Septic System Upgrade Permit Request
Design Narrative
This is a design narrative for a permit to install a septic system upgrade on the subject property. The proposed
upgrade will serve an existing three-bedroom (3) house with potential future plans to expand to four (4) bedrooms.
This lot and surrounding lots are served by private wells. Currently there are no wells within 100 feet of the
proposed upgrade.
1. Soils:
A test hole was performed on this lot by Pannone Engineering Services, LLC on 3/22/2023. Groundwater was not
observed during the excavation of the test hole, and bedrock was not encountered. No groundwater was observed
in the test hole monitor tube after 7-days. Based on the results of the percolation test and overall soils appearance
an application rate of 1.2 gpd/SF was used for the design of a conventional wastewater treatment system in the area
of the test hole.
2. Soil Absorption System Design.
a. See Sheet 1 of the design package.
3. Surface Water:
There is a mapped open channel drainage along the drive of the subject property. This is an ephemeral drainage and
is not considered surface water per MOA 15.65 code definition. Best management practices should be used during
installation to control sediment and storm water runoff. All disturbed areas shall be seeded after installation.

4. Topography:
Please see attached site plan for area topography. There are slopes exceeding 25% less than 50 feet down gradient
of the proposed drain field. The proposed drain field design conforms to the steep slope provisions of AMC
15.65.210.B.4.a. Exception.
The soils in this area are Hydrologic Soils Group A which contains organics underlain by a well graded to poorly
graded gravel below (SP), as shown on the soils log.
Due to the nature of and drainage capacity of the soils found in the area (HSG A) the designed infiltration line of 46%
for septic system installation on steep slopes should be used instead of the assumed/worst case soil classification
infiltration line of 25%. The slopes and soil types enable installation of the proposed system without the system
effluent daylighting at any distance of concern. Please see sheet 2 of the attached design package for the slope
detail.
5. Drawing Markings: The Drawings are marked “For MOA Review Only”. When written notification that the
review is complete and no further comments are received from MoA On-Site Department, the note will be removed
and “For Construction” drawings will be issued.
The proposed installation will not affect the future development of this or the surrounding lots.

In reference to the above mentioned lot.
We, believe Ted Rumsey installed or had the old septic crib installed.
It is unkno~nwhen it was installed, to the best of our knowledge there
were no records kept. The dimensions of the old crib are 12'x12'x9' deep,
with no sewer rock, and it appears to be in good condition.

Dear Mr. Lynn:
USKH has completed a soils investigation of the above-captioned property for the
purpose of determining suitability for the use of on-site wastewater disposal
systems. Our findings are as follows:
Elevations on the property range between approximately 561 feet above
MSL on the southwest corner to approximately 719 feet above MSL on the
northeast corner.
2. The average slope on the property is greater than 20%; some areas have
slopes greater than 25%. All test holes were located in areas wlnere
the local slope is less than Z5%.
3. Soil testing was performed using a track-mounted Nodwell auger rig.
One test hole was drilled on each lot, as shown on the attached
exhibit.

No bedrock was encountered in any of the test holes.
No groundwater was encountered in any of the test holes.
Soils log and mech4nical analyses (attached) indicate the soils
consist of sandy gravels in the GW-GM classifications. Accordingly,
no percolation tests were needed to confirm that the soils are
suitable for on-site systems.

with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.

A permit issued by this Department for an individual well
and/or on-site sewer system has expired as of December 31,
1986.
Permits are issued on a calendar year basis by authority
of Municipal Ordinance. A new permit must be obtained from
this Department for any well and/or on-site sewer system not
installed by the expiration date.
If you have drilled the well, a well log needs to be sent to
this Department for documentation of the installation and to
close the permit.
If a private engineer inspected the installation of the on-site
sewer system the original as-built inspection report (three part
form) must be sent to this office for review and approval, and
for documentation.
