Ladies and Gentlemen:
I am writing to request a septic system installation permit for the above referenced property. The
existing septic system is failing and the owner would like to replace it. The proposed system will
serve a 4 -bedroom single-family residence. Soil logs, design calculations, a site plan, design
drawings and construction specifications are enclosed for your review.
Design Calcs: No groundwater observed.
(7/22/2014).
Soil Rating. From Test hole 7/15/14
1.4 min/in = 1.2 gal per sq.ft./day
No. of Bedrooms 4
Required Area per Bedroom: 15011.2 =125 sq.ft.
Total area required: 125 x 4 = 500 sq.ft.
System: We are proposing a deep trench absorption field connected to the existing system via a
diverter valve. The absorption field will consist of two trenches 25 feet long and 2 feet wide and
contain 8.5 feet (850 sq.ft. effective) of sewer rock. The trenches will be connected via a 50/50
flow splitter and double cleanouts will be installed after the tank and before the diverter valve.
All piping under the driveway will be insulated.
Soils: A Test hole was excavated on July 15, 2014. See the attached soil logs. Ground water
monitoring will continue through construction of the septic system. Groundwater was not
observed on July 22, 2014.

Surface Water: There are no surface waters within one hundred feet of the proposed septic
system upgrade.
Topography: The ground level is generally level around the entire lot.
Waivers: None
The installation of this septic system will not prevent wells and septic systems from being
installed on the adjacent lots. The proposed septic system will not change the general slope of
the area. Ponding and/or concentration of surface runoff will not result from this installation.

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of On -Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

Subject: Lot 4 Block 1 Huffman Subdivision #1
Approval for your individual sewer and water facilities
cannot be granted until the following items have been
completed:
(1) The water analysis report be delivered to this
department from Chem Lab, 5633 B Street, for
our review.
(2) Locate and expose the well for our inspection to
determine proper construction, also to insure the
minimum distance requirements are met between your
well and sewer system.
(3) A four(4) inch cast iron cleanout needs to be installed
to the sewer system.
4) The septic tank pumped with a receipt submitted to
this department.
(5) An adequacy test be performed on the existing leaching
area. This test will determine if the system is
adequate according to National Standards. A listing
o.f private firms performing the test is enclosed. This
report needs to be submitted to this department for review.
Please notify this department for reinspection when the
noted descrepancies have been corrected. If there are any
further questions, please call this department at 264-4720.
