The subject permit, issued April 6, 1998 by this office for a single fanfily well and/or on-
site wastewater system, has expired as of Aprii 6, 1999.
A new permit must be obtained from this office for a well and/or on-site wastewater
system NOT installed by the expiration date.
If you have drilled the well, a well log must be sent to this office for documentation of the
installation and to close the permit.
If a licensed Professional Engineer has inspected the installation of the on-site wastewater
system, the original as-built inspection report must be sent to this office for review,
approval and documentation. All inspection reports must be submitted within 30 days of
construction completion.
When applying for a new permit, the fees are: $320.00 for an on-site wastewater permit;
$120.00 for a well permit and $440.00 for a combined on-site wastewater and well permit.

We are submitting an application for the installation of an onsite waste water disposal system for this lot. The
submittal consists of three (3) drawings showing the present improvements on the lot and the adjoining properties,
(sheet 1/3), the proposed location of the waste water disposal system, (sheet 2/3), and a schematic of the system,
(sheet 3/3). Soil logs and percolation tests of applicable testholes are also enclosed.
Mr Hathhorn has obtained a waiver from ADEC with respect to the proposed Class C well on lot 6A. A copy of this
waiver is enclosed. By subdivision agreement, the well on Lot 6A is required to provide water to lot 7. We
proposed to install a bottomless Orenco Intermittent Sand Filter. The design is based on the following:
No groundwater or impervious soils to 15 feet.
Soil Rating: 4 gal/fi/day
No. of Bedrooms 4
Required Area per Bedroom: 150/4 = 37.5 sq.ft.
Total area required: 37.5 x 4 = 150 sq.ft.
Area of Sand Filter: 10 X 36 = 360 sqft
The installation of this septic system will not prevent wells from being installed on the adjacent lots.
There are no developed or natural surface / sub surface drainage courses on this or the adjacent lots.
The proposed septic system will not change the general slope of the area. Ponding and/or concentration of surface
runoff will not result from this installation.

Waiver Class C Well to On Site Wastewater Disposal System:
Lot 7, Jewel Lake Heights
Gentlemen;
The owner of Lot 7, Mr. Marvin Hawthorn, has attempted for several years to obtain a permit to develop his lot. His
effort has been in the direction of obtaining a Municipal permit for a well and septic system. The Municipality has
been unable to grant the waiver required to locate both a well and a septic system on Lot 7.
Lot 6A was developed in 1976. A recorded document filed in 1973 obligated the developer/owner of Lot 6 to provide
domestic water to Lot 7, whenever it became required. This requirement will classify the well on lot 6A as a class C
well. The septic system on Lot 6A seems to have been installed with this in mind. 1 have included information on this
septic system and the reported soil conditions.
Mr. Hathhom is looking at obtaining water from the well on Lot 6A. This will require a waiver from ADEC since
the separation distances have changed since 1973. Mr. Hathhorn intends to install a Bottomless Intermittent Sand
Filter. The ISF has been shown to continuously treat the wastewater to better than secondary treatment requirements.
Local performance data of this system should be on file with ADEC. The site specific soil conditions has not been
verified, but information from the adjoining lots strongly indicates that sand and gravels overlaid by several feet of silty
sand, or silt, will be present.
The attached siteplan shows the proposed location of the ISF as well as the location of the wells on the adjoining
properties.

Dear Mr. Spurkland:
This letter is in response to the information received in this office June 16, 1997,
requesting a waiver from the required horizontal separation distance between a Class
"C" public water source and a wastewater disposal system. The Department has
completed its review of the submitted information which included a brief cover letter,
soils log, well log, proposed site plan, and a copy of an easement for water service to
Lot 7 from Lot 6 Jewel Lake Heights Subdivision. Based upon this review it appears
that the lesser separation distance to the proposed wastewater disposal system should
have minimal contamination potential on the existing well. However, the Department
has the following comments before further review and/or approval of the requested
waiver:
A review of this office's files failed to provide verification that the existing
water system on Lot 6A was approved. If there is a health authority
approval for this water system a copy should be forwarded to this office.
A capability assessment of the existing well on Lot 6A will need to be
submitted to this office verifying that the water system can meet the
demand requirements of a class "C" public water system.

