Well and Septic System As -Built
The absorption trench for tot 5, Block 3, Denali View Subdivision was placed in July of
2003. At the time of construction the contractor requested a 5' wide absorption trench
in lieu of the designed 15' wide shallow bed to facilitate construction. The slope of the
lot is more conducive to trench construction and trenches typically have longer useful
lives than beds. We have no verification or documentation indicating the design
change was approved by the City, but we recall verbal discussions concerning the
subject. A fee of $115.00 has been paid for your review of the design change.
The well is located approximately 55' northwest of the design location. We are unsure
why the well driller placed the well at this location other than the ease of access. We
were not notified of the location change prior to the construction of the well. Placement
of the well in its current location more closely shadows the well to the west will have no
adverse affect on the well or septic system on this lot.

An On -Site Water/Wastewater Permit, number SW030092 issued by this office for a single-family
system, will expire on April 21, 2004. This permit was valid for 365 calendar days.
If this was a well permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be sent to
this office for review, approval and documentation. This as -built inspection report must be signed by
the licensed Professional Engineer who inspected the installation of the system. As -built inspection
reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A new permit for a second year may
be issued for a fee of $115.00 if the renewal application is received on or before the expiration date of
the original permit.
When applying for a new permit, the fees are: $460.00 for a wastewater permit and $175.00 for a well
permit.

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The owner of Lot 5, Block 3, Denali View Subdivision intends to construct a four-
bedroom home on the lot. We are therefore requesting a permit be issued for the
construction of a new well and septic system to serve the home. The attached Site
Plan and backup documentation identify the location and configuration of the well and
the new septic system and altemate site and the parameters used in the design. Also
identified on the plans are the test hole locations and well and septic systems on the
surrounding lots. Drainage arrows and contours are shown indicating the current
drainage pattems. The drainage patterns will be maintained after construction.
Shallow bedrock and wells on adjacent Tots minimize the area available for placement
of a septic system. We are therefore proposing to construct the Advantex Treatment
System with an AX20 Mode 1A 40x94 Pod for this lot.
The test holes placed near the location of the proposed absorption bed indicated clean
gravel with varying amounts of silt and a percolation rate of 8.6 minutes per inch. No
ground water was found during placement of the test holes and none was noted during
the monitoring period. Bedrock, however, was encountered at 9' below the surface.
We are therefore proposing to place the Advantex System and a new 30' long by 10'
wide absorption bed. The total depth of the trench will be 2' below the surface with the
distribution pipe at 1.5' below the surface. Sufficient fill will be placed to prevent
freezing of either the septic tank or the absorption trench.
The ground surface on the lot slopes as shown on the site plan from south to north at
varying slopes. The absorption bed will be constructed parallel with the slope as much
as possible in conformance with Municipal requirements. All components of the new
septic system will be constructed a minimum of 100' from any surface water and wells
in the area.
If the system is constructed in accordance with our design the following statements
apply:

1. The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattem will be maintained.

ON-SITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 5, BLOCK 3, DENALI VIEW SUBDIVISION
GENERAL:
1. The scope of this project includes the procurement and placement of a
new Advantex Treatment System with an AX -20 -Mode la 40x94 Pod
at the location shown on the Site Plan. The new septic system also
includes the construction of a new 30' long X 10' wide absorption bed
at the location also shown on the Site Plan and at the configuration
shown. The distribution line in the bed will be placed at 1.5' below the
existing ground surface. The total depth of the bed will be 2' below
existing ground. The new system must be placed a minimum of 100'
from the proposed well on the lot and those on adjacent lots and 10'
from the water service line between the well and the house. It must
also be placed a minimum of 100' from any surface water.
2. Construction shall be in accordance with the approved site plan,
design drawings, Municipal Permit with any special provisions or
conditions, and all applicable State and Municipal Wastewater
Disposal Regulations.
3. The Contractor shall be responsible for obtaining all underground utility
locates and for the layout of the septic system and verification of the
location of all lot lines.
4. Unless specifically agreed otherwise, the contractor shall be
responsible for final grading areas subsequently depressed from soil
settling. Property owner shall be responsible for revegetation of
affected areas unless specifically agreed otherwise.
5. Contractors installing wastewater disposal systems must be certified
by the Municipal Department of Health and Human Services for
system installations. Owners installing their own systems must receive
prior approval from D.H.H.S. before beginning system installation.
SEPTIC TANK INSTALLATION
1. The Advantex Treatment System must be installed in conformance
with specifications and regulations fumished by the manufacturer,
Anchorage Tank and Orenco Systems Incorporated. No variances
from the manufacturer's specifications will be allowed without
permission from the manufacturer and Municipal representatives.
2. A septic tank is to be constructed by a certified septic tank
manufacturer. Construction shall include two 4" cleanouts for pumping
access.
3. The septic tank shall be sufficiently bedded to prevent settling or
shifting of the tank.
4. All standpipes on the septic tank shall extend a minimum of 12 inches
above final grade.

