I. The scope of this project includ~ the installation of a 1000
gallon septic tank and a l~achfi~ld trench to s~rve the 3
bedroom r~sidence located on the referenced property.
2. Construction shall be in accordance with the approved site plan
and d~sign drawings, M~nicipal p~rmit with any special
provisions or conditions, and all applicab£e State and
Municipal Wast~wat~r Disposal Regulations.
5. The. contractor shall be r~sponsible for obtaining any necessary
underground utility lo~s.

SEPTIC TANK INSTALLATION:
I. A septic tank is to be constructed by a czrt~fied septic tank
man~f, act~r~r. Construction shall include ~o 4" cl~anou~ for
p~mprng access.
2. The septic tank shall b~ sufficiently bedded to pr~ven~
s~ng or shifting of th~ tank.
All standpipes on the septic tank shall ~xtend a minimum of 12
inch~ abov~ final grade.

I. Ex,rate the proposed trench to the dimensions shown on the d~sign. The
bottom of the excavation shall be within 2 inch~ of level. If the
sidewalls of the excavation become smeared, they must be raked or
s~rat~hed (roughed-up) before grav~ (s~w~r rock) placement.
2. Once the gravel is installed, the distribution pipe is to be installed
level with th~ p~for~o~ faced dow~d. G~v~ ~ ~ to 6~ p~c~d
ov~ ~ ~6~on pip~ to provid~ a mi~ of 2 inch~ of cov~
ov~ th~ pip~.
3. A silt barrier must be installed b~tween the final gravel layer and the
native soil backfill. Ensure the silt barrier covers the entire gravel
surface before pla~ing backfill.
4. Monitor tubes shall be of four (4) inch diameter and installed
approximately in the lo~atio~ shown on the d~sign. The portion of the
monitoring tube extending through the gravel shall be p~rforated from
the bottom of the trench to the inv~t of the distribution pipe. This
is equiv~ent to the effe~£ive depth of the gravel as noted on the

6. All leachfield gravel (sewer rock) sha~ be 0.5"-2.5" s~reened gravel
with less than 3% passing the #200 sieve.
7. When sand is being ~sed as a filter material, its gradation
specifications must conform to c~rrent M.O.A. or D.E.C. requirements.
Typi~a~y there will be a rain/mum of three (3) inspections required during the
installa~on of the wastewater disposal system. Th~se inspections wi~ occur as
follows:
I. The first inspection must be conducted after the excavation of ditches,
pits, trenches, or beds and before the installation of any gravel. A
septic tank may be s~ in place, but may not be ba~kf~led before this
insp¢~£ion.
2. The second inspe~'.ion must be conduced after the placement of the si~t
barrier, gravel, ~.d~.'.,~'~J~ib~tion lines, standpipes, ~anouts, and
insulation, but before the placement of any other ba~kf~.

2. Construction shall be in accordance with the approved site plan
and design drawings, Municipal p~rmit with any special
provisions or conditions, and all applicable State and
Municipal Wast~wat~r Disposal Regulations.
$. The contractor shall bg r~sponsible for obtaining any necessary
underground utility locates.
4. Unless specifically agreed otherwise, the property owner shall
be responsible for final grading ar~as subsequently depressed
from soil s~tt~ing.
5. Contractors installing wast~wat~r disposal systems must be
ceAtified by the Municipal Health Department for syst~
installations. 0whirs insta~ng their own systems must also
receive prior approval from the Municipal Health Department.
SEPTIC TANK INSTALLATION:
I. A septic tank is to be constructed by a certified septic tank
manufacturer. Construction shall include two 4" cleanouts for
pumping access.
2. The septic tank shall be sufficiently bedded to prevent
s~ttling or shifting of the tank.
3. All standpipes on the septic tank shall extend a minimum of 12
inches above, final grade.

building foundation. In the line between the tank and the leachfi~ld
there shall be two adjacent cl~anouts (unless an effluent pumping system
exists within the septic tank). These ~l~anouts shall be located on
undisturbed soil not more than 10' from the tank. The first cl~anout,
in line, shall be to ~l~an toward the l~achfi~ld. The second cleanout
shall be to clean toward the, septic tank.
6. Final grading over the septic tank shall be such that a positive slope
exists away from the septic tank.

I. Excavate the proposed trench to the dimensions shown on the d~sign. The,
bottom of the excavation shall be within 2 inch~s of level. If the
sidewalls of the excavation become smeared, they must be raked or
scratched (roughed-up) before gravel (s~w~r rock) placement.
2. Once the gravel is installed, the distribution pipe is to be installed
level with the p~rforations faced downward. Gravel is then to be placed
over the distribution pipe to provide a minimum of 2 inch~s of cov~
over the pipe.
3. A silt barrier must be installed b~tween the final gravel layer and the
native soil backfill. Ensure the silt barrier covers the entire gravel
s~rface before placing ~ackf~.
4. Monitor tub~s shall be of four (4) inch diameter and installed
approximately in the locations shown on the design. The portion of the
monitoring tube extending through the gravel shall be perforated from
the bottom of the trench to the invert of the distribution pipe. This
is equivalent to the effective depth of the gravel as noted on the
design.
5. Backfill over the final grav~ layer must not be less than twenty-four
(24) inch~s. Insulation must be installed when the backfill depth is
l~ss than thirty-six (36) inches. The finish grade over the trench must
be mounded to prevent the formation of a depression after s~ling.

Typically th~r~ will be a minimum of three. (3) inspections r~quir~d during the
installation of the wast~wat~r disposal syst~. These inspections will occur as
follows:

3. The final inspection is to occur upon final grading of the property.
Often there will be more than these 3 inspections required, ~specially with the
installation of multiple trench~s, sand filters, pressurized distribution
systems, etc. Thus, the inspecting engineer is to be contacted at least 24
hours prior to the start of construction. If necessary, a pre-construction
meeting will take place on-site. The inspecting engineer will not coordinate,
direct or control in any way the contractor's activities.
The owner shall contra~t with the contractor to perform the work outlined in
th~se specifications and plans and in accordance wi~h the attached M.0.A.
p~rmit. There will be no contractual arrangement existing b~tween the
contractor and S ~ S Engineering. S & S Engineering shall be the owner's
representative and will inspect the work as stated above to document the
contractor's a~tivities. Final acceptance of the contractor's work r~sts with
the owner and the M.O.A.
S & S Engineering shall have no ~iability to the owner or to others for acts or
omissions of the contractor or any other p~rsons p~rforming work on this project
or the failure of the contractor to carry out the work in accordance with th~se
construction documents. S & S Engine~ring's inspecting engineer will not be
r~sponsible for the construction m~ans, m~thods, techniques, sequence,
procedures or the safety precautions incident to this project.

The subject permit, issued October 16, 1992 by this office for a
single family well and/or on-site wastewater system, has
expired as of October 16, 1993.
A new permit must be obtained from this office for a well
and/or on-site wastewater system NOT installed by the
expiration date.
If you have drilled the well, a well log must be sent to
this office for documentation of the installation and to
close the permit.
If a licensed Professional Engineer has inspected the
installation of the on-site wastewater system, the original
as-built inspection report must be sent to this office for
review, approval and documentation. All inspection reports
must be submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $320.00 for an
on-site wastewater permit; $120.00 for a well permit and
$440.00 for a combined on-site wastewater and well permit.
