4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
must be either: A. Open and closed on the same day.
B. Covered, sealed, and heated to prevent freezing.
5. The following special provisions.
-SEPTIC TANK INSTALLATION MAYBE LOCATED NO LESS THAN FIVE FEET FROM ANY PROPERTY LINE OR
BUILDING FOUNDATION; TEN FEET FROM ANY WATER SERVICE LINE; ONE HUNDRED FEET FROM ANY
SURFACE WATER; AND ONE HUNDRED FEET FROM ANY PRIVATE WELL; AND THE SEPARATION
DISTANCES REQUIRED BY 18AAC72 FROM WATER SUPPLY WELLS.

Proposed Septic Tank Replacement
To Whom It May Concern:
Attached please find an application to install a new 1250 gallon steel septic tank at the
referenced 4 bedroom house in Anchorage. The existing septic tank, installed in 1987,
has collapsed and will be abandoned in place. The new tank will be placed more than
100 ft from any private wells. No work is proposed on the drainfield.
I am not aware of any adverse effect that this septic upgrade would have on any
adjacent properties' water and wastewater treatment needs.

Discussions have been held with Mr. Roll Strlckland of D.E.Q. ~egard. l.ng solls
information for this lot. Mr. Strlckland provlded Information on the lot across
the street (Lot 14 Bk 2) which showed gravelly sand wlth a trace to some silt
and cobbles. A rating of 85 was given for on-slte sewage whlch, corresponds to
the general rating for the area. Our own experience based on a:test hole for
Lot 19 Bk 2 indicated a rating of 125. A copy of this report Is enclosed for
reference. ~
Based on these discussions and the. existing data It was felt that on-slte
sewage wouldn't pose any problems. Also we have shown some proposed locations
on the topographlc map for seepage plts and wells. These locations meet D.E.Q.!s
criteria for separation and should enable a builder adequate room to 'Install an
on-site sewage disposal system.

in the Certificate of On-Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or wastewater

In conducting an adequacy test,I attempt to provide a thorough,conscientious engineering analysis of the system in accordance with MoA COSA
guidelines and regulations.The reported results describe the performance of the system under the conditions encountered at the time of the test,
and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil
condition,ground water levels that may fluctuate during the year,and the water usage of the family being served by the system.These conditions
are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future
performance of the system,nor do they guarantee that there are no hidden defects or encroachments.Therefore we cannot provide any warranty

Mr. Crewdson:
As we discussed via phone today, on 11/2/2011 we presoaked the drainfield on the subject
property with 2000 gallons of water (delivered in 500 gallon loads) and it remained dry
throughout the test. On 11/3/2011, an additional 500 gallons was introduced into the drainfield,
and it remained dry. Based upon this data, the drainfield absorption capacity was deemed to be
greater than 600 gallons per day.

The trench was approved in 1982 at 149 feet (see inspection report). The 2005 septic tank upgrade
permit and inspection report appear to be drawn incorrectly and do not agree with the approved 1982
installation. The 1987 septic tank upgrade inspection report shows swing-tie dimension arrows that
could be mistaken for the location of the drainfield. The important point to notice on the 1987 report is
the 27 feet of pipe replaced as noted in the remarks. That section of pipe is likely leading to the
current drainfield which was installed in 1982.
The drainfield was approved in 1982 with a total depth of 2 feet. The current COSA shows 3 feet
from existing grade, which is assumed to be due to subsequent landscaping after the drainfield was
originally installed.

Water Well Advisory
Certificate of On-Site Systems Approval (COSA) # OSC111445
During a recent COSA on-site inspection and test of the potable water
supply well on Block 1, Lot 14 of Prospect Heights subdivision, the well's
productivity was determined to be 0.74 gallons per minute. The minimum
well productivity required by this Department (AMC 15.55) for a 4-bedroom
residence is 0.42 gallons per minute. Although the subject well currently
exceeds this minimum requirement, all parties concerned are advised that the
production capacity of the well may fluctuate. Restriction of non-critical
water uses such as washing cars and watering lawns and gardens may be
required.
This advisory must be attached to all copies of the subject Certificate of On-
Site Systems Approval.