Dear Mr. Spurkland:
This letter is in response to the information received in this office September 16, 1997,
addressing this office's comments concerning a waiver from the required horizontal
separation distance between a Class "C" public water source and a wastewater
disposal system. The Department has completed its review of the submitted
information which included a brief cover letter, and copies of previous health authority
approvals from Lot 6A concerning well and septic capacity. Based upon this review it
appears that the previous concerns regarding well production and approval have been
adequately addressed. Therefore, for the concerns of this Department a waiver
granting a lesser separation distance and connection to the well on lot 6A Jewel Lake
Heights is approved subject to the following conditions:
Design calculations based upon a recent flow test showing that adequate
storage capacity is available to meet peak demand requirements for both
residences.
This water system has been assigned the following public water system
identification number 218547. Please reference this number when
submitting future water samples so that this system maybe properly
credited.
Please submit a contact name, address, and telephone number of the
person(s) responsible for operating this water system.
In accordance with 18 AAC 80.800 lead-free pipe, solder and flux are to
be used in the c¢,nstruction of this water system.
As-built drawings under the stamp ofa Pr3fessional Engineer (P.E.) of the
completed water system will need to be submitted to this office within 90
days of completion.
Attached is a Construction and Operation Certificate for Public Water Systems with
the Approval to Construct section completed verifying this Department's approval.
Operation approval of this class "C" public water system will be granted when the above
conditions are satisfied.

AMC Chapter 15.65 requires that a replacement site be identified for the absorption system. Such a replacement
site has been shown, though it is unlikely that a replacement site will be needed. A failure of a bottomless sand
filter may happen if the air supply fails. In that case a biomat may develop on top of the sand. If so, the filter will
have to be repaired by repairing/replacing the air supply and replacing the top layer of sand. A biomat will not
form at the interface between the sand filter and the natural sand.
The size of the "replacement" drainfield is based on acceptable soil between 5 and 9 feet. Total length of the
drainfield is 150/8 = 18.75 ft.
The installation of this septic system will not prevent wells from being installed on the adjacent lots.
There are no developed or natural surface / sub surface drainage courses on this or the adjacent lots.
The proposed septic system will not change the general slope of the area. Ponding and/or concentration of surface
runoff will not result from this installation.

Dear Mr. Spurkland:
This letter is in response to the information received in this office January 30, 1998,
concerning a previously issued waiver granting a lesser horizontal separation distance
between a class "C" well and a proposed on-site wastewater disposal system. The
Department has completed its review of the submitted information which included a brief
cover letter, and a proposed design of an on-site wastewater disposal system employing
an intermittent sand filter. Based upon this review and previous information on file, the
Department has the following comments in addition to those previously stated regarding
this waiver in accordance with State Drinking Water Regulations (18 AAC 80.030):
1. Verification of the local groundwater gradient, depth of the shallow
groundwater relative to the surface level of Jewel Lake. The local (shallow)
groundwater gradient is assumed to be in the direction of Jewel Lake which
is away from the well.
A minimum separation distance of 60 feet must be maintained between the
well on Lot 6A Jewel Lake Heights and the proposed septic tank for the on-
site wastewater disposal system on Lot 7. The septic tank will need to be
placed within an impermeable iiner with a nominal thickness of 25 mil.
A minimum separation distance of 75 feet must be maintained between the
soil absorption system and the class C well on Lot 6A.
The issuance of a waiver significantly less than the separation distances
outlined under the regulations (18 AAC 80.030, 18 AAC 72.015) is not
taken lightly by this Department. Jewel Lake is classified as an impacted
water body, therefore, additional treatment of the septic effluent to remove
suspended solids, nitrate, and bacteria prior to a subsurface discharge
should be considered. A lesser separation distance than what is outlined
above in condition number three may be considered if an on-site
wastewater disposal system capable of providing additional treatment such
as a trickling filter and intermittent sand filter with a final discharge into a
soil absorption system is implemented.

Enclosed is a recent survey of subject lot showing the well on Lot 6A, the location of the test hole
on lot 7 and the location of the shore'line. The survey shows that the distance between the well and
the shoreline is 195 feet. A 75 foot waiver to the well and 100 feet separation to the shore line
leaves 20 feet for the 8 foot wide sandfilter. Your concern regarding sufficient area to construct the
system should be satisfied.

Dear Mr. Spurkland:
I received the survey conducted by Land Surveyor Jeffery Gastaldi dated March 11, 1998.
The purpose of a survey at this point is to establish the mean annual high water mark of
Jewel Lake. This survey only locates an 'approximate shoreline' which is further
explained in the Legend as ".. approximate due to snow cover at the time of this survey."
In order to determine the mean annual high water mark of Jewel Lake, a survey must be
conducted after the ice has receded from the shoreline, preferably in May or June.

Your request for waiver of the required 100 feet separation from
a septic system to a well has been denied. The waiver requested
was for a separation distance of 65 feet from the well on lot 6A
to a proposed septic absorption field on lot 7.
This'office will not issue a permit for construction of a
wastewater disposal system on an undeveloped lot unless the
conditions as outlined in the Anchorage Municipal Code,
Wastewater Disposal Regulations, 15.65.060, Subsection A, exist.