7. Final grading over the tank shall be such that a positive slope exists
away from the septic tank.
DRAINFIELD CONSTRUCTION:
1. The drainfield shall be constructed to the dimensions shown on the
design. The bottom of the bed shall be within 2" of level.
2. Distribution piping must be placed level with perforations down atop a
level bed of drainfield rock. Rock should then be placed over the pipe
to provide a minimum of 2" of cover.
3. A silt barrier or geotextile fabric must be placed between the drainfield
rock and the natural soil backfill.
4. Monitor tubes must be 4" in diameter and installed at the locations
shown on the design. The portion below ground must be perforated.
5. Contractor shall verify the septic tank and drainfield are a minimum
100' away from any private water wells in the area, 150' from a Class
"C" Well or 200' from any community well.
6. Direct bury insulation must be placed over the distribution system if
Tess than 3' of backfill depth is available. Finish grade over the trench
must be mounded to prevent settlement or depressions.
7. Grade area surrounding the absorption trench to drain away.
8. A minimum 2' of accepting soil is required below the drainfield rock for
a 5' wide trench. Contractor shall verify this condition prior to
placement of the rock. AU pockets of unacceptable materials must be
removed and replaced.
.MATERIAL SPECIFICATIONS:
1. Septic tanks must be constructed by a municipally approved septic
tank manufacturer.
2. The following pipe materials are approved for use in septic system
installations in the Municipality of Anchorage:
Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated
and solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and
ASTM D2662 or A.B.S. (perforated and solid).
3. Insulation shall be at least 2" thick extruded direct burial polystyrene
(Dow Chemical Co. Styrofoam HI or equal).

4. Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Femco, or equal).
5. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed
between the final drain rock layer and the native soil layer.
6. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing
the #200 sieve.
INSPECTIONS:
Municipal Ordinance requires a minimum of two inspections. These
inspections must be conducted under the supervision of a professional
engineer registered in the State of Alaska. The first inspection must be
conducted after the excavation of trenches, beds or pits and before the
installation of any gravel. A septic tank may be set in place, but may not
be backfilled.
The second inspection must be conducted after the placement of the
geotextile fabric, gravel, distribution piping, standpipes, cleanouts and
insulation. No backfill should be in place at the time of inspection.
Contractor shall provide a copy of all field survey layout and construction
notes for use in preparing the certified as -built of the completed system.

"OWNER," and the Municipality of Anchorage, herein the "MUNICIPALITY." In
consideration of the mutual covenants contained herein, the parties to this Memorandum of
Understanding agree as follows:
1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality grants
permission to Owner to utilize and operate an Advanced yip ewater Treatment System

Anchorage, Alaska.
2. Definitions.
A. Alteration. Any change to the design or function of an AWWTS that
includes the installation or removal of any parts, components or pieces
not included in the original construction permit and design. Prior to
performing any alterations to an AWWTS the owner must obtain a
Wastewater Disposal System Construction Permit from the Municipality
pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65
B. Certificate of On -Site Systems Approval. An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with AMC

