To whom it may concern:
This is a request for a septic approval on the above referenced lot. The existing
system was installed without a permit, undocumented. We have documented the
system by excavating the old trench and installing a new 1250-gallon plastic tank,
see new tank permit. A single test hole was excavated to 17 feet and the soils
consisted of poorly graded sand (SP) for the entire depth. Water was not observed
during excavation or after the 7-day monitoring period. The perc rate was measured
at 4 minutes inch. A single test hole did not cover the entire field, but the original
system had a 125 sf/bedroom rating, see the MOA file for the soil log. A simple
5-wide trench was installed with a gravity flow 1250-gallon plastic tank. The slope
around the field is flat where the field is located then slopes at about 10 percent
towards the east. A small 4 -oot tall block retaining wall parallels the trench but
doesn’t impact the field due to the base of the wall being the same elevation as the
top of the field.
This system has not impacted any of the neighbors or encroached on any wells,
septic or open water issues.
We are also requesting a four-foot waiver between the tank and the field.

To Whom it may concern:
This is a request for a septic tank permit on the above referenced lot. The tank is
over 20 years therefore it needs to be replaced. Also the leach field is
undocumented. The system was excavated and found a 5-wide trench with 4-feet of
filter rock and 75 feet long. A new monitoring tube was installed for the testing of
the system. A new test hole, see the site plan, was done which found silty sand for
the entire 17-foot depth, with no water. The perc rate was 2 inches per inch. We are
waiting on the 7-day water reading. We are requesting a tank permit after which a
COSA will follow.
This septic tank installation will not impact any of the surrounding neighbors.

Impacts to Adjacent Properties
Dear Onsite Services Engineer:
We hereby apply for a permit to construct a new septic tank on Lot 4,
Craig Subdivision. The existing tank has collapsed and a new tank must be
placed immediately to prevent damage to private property and create a
possible health problem. The attached site plan and backup documentation
identify the size and location of the new tank to serve the four bedroom
home to be constructed on the lot. The plan also shows the location of the
well on the lot as well as the adjacent lot along with their protective
100' radii.
The new tank will be placed in the same location as the old tank. The old
tank will either be removed from the lot or crushed, backfilled and
abandoned on site. In addition, two new post tank cleanouts will be placed
prior to the new tie in to the existing absorption trench. The surface area
at the location of the new tank is flat and should present little difficulty
to the construction of the new tank.
If the system is constructed as designed the following statements apply:
The system, if constructed as designed, will have no adverse impact
on the wells in the area or those to be constructed in the future.
The system, if constructed as designed, will have no adverse impact
on existing septic systems in the area or those to be constructed in
the future.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Cedificates of Health Authority Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority Approval are
valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued with
new water sample results. (Certificates may be reissued for a period of up to one year with valid water samples.)
Certificates are valid for one year for properties served by Class A or B wells or a public water system. The
Municipality of Anchorage Is not responsible for errors or omissions in the professional engineer's work.
4. STATEMENT OF INSPECTION BY ENGINEER

based on procedures outlined in the Health Authority Approval Guidelines for this application, shows that the

1. Easement. Chugach is the grantee or user of an easement for the construction.and
maintenance of electrical facilities, as follows:

Permittee acknowledges the validity of the Easement. Permittee xvarrants and represents
that Permittee is the owner of the fee simple interest in the land subject to the Easement and the
following described real property to which the Easement is adjacent:
Lot 4, Craig Subdivision, according to Plat Number 77-105, recorded on the 16th day of
June 1977, on file in the office of the District Recorder, Anchorage Recording District,
Seward Meridian, Alaska,
(herein "Parcel").
2. Encroachment. Permittee proposes to continue the existence of the following
improvement(s) on or within the real property subject to said Easement:
A well located at a point 4' within said Easement and a retaining within said Easement
for a distance of 8',
(herein "Encroachment").
3. Permit. 'Chugach hereby authorizes Permittee to continue the existence of the
Encroachment in its proposed location subject to the covenants set forth in this permit.
4. Indemnification. Permittee shall indemnify and hold Chugach harmless from any
claims for personal injury, property damage or other loss arising in any way from the continuation of
the Encroachment.
5. No Expansion or Increase. Permittee shall not in any way expand or increase the
extent to which the Encroachment occupies any of the real property encumbered by the Easement
without the prior written consent of Chugach. Chugach has no obligation to provide such consent.
6. No Interest in Real Estate Acquired. Permittee hereby acknowledges that the
Encroachment and any additions thereto, even if in violation of this permit, has not and shall not in
the future give rise to any interest or estate in the real property subject to the Easement or any other
real property. Without in any way limiting the foregoing, Permittee acknowledges that the
Encroachment is not adverse to any interest of Chugach and its continuation is entirely with the
permission of Chugach for purposes of the common law doctrine of adverse possession. This permit
creates no interest in the real property subject to the easement.
7. Modification of Utility Facilities. If the electric utility facilities located within the
Easement are upgraded, added to, replaced or reconstructed, Permittee shall pay that portion of
Chugach's total costs for such modification which is reasonably attributable to accommodating or
preserving the Encroachment. If such payment is not made within thirty (30) days of written demand
for same, Chugach may terminate this. permit in accordance with paragraph 9, below.

Tests were performed at Lot 4, Craig Subdivision on Jan. 12, 1987
for a Health Authority Approval. An adequacy test was run on the
septic system, however the soil absorption system was unable to
accept the required amount of liquid and failed the test.
Municipal records show the soil down to a depth of ten feet to be
a gravelly sand with a rating of 125 sq. ft. per bedroom. The
owners were unaware of any prior overflows and I saw no evidence
of any spillage. If a spill did occur it would overflow onto the
edge of the driveway where it would be very visible. The well is
on a high point of the lot and any overflow would drain away from
the well and away from the house. No other wells in the
neighborhood would be affected.
It is recommended that a Cgnditional Health Authority Approval be
issued, with the following stipulations:
Consgruct a new ~..Qil absorption system after spring
breakup.
2. Then, abandon the existing soil absorption system.
Then, install a cleanout on the second compartment of
the septic Tank. Extend the first cleanout above
ground level.
The owner should have the tank pumped immediatly if any
overflow from the trench standpipe is noted.
