B. Covered, sealed, and heated to prevent freezing.

minimum from a slope greater than 25%.

Septic System Permit Upgrade Request
Ladies and Gentlemen:
I am writing to request a permit to construct an upgraded septic system be issued for this property. The
proposed systems will serve an existing three-bedroom house. Currently the lot is developed. The lot is
served by a public water supply. The existing septic system shall be reused. The surrounding developed lots
are served by the same public water systems. No private wells are located in the surrounding area.
1. Soils. A single test hole was excavated by Pannone Engineering Service in February of 2011. See the
soil log attached. Ground water was monitored for seven days. No ground water was monitored to a depth
of 15 feet below ground level after the monitoring period. Bedrock was not encountered in the test hole. It is
my opinion, based on the results of the percolation tests and overall soils appearance; an application rate of
1.2 gallons/day/square feet should be used, using a conventional wastewater treatment system.
Soil Absorption System Design.
a. See Sheet 2 of the design package
3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and the
existing drain field. The proposed drain field upgrade will maintain at least 100 feet from all surface water
and drainage ditches.
4. Topography: The average topography in the area of the proposed septic system is approximately 5
percent based on the survey information in the area of the septic system. There is an existing steep slope
approximately 30 feet from the proposed drain field. I request that a slope waiver be granted. The attached
section shows effluent will not day-light from the steep slope adjacent to the proposed drainfield. It is not
likely the effluent will infiltrate at a 25% slope since the soil absorption system is installed in clean sands
percolating at 1-5 MPI.

5. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification
that the review is complete and that there are no further comments is received from MoA On-Site
Department, the note will be removed and "Issued for Construction" drawings will be issued.
The proposed installation will not affect the future development of the surrounding or existing lots. There
are no wells or septic systems within 100 feet of the proposed septic location.

Ret Adequacy Test at Lot 8, Block l 1, Preuss Subdivision

Dear Dan:
Based upon your conversation with Fred Kenley of December 1, 1995, t prepared the
following report to document the adequancy test at the subject property.
GENERAL: Fred Kenley performed an initial site test on 11/21/95 and found that there
was 50 5/8" of water in the inlet side of the tank, and there was only one tank dean out.
There was approximately 3" of water in the after tank clean out, which was also very close
to the start of the drain field. Research at your office indicated that this septic system was
installed in 1977, the total depth being 10' with 5' of gravel 3' wide and 34' long.
The far end of the dram field contained a sump that was 110" deep from ground level,
with the invert of the drain pipe located 60 3/4" from the bottom of the sump. The sump
contained 66 1/2" of water. It was initially thought that the system had failed; however,
he did run approximately 130 gallons of water at a rate of 4.1 gallons per minute through
the after tank dean out, whereupon the water in the sump rose to 70".
Fred conferred with you the next day and in conversation it was determined that the
system was perhaps installed with the holes in the perforation pipe turned up and the 6"
sump being a solid pipe, as some systems were constructed this way during the 1970's and
later abandoned. You advised Fred to check with the homeowner about the possibility of
installing a monitoring tube and performing a new test.
Stewart Gilbert was contracted to install a monitoring tube. This was done on 11/24/95,
located approximately 10" from the sump at the same ground level, and driven to a depth
of 77" from the ground level. At this time, 21" of water was observed in the monitoring
tube, indicating that the water level was 11" below the top of the drain pipe.
The second test was performed by Fred Kenley on 11/25/95. The intiat depth of water in
the sump pump was 66 5/8". The tank level was again 50 5/8"; however, the level in the
new monitoring tube was 18 1/2", a drop of 2 1/2" from the prior day. The amount of
water introduced into the after tank clean out was 498 gallons, at a rate of 3.6 gallons per
minute. At the end of the test, the sump level rose to 71 1/8" and the monitoring tube
level Was 42 3/8", which placed the water approximately 10 3/8" above the top of the
absorption pipe. The monitoring tube measurement was 15 1/2" 24 hours later, thus 14
1/2" below the mc~r.':tc, r~ng ~,bc, ll9,~ever, the test clearly went above the accepted level.
Again, Fred Kenley conferred with you and you suggested that the test be run again with
water being introduced into the system at a slower rate to see if the water level could be
prevented from rising above the level of the septic gravel.
Another adequacy test was performed on 11/29/95. The sump reading was 65 3/8", the
monitoring tube water level was 6 3/8", being 25 1/8" below the top of the drainage pipe,
indicating that the trench was 65% full of water. This was the lowest reading during the
work on the site. The tank level was again 50 5/8", and 451 gallons of water was: :: '~?~>~'
introduced into the after tank clean out at a rate of approximately 1.7 gallons per minute.
It was monitored until the water rose to approximately 32", at which time the rate offlow~)
was cut back to 0.5 gallons per minute. The water level in the system did not rise while
the remaining 128 gallons (to make up the 451 gallons) was introduced.
Immediately after the test, there was 60 1/2" of water in the sump and 32" of water in the
monitoring tube. The reading, 24 hours later, at 8:12 p.m. on 11/30/95, was 66" in the
sump and 10" of water in the monitoring tube. This indicated that only 350 gallons were
absorbed over the 24-hour period; however, it was determined that since the water did not
rise for a period of approximately 2 hours, the system could be passed, but with a caution
that the system is operating within the capability of an older system, as it was installed in
1977. There is no guarantee how much longer it will last.

Dear Ken:
As you requested, in addition to the adequacy test previously
performed by this office, a well inspection was als~ performed
It was found that the well casing wa~ equipped with an~adequate
sanitary seal and that the wires from the well were adequately
protected. The ground around the well casing was adequately
sloped away from the well. At the same time this inspection
was performed, a water sample was taken from the wet bar in the
basement and submitted to Chemical andGeological Laboratories
of Alaska for analysis. The results of this test were satisfactory.
A flow test was also attempted, however, the static water level in
the well casing was below 500 feet and beyond the reach of the
probes. In talkin9 with ~he occupants of the house and having
them relate the experiences that they are encountering with
shortages of water. It appears that the well is producing less
than 15 gallons per hour. If this is the case, it would be safe
to assume that the well Xs producing less water than req~lired to
meet the consumptive needs of this residence. It is suggested
that increased storage be provided or increased production in
the well. be obtained.
