Based on this information, DHHS has no objection to the plat, provided the following are required as conditions of final plat: 1o Properly abandon the existing sewer system. 2. Abandon the existng well as per the engineer's . specifications. We will require proof from the engineer that the work was done. 3. Provide scaled designs for all three sewer installations on Lot 3A. These designs must be surveyed and shown on the final plat. 4. Plat both well sites° 5. Provide water monitoring results through October. The topographic map submitted with the ~ application does not identify the plat area. We request that the engineer verify the slope in the area of the proposed on-site wastewater designs. The engineer must provide additional testholes on Lot 3A so that the three designs shown meet installation requirements. We request at least one additional testhole on the east side of the reserve area of Lot 3B to establish the water table level and verify that all portions of this area are suitable for wastewater disposal. During the field inspection this office found a pond of water immediately to the south of the existing sewer system. We request the engineer evaluate this low area to determine if this is permanent surface water. Provide information on drainage from these lots. We will require water monitoring through October. The existing sewer system is encroaching on ground water and must be abandoned. As proposed, the new sewer systems will overlay the existing well. We will require that the engineer submit to us a plan for the proper abandonment of this well, including any special procedures planned for Department of Economic Development and Planning June 25f 1992 Page Two S-9266: Lots 3A, 3B Block 4 Drake Subdivision - Continued the absolute protection of the potable water aquifer° We will require~ th~bedrooms be limited and design sites shown on final plat for Lot 3Ao Lot 3B will need a redesign of the reserve area to allow for a driveway to the lot. As submittedf we are unable to support this plato We recommend it be returned to the applicant for additional information and resolution of problems.
