Our Work Order No. 77842 Dear Ms. Allen: Attached herewith is the test hole boring log, perculation test results and our comments regarding soil conditions encountered on the subject lot. In accordance with your request, our investiga- tion was conducted on August 30 and 31, 1977. A single test hole was placed on the lot as indicated on the attached location sketch. Drilling was accomplished utilizing a mobile drill rig B-50 mounted on a track vehicle owned and operated by Denali Drilling. The purpose of the investigation was to provide soils data for establishing the feasibility of onsite disposal of sanitary wastes. The test hole was extended to a total depth of 16 feet below the existing ground surface. No ground water table was encountered in the test hole, in addition, the soils near the bottom of the hole were relatively dry. Soils were as indicated on the attached boring log. After completion of the drilling, the test hole was backfilled and hand tamped to approximately the 12 foot level and filled with water. Approximately 24 hours later, the test hole was refilled with water to approximately the 6 foot level and a perculation test conducted in accordance with the manual of septic tank practice of the U.S.P.H.S. Measured drop in the water level during the last 30 minutes of the test was 6.75 inches for a computed perculation rate of 4.4 minutes per inch. SUBJECT: Lot 9, Block 1, Conifer Heights Subdivision Certificate of Health Authority Approval We inspected the well head on Lot 9, Block 1, Conifer Heights Subdivision on Tuesday, March 11, 1997. A sanitary seal has been installed and the wiring is now in conduit. The Certificate of Health Authority Approval can now be issued. Please let me know if you have any questions or comments. February 28, 1997 Municipality of Anchorage Department of Heath & Human Services 825 "L" Street Anchorage, AK 99502-0650 Attention: On Site Services Engineer Subject: Lot 9, Block 1, Conifer Heights Subdivision Certificate of Health Authority Approval Dear Reviewer: The owner of the subject property is currently refinancing her home and the bank is requiring a Certificate of Health Authority Approval. The attached package is submitted for your review. Please note the well does not currently have a sanitary seal or wiring encased in conduit. The owner, however, has assured me this will be completed by early next week and should not delay the processing of the certificate. We will verify the repairs have been made and notify you. The septic system on the lot was constructed in 1977 and is nearing the end of its useful life. A substantial buildup of sludge was noted in the system during our initial inspection which appeared to be clogging the distribution line. An initial test on the system indicated' a very high absorption rate. The water level during the test, however, rose above the invert of the distribution line. The sludge buildup was removed from the system and the absorption trench allowed to stabilize over a one month period. A second adequacy test was then performed. The water level in the trench was measured at 60" prior to the start of the second adequacy test. 'Water was introduced into the trench at a slow rate to assure the level did not rise above the distribution piping. At no time during the test did the water level rise above the invert of the distribution pipe. The septic system remained in service during the test. Lot 9, Block 1, Conifer Heights Subdivision February 28, '1997 Page Two A total of more than 600 gallons of water was pumped from the well directly in to the absorption trench. During the test period a minimum of 300 gallons was added to the system through household use. After 24 hours the water level did not return to its pre test level, but nearly 600 gallons had been absorbed by the system. The system continued in use after the test. The water level in the trench was measured at 60" during our next visit to the site which was approximately 2 days later. The owner of the property has been informed the system is nearing the end of its useful life and no guarantee as to how long the system will last has been made as a result of our test.
