Dear Cindy Ellis:
Your request for a waiver of the required 100 feet horizontal separation from the
septic tank to private well has been approved. The approved separation distance is 70.0
feet. This waiver applies to Lot 16 Well to Lot 6&15 Septic Tank only.
This waiver approval applies to the existing septic tank to private well separation only.
Any future upgrade to the on-site wastewater disposal system will require all separation
distances be met or another approval from this department.
If there are any further concerns or questions regarding this waiver, please call our office
at 343-7904.

Seatic Tank Data
The septic tank on John Wells S/D Block 5 Lot 6&15 was installed in
1982. The permit was issued by the Onsite department however the
tank was placed 70' away from the well located on Lot 16. The tank
and leachfield installed in 1982 that encroached three wells has been
in existence for over 20 years. Watkins Engineering took water
samples for each of the encroaching wells. And all three tested with-
in Municipal standards.
Well Data
John Wells S/D Block 5 Lot 16 has a well log. Watkins Engineering
during their Health Authority Inspection determined water samples
tested to be 0 coliform, .193 mg/I nitrates, and 0 other bacteria. Lot
16 well log shows the well was done by M -W Drilling. The well log
shows soil materials similar to other well logs such as depth of well
and static water level and well production. however the Lot 16 well
log doesn't show the clay and hard pan that all the other well logs
show in the area. It was determined by the on-site department that
the well log 16 doesn't show accuratelt the hard pan that all the other
well logs show. Other well logs in the area shows there is a confined
aquifer with an impermeable layer of blue clay and hardpan at 40 ft to
140ft. Using averages and Lot 18 well log I determined 83 feet of
impermeable layer.

Septic System Adequacy Test
5-104
I hereby certify that I have made an adequacy test of the existing septic
system on Lot 16 Block 5 John Wells Subdivision.
The system consists of a 1000 gal. septic tank followed by a 12' x 12'
x 10' leach pit serving a two bedroom dwelling.
The test consisted of surcharging the system with 600 gallons of water
and determining the rate of percolation by measurement of the falling
surface of water in the leach pit.
The percolation capacity is 1.1 gallon per minute. No appreciable sludge
build-up is present in the septic tank.
The system is adequate.

Subject: Lots 5 & 1.6 Block 5 John Wells Subdivision
Approval for your individual sewer and water facilities
cannot be granted until the following items have been
completed:
(1) The water analysis report be delivered to this
department from Chem Lab, 5633 B Street, for
our review.
(2) A four(4) inch cast iron cleanout needs to be installed
to the septic tank and/or leaching area.
(3) The septic tank pumped with a receipt submitted to
this department.
(4) An adequacy test be performed on the existing leaching
area. This test will determine if the system is
adequate according to National Standards. A listing of
private firms performing the test is enclosed. This
report needs to be submitted to this department for our
review.
