The sizing of the system is apparently correct based on the shallow trench system with 4' of gravel beneath the drain pipe. In the event the alternate site requires development the line exiting the septic tank will be encased in 6" steel pipe in the area beneath the driveway to prevent crushing or other damage to the line. Finally, I have affixed an original stamp and signature to the soils log for Test Hole No. 3. Please advise if further information is necessary prior to issuance of the septic system permit. No systems on adjacent properties are located within 200' of the planned systems on this property. The terrain of this lot gently slopes as shown on the attached site plan. Soils encountered in the three testholes located on the lot were fairly consistent and excellent for an onsite septic system. Groundwater was not observed in the monitor tubes. The following comments are appropriate for this lot: The system, if constructed as designed, will have no adverse impacts on the wells currently in use or to be placed in the future on lots located in the area. The system, if constructed as designed, will have no adverse impact on existing septic systems in the area or those to be constructed in the future. The system, if constructed as designed, will have no adverse impact on reserved space either surface or subsurface on any lots located in the area. o The system, if constructed as designed, will have no adverse impact on drainage patterns in the area. The approved site cannot arbitrarily be changed without the written approval of this o~fice. The site depicted on the site plan (Sheet 1 of 3) attached to your'.~as-built is in the vicinity of the site designated as the reserve site. You must confirm that there is still sufficient space for the reserve site. The explanation given (in the "Remarks" section of the as-built) for changing the original site was "location moved due to surface water (Pond)." December 2, 1992 Page Two o The soil test dated May 2, 1992 at the site of the absorption field showed a percolation rate of 22 minutes per inch which has an application rate of 0.6 gpd/sf. For a five (5) bedroom house, this would require 1,250 square feet of absorption area. Based on four (4) feet of gravel below the perforated pipe, the total length of the five (5) foot wide drainfield is 125 feet. The as-built shows the combined length of the two trenches to be 96 feet. Your explanation (in the "Remarks" section of the as-built) for the 96 feet of trench was "visual rating of soil better than .6 gal". Visual soil ratings will not be approved, and the deviation from the percolation rate must be approved by this office PRIOR TO CONSTRUCTION. Assuming that you were at the site during construction and both inspections, it was your responsibility to have the construction discontinued until each of the above permit deviations were reviewed and approved by this office. By copy of this letter, the owner is hereby advised that the on-site wastewater system is not approved, and that continued operation can result in the issuance of a Notice of Violation and possibly a Citation. You are therefore strongly urged to rectify the herein defined discrepancies within the next fifteen (15 days. If there are any further questions, please call our office at 343-4744. This inves~tigation was in response to a letter from The Municipality of Anchorage, Depart- ment of Health and Human Services, dated December 2, 1992, where discrepancies between the installed septic system and the permit for the installation of the septic system were addressed. To SOmmarize: The Municipality would not approve the installation until the discrepancies were corrected. The most severe point was that the septic system did not have the physical size required for a 5-bedroom system. The total length required is 125 feet. The installed system has a 'total length of 96 feet. With other words, the system must be e~tended 50 feet. A condition that was not addressed bv the Municioality is the pond. Any septic system must be 100 f~et from an,x' surface water. I measured 75 feet +- between the septic system ~d the closest edge of the pond. I suggest that you fill in the-low area be- tween the drive way and the proper pond to obtain the proper 100 feet separation. John Smith signed off on your system based on my assurance that any deficiencies would be 2orrected. Dave and Kris Mulholland Property Dear Clair: Per the request of Mr. & Mrs. Mulholland, I performed an adequacy test on the subject well. Per the drilling log, the well is 240 feet deep, with bedrock at 38 feet. The bedrock portion of the well is lined with PVC pipe. Prior to starting the test (8/24/97) the water level was 46 feet below the top of the casing (BTC). Water was pumped at an average rate of 3.69 gpm for 80 minutes (295 gallons), which caused the level in the well to drop 143 feet, to a depth of 189 feet BTC. The flow was then reduced to an average flow rate of 1.59 gpm for the next 160 minutes, during which time the water level in the casing rose to 176 feet BTC. In short, at a flow rate of 1.59 gpm, the well was gradually recovering. Based upon this data, it can be concluded that the well production exceeds 1.5 gpm, or 2160 gallons per day. This Municipality of Anchorage requires that a well, serving a 5 bedroom house, produce at least 0.52 gpm, or 750 gallons per day.
