of the Department of Health and Human Services has reviewed
the following cases and has'these comments:

No objection.
S-9426:
Lot 13A Block 2 Hampton Hills Subdivision #1
No objection.
S-9427:
Riverview Estates Subdivision
No objection.
S-9428:
Crabtree Acres Subdivision
This office cannot support this plat until the
following comments are addressed:
1. The developer must prove well water
availability for the proposed lot.
2. The soils test for the prop6sed lot must be
submitted on a form provided by this department.
No ground water monitoring results were
submitted with the soils test~%in<.~question.
3. A separate site plan must be submitted showing
the reserve area for future wastewater disposal
systems as required by~-15.65.180 B.1. or~2.
This must be shown for the new proposed lot
and the existing lot.
4. The developer must show how far the existing
septic tank and absorption field will be
