Robinson, Civil Engineer, On-site Services~~ Department of Health and Human Services SUBJECT: S-8941: Drake Subdivision - Amended Comments Based on additional information and data received from Corwin and Associates, Inc. the six comments in our memQrandum dated June 20, 1990, are amended to read as follows~ The sites for both wells (Lots 4A, 4B) must be designated on the approved plat at the same locations indicated on Corwin's "Sewer System Location Plan" dated 6-20-90. (Copy attached.) The location of both driveways must be designated on the plat at the same locations indicated on Corwin's "Sewer System Location Plan" dated 6-20-90. This is imperative so as to insure that the required sewer reserve area is available and usable. Robinson, On-site Se Department of Health and Human Services / SUBJECT: Request for Comments on Subdivisions July 6, 1990 The Environmental Services Division, On-site Services Section, of the Department of Health and Human Services has reviewed the following case and has thes~ comments: S-8941: Lots 4A, 4B Drake Subdivision Topographic detail (contour lines) should be on a copy of the proposed plat (drawn to scale) and the required "total reserved space" for the original and two replacement on-site wastewater disposal systems specifically identified and delineated. The photocopy of the contours submitted is not to scale and the various ground slopes cannot be determined. The "Sewer System Location Plan" submitted is not satisfactory for the following reasons: 1. There are three (3) proposed well locations on the two proposed lots with at least three (3) rather vague 100 foot protective well radii shown. 2. The required "reserved space" shown is vague and poorly defined. 3. The areas shown as "reserved space" cannot be o o utilized unless comments are placed on the , /. recorded plat specifically locating the driveway access to both lots and building sites thereon. The street alongside the north line of the proposed Lot 4A is shown to be Mallard Avenue whereas it is shown to be Merganser Avenue on the proposed subdivision plat. There are no well sites shown for existing I~ lots 5B, 5C, 5D, 34 and 37. The well sites ~ must be shown along with their respective protective radii. The test hole sites must be shown.
