installation will be greater than 50 feet from the shoulder of the steep slopes. 6. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification that the review is complete and that there are no further comments is received from MoA On -Site Department, the note will be removed and "Released for Construction" drawings will be issued. The proposed installation will not affect the future development of the surrounding or existing Tots. There are no wells or septic systems within 100 feet of the proposed septic location. Also, this letter will serve as a "Pink Sheet" as it includes additional comments that must be addressed before the permit is issued. 1. Regarding effluent entering the middle of the field: Please see the attached Engineer Bulletin 94-3 dated June 1, 1994. Item number 2 states "Any absorption systems with a design length greater than 50 feet shall be entered at the midpoint, unless otherwise approved by DHHS." 2. Regarding cleaning/thawine accessibility: If there are specific items to be used in the construction of a septic system please document this within the permit application. In your response you mentioned that a sweep -tee clean-out will be installed at the beginning of the field. Code requires the sweep to be directed toward the field. Are you proposing a different direction? If so, the field will not have adequate cleanouts and will be out of compliance with AMC 15.65. 3. Regarding percolation test procedures: See the attached copy of page 41 of the EPA Design Manual, Table 3-8. Test Hole 1: Based on the `Water Level Readings' provided on your percolation test, the depth of the water in the percolation hole exceeds 17" near the end of the test. Per the EPA manual, "At no time during the test is the water level allowed to rise more than 6" above the gravel". Test Hole 2: 1. Again, based on the 'Water Level Readings' provided on your percolation test, the depth of the water in the test hole is greater than 6". 2. The manual requires a minimum of three 30 minute readings to be made; you have not provided three 30 minute readings. The manual does allow 10 minute readings when the first 6" of water added after Co171mttnity, Security, Prosperity Steve Pannone 3/1/2007 Page 2 of 2 the soaking period seeps away in less than 30 minutes; your test did not meet this condition. New percolation tests are to be performed and submitted for review before the permit will be issued. Please address these comments and resubmit for review. In the future, when our office notifies you of obvious errors or omissions in your project submittals by use of the standard On -Site Sewer/Well Submittal Comment Sheet, we anticipate you will do a thorough re -review of your work and make the necessary corrections to come into compliance with AMC 15.65. If you request this office to give you detailed written instructions of how to make your corrections, an engineer review fee will be charged for the additional time. Our standard engineer review fee is $115.00 per hour with a minimum one (1) hour charge. If you have any questions of the above please contact me at 343-7907. Sincerely, Daniel J. Box 196650 Anchorage, Alaska 99519-6650 ENGINEER BULLETIN 94-3 TO: Engineers Involved with Design, Inspection and Construction of On-site Water and Wastewater Disposal Systems SUBJECT: Clarification of some design considerations. 1. Per AMC 15.65.060.11.4, the width of an absorption bed must not exceed 15 feet. Recently, the On -Site section routinely approved changes to this requirement. In the future, an exception to this requirement will be approved only after a thorough review and justification submitted by the engineer. 2. Any absorption systems with a design length greater than 50 feet shall be entered at the midpoint, unless otherwise approved by DHHS. 3. No absorption systems will be allowed which are designed on a "spill-over" concept, i.e. two trenches with the second trench only receiving effluent after the first trench has been filled. 4. Approved sands used to raise mounds or bed systems to insure the 4 foot separation from the bottom of the absorption field to the water table are: a. Road sand from Central Paving Products (Palmer). b. ASTM D 33 concrete sand available from Quality Sand (Wasilla). c. Pit run sand from Beaver Lake Sand & Gravel (Wasilla). d. Any sand from a source other than a, b, & c above must have a sieve analysis performed which indicates that the product is a sand and has less than 4% passing the 1/100 sieve and less than 2% passing the #200 sieve. The average flow in each 24 hour period exceeded that required for a four bedroom residence. It can be concluded from this test that the waste water disposal system serving the four bedroom residence located on this property is currently functioning adequately. However, the system cannot be guaranteed against subsequent failures. If we may be of further ER A AFE•, P.E.