15.65. These approvals certify that the systems are adequate for the
homes that they support and meet the codes that were in place at the time
of system construction.
C. Damage. Any man-made or natural change in a system that would
inhibit the system from performing as designed.
D. Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AWWTS that were included
in the original design which would allow the AWWTS to continue to
perform as designed.
E. Permit (Construction). An On -Site Wastewater Disposal System
Construction Permit as defined by AMC 15.65.
F. Permit (Operating). An Advanced Wastewater Treatment System
Operating Permit. An annual permit, issued by the Municipality, that
allows the Owner to operate an AWWTS, upon meeting all requirements
of this agreement, the conditions of the Operating Permit, the
requirements of the On -Site Wastewater System Construction Permit
and all relevant provisions of AMC 15.65.
3. Fee. Owner shall pay to Municipality an annual fee of
(5 .00), payable on or before the issuance of the operating permit and annually
thereafter. The annual fee is due on or before the anniversary date of the approval by the
Municipality of installed system.

4. Term. The term of this Memorandum of Understanding shall be for the life of the
AWWTS. The term begins on the date of approval by the Municipality of the installed system
and shall continue while the AWWTS system is in use or operational or until the property is
sold or title is transferred by owner and a new certificate of on site approval is issued to the
new owner or transferee of the property.
5. Alterations, Installation and Removal of Additional Equipment. Owner agrees not
to make any alterations, removal of parts or additions to the AWWTS without a Construction
Permit from the Municipality.
6. Maintenance and Repairs.
A. Throughout the term of this Memorandum of Understanding, the Owner shall
maintain AWWTS in good repair. In addition, it shall be the responsibility of the Owner
•
during the term of this Memorandum of Understanding, and any extensions or renewals
thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3)
adjustment(s), (4) replacement costs, and (5) inspection costs. Further, owner agrees to
comply with all applicable ordinances, laws, regulations, rules and orders for the AWWTS.
B. Owner agrees to provide the Municipality a written schedule of routine
maintenance and repairs which have been performed on the system pursuant to the terms and
conditions contained in the Owner's AWWTS Operating Perrnit. This schedule shall be
submitted to the Municipality annually upon the renewal of the permit. The schedule of
maintenance and repair contained in the Owner's AWWTS Operating Permit is:

C. Owner acknowledges that the fine schedule for failing to maintain and repair an
AWWTS are codified in AMC 14.60.
D. Owner agrees that only maintenance, repair personnel certified by the
Municipality will inspect and make any necessary maintenance, repairs or permitted
alterations to the systems.
E. Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
F. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On -Site Systems Approval.
G. Owner agrees that the relevant provisions of the standard specification
guidebook for AWWTS is the governing professional guidelines for the construction,
maintenance and repair of the Owner's AWWTS.
7. Nonwaiver. The failure of either party at any time to enforce a provision of this
Memorandum of Understanding shall in no way constitute a waiver of the provisions, nor in
any way affect the validity of this Memorandum of Understanding or any part hereof, or the
right of such party thereafter to enforce each and every provision hereof.
8. Amendment.
A. This Memorandum of Understanding shall only be amended, modified or
changed by a writing, executed by authorized representatives of the parties, with the same
formality as this Memorandum of Understanding was executed and such writing shall be
attached to this Memorandum of Understanding as an amendment.

B. For the purposes of any amendment modification or change to the terms and
conditions of this contract, the only authorized representatives of the parties are:

C. Any attempt to amend, modify, or change this contract by either an
unauthorized representative or unauthorized means shall be void.
9. Jurisdiction: Choice of Law. Any civil action arising from this Memorandum of
Understanding shall be brought in the Superior Court for the Third Judicial District of the
State of Alaska at Anchorage. The laws of the State of Alaska shall govern the rights and
obligations of the parties under this Memorandum of Understanding.
10. Severability. Any provisions of this Memorandum of Understanding decreed invalid
by a court of competent jurisdiction shall not invalidate the remaining provisions of the
Memorandum of Understanding.
