All construction shall be in accordance with:
1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing
Special Provisions:
Q� Due to the organics in the accepting soil, the MOA inspector is required to be at the bottom of hole trench
inspection. If he determines that there is too much organics then work will stop, and a change order will
be required.

Change Order
Dear On -Site Services Engineer:
During the construction of the new absorption trench for Contour Acres #3 Block 3 Lot 5 it was
discovered that the previously permitted design would place the absorption area in unfavorable
soils. The soils consisted of a large amount of fill in one portion of the trench gradually becoming
a sandy mixture with high silt content in the other. The represented a large variance in the
absorption rates. This contrast led to the decision to dig an additional test hole. The new test hole
was moved closer the west property line where a more favorable soil profile was observed. We are
proposing that the design and location of the absorption trench are adjusted to utilize the better
soils.
Please refer to the attached soils log, redesigned septic plan, and profile pages for the
recommended adjustments.

Septic system design
Dear On-Site Services Engineer:
The owner of the above lot has a septic system that has failed, so we are submitting this permit
application for the installation of a new tank and field. The attached site plan identifies the location
of the existing 3-bedroom home and well as well as the proposed septic location. No conflicts exist
between this proposed system and any other well or septic system, whether on this lot or adjacent
lots.
The ground surface on the slopes moderately toward the back of the lot, between 10%-20%, but
does not exceed 25%. Contours are shown on the site plan showing the grade and direction of flow.
Stormwater drainage will not impact this septic system. The new trench will be constructed parallel
to the slope as much as possible.
The trench is located in a soil layer that perks less than 1 min/in, but is not gravel. Instead, it
appears to be composed primarily of silty/sandy peat with some small amounts of larger organic
matter mixed in. Due to the possible compressibility of this layer, we have set the bottom of the
trench on the layer below, which appears to be a mix of silt and clay. We have submitted samples
to the lab for both layers, for the upper layer to prove that it’s not gravel, and for the lower layer
to prove that it’s not clay (and therefore impermeable).
Wells on this and adjacent lots are shown. The new system will be a minimum of 100’ from all
wells and surface water, and more than 5’ away from the septic tank. Please refer to the attached
test hole logs, soils lab analysis (currently pending), plan and profile pages for the septic design.
If this design is followed, there will be no adverse impacts to adjacent properties.

REFERENCE: Lot 5; Block 3; Contour Acres Subdivision
Request yo~ expedite issuance of a permit to upgrade the septic system
serving the four bedroom house on the referenced property.
A test hole was excavated and a percolation test performed in the area
of the proposed upgrade. The approximate location of the test hole is
located on the attached site plan. The monitoring tube within the test
hole has been checked and found to be dry. Attached is the proposed
upgrade design.
There are no points of contamination within the proposed well radius
which can be seen on the attached site plan.
We do not anticipate any adverse effects on neighboring properties by
the installation of the proposed septic system.
During a~ inspection of the existing septic system, effluent was fo~d
to be surfacing in the area of the leachfield, therefore we request
that youexpedite iss~anceofthispermit.

1986.
Permits are issued on a calendar year basis by authority
of Municipal Ordinance. A new permit must be obtained from
this Department for any well and/or on-site sewer system not
installed by the expiration date.
If you have drilled the well, a well log needs to be sent to
this Department for documentation of the installation and to
close the permit.
If a private engineer inspected the installation of the on-site
sewer system the original as-built inspection report (three part
form) must be sent to this office for review and approval, and
for documentation.
