It is also located inside the 100' protective radius of the well on the lot. We are proposing to replace the tank with a new 1,000 -gallon steel septic tank to serve the three-bedroom home on the lot. We are therefore requesting a permit be issued for the construction of a new septic tank to serve the home. The attached Site Plan and backup documentation identify the location and configuration of the existing septic system and the location of the new tank. Also identified on the plans is the existing well on the lot. The existing tank on the lot will be properly disposed in accordance with Municipal Code. The drainage patterns on the property will be maintained after construction. The ground surface on the lot slopes to the southeast at a 2% grade in the area of the new tank. The tank will be placed in the same location as the existing tank at nearly the same elevation. It will then be tied in to the existing service line. The tank will be constructed parallel with the contours of the land as much as possible in conformance with Municipal requirements. If the tank is constructed in accordance with our design the following statements apply: The tank, if constructed as designed, will have no adverse impact on the wells in the area or those to be constructed in the future. 2. The tank, if constructed as designed, will have no adverse impact on existing septic systems in the area or those to be constructed in the future. Lot 6, Block 3, Conifer Heights June 26, 2012 Page 2 of 2 3. The tank, if constructed as designed, will have no adverse impact on reserve space, either surface or subsurface, on any lots located in the area. 4. The tank, if constructed as designed, will have no adverse impact on drainage patterns in the area. The current drainage pattern will be maintained. Sincerely, Michael E. Anderson, P.E. Work also includes the procurement and placement of a new 1,000 -gallon septic tank at the location shown on the Site Plan. Work also includes the placement of a foundation cleanout and placement of two new post tank cleanouts. The new delivery line must then be tied into the existing outfall line to the absorption trench. The new septic tank must be placed 10' from all water service lines in the area and 100' from any surface water in the area. A minimum of 4' of cover is required over the septic tank. The new tank must be placed outside the 100' protective radius of the well on the lot. 2. Construction shall be in accordance with the approved site plan, design drawings, Municipal Permit with any special provisions or conditions, and all applicable State and Municipal Wastewater Disposal Regulations. 3. The Contractor shall be responsible for obtaining all underground utility locates and for the layout of the septic system and verification of the location of all lot lines. The Contractor must also verify the location of wells in the area and assure that the separation distances are met. 4. Unless specifically agreed otherwise, the contractor shall be responsible for final grading areas subsequently depressed from soil settling. Property owner shall be responsible for revegetation of affected areas unless specifically agreed otherwise. 5. Contractors installing wastewater disposal systems must be certified by the Municipal Development Services Department Building Safety Division On -Site Water and Wastewater Program for system installations. Owners installing their own systems must receive prior approval from the On -Site Water and Wastewater Program before beginning system installation. SEPTIC TANK INSTALLATION A new 1,000 -gallon septic tank must be procured from an approved source and installed at the location shown. 2. SIGNED: ISSUED B ...... The scope of this project is the investigation for suitability of an on-site sewerage system. Included in this transmittal are: Test Hole Location Sketch Figure 1 Test Hole Log Table A Explanatory Information Sheets 1-3 The exploration was conducted with a Nodwell mounted B-50 drill rig using a continuous flight solid stem auger. The rig is owned and operated by Denali Drilling, Inc. Drilling was supervised, the test hole logged and percolation test performed by Mr. O.M. Hatch, geologist for Alaska Testlab. The log of the test hole is included as Table A of this report. In interpreting the log, it would be helpful to utilize the explanatory information contained in Sheets 1-3 of the report. When drilling was completed a 3/4" slotted PVC pipe was inserted in the hole to aid in determining the free water level. For the percolation test, the test hole was filled with water and left overnight to saturate. On returning the next day, the hole was refilled with water and the drop in the water level carefully monitored over the next. 60 minutes. This procedure is not a standardized percolation test, however, we understand that the Municipality of Anchorage, Department of Public Health and Environmental Protection prefers tests performed in this manner to evaluate a site for a proposed on-site sewerage system. Mr. Don Decker March 6, 1978 Page 2 Using the above test, the observed minimum percolation rate was 6.7 minutes/inch. This corresponds to a required absorp- tion trench area of 140 square feet/bedroom as we understand local requirements. No water table was observed during drilling. It should be noted though, that the free water level normally fluctuates seasonally and with precipitation. We hope the above meets your present needs. If we can be of further service, please feel free to contact us. Very sincerely, ALASKA TESTLAB . Mark Hansen RMH:rb Enclosures P. OVED : Melvin R. Nichols, P.E.
