RE: SEPTIC SYSTEM DOCUMENTATION — INSPECTION REPORT
LEGAL: CHUGACH PARK ESTATES BLOCK 3, LOT 8
During the COSA process — no inspection report for permit #SW930116 was found in the MOA
file. The owner provided an unsigned septic as -built with elevations by Lous Butera, CE -6736
of the referenced property. In conjunction with this and MOA record documents, attached is the
inspection report per recent visual observations and measurements taken. Based on this
investigation, it appears the system was constructed as originally permitted. Please contact us if
you have any questions.

The subject permit, issued May 24, 1993 by this office for a
single family well and/or on-site wastewater system, has
expired as of May 24, 1994.
A new permit must be obtained from this office for a well
and/or on-site wastewater system NOT installed by the
expiration date.
If you have drilled the well, a well log must be sent to
this office for documentation of the installation and to
close the permit.
If a licensed Professional Engineer has inspected the
installation of the on-site wastewater system, the original
as-built inspection report must be sent to this office for
review, approval and documentation. All inspection reports
must be submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $320.00 for an
on-site wastewater permit; $120.00 for a well permit and
$440.00 for a combined on-site wastewater and well permit.

EASEMENT AGREEMENT
THIS EASEMENT AGREEMENT (the "Easement") is made
effective as of ,/ , 19930, by and between Kelly
Kavanaugh and Donna Pircher, husband and wife, ("Grantees") of P.O.
Box 230602, Anchorage, Alaska, and William A. Platzek and Mildred
I. Platzek, husband and wife, ("Grantors") of P.O. Box 670522,
Chugiak, Alaska.
WHEREAS, Grantors are the owners of certain real property
described as follows!
Tract 1 of SCIMITAR SUBDIVISION, UNIT NO. 3, according to
Plat #80-51, Anchorage Recording District, Third Judicial
District, State of Alaska;
hereinafter "Grantors' Property"; and
WHEREAS, Grantees are the owners of certain real property
described as follows:
Lot Eight (8), Block Three (3), of CHUGACH PARK ESTATES,
according to Plat 72-188, filed in the Anchorage
Recording District, Third Judicial District, State of
Alaska;
hereinafter "Grantees' Property"; and
WHEREAS, Grantors desire to grant an easement for
driveway purposes over a portion of Grantors' Property for the
benefit of Grantees and Grantees' Property;
NOW, THEREFORE, the parties agree as follows:
SECTION ONE
CONVEYANCE OF EASEMENT
Grantors hereby agree to grant and convey to Grantees a
perpetual easement (the "Easement"), subject to any prior
easements, for driveway purposes over a portion of Grantors'
Property, which Easement is more particularly described as:
A section of land running 80 feet in a north -south
direction and 30 feet in an east -west direction
immediately adjacent to the southeast border of Grantors'
Property.

SECTION TWO
"DRIVEWAY PURPOSES" DEFINED
"Driveway purposes" as used in this Easement means a
residential driveway, and it is agreed that no vehicles of more
than four (4) axles or weighing more than eighty thousand (80,000)
pounds shall use or travel across the Easement.
SECTION THREE
CONSTRUCTION AND MAINTENANCE
The driveway shall be constructed and maintained in good
repair by Grantees at their sole cost and expense. The driveway
shall be constructed and maintained of the following material:
cobblestone, sand, and gravel.
SECTION FOUR
PRIOR UTILITY EASEMENT
The Easement shall be subject to, and in no way shall
interfere with, any previously granted utility easements.
SECTION FIVE
GRANTORS' TITLE
Grantors have marketable title to the Grantors' Property
and the right to grant the Easement.
SECTION SIX
EASEMENT TO RUN WITH LAND
The Easement shall run with the land and shall be binding
upon and shall inure to the benefit of the parties to this
Easement, their respective heirs, successors, and assigns.
SECTION SEVEN
NOTICES
Any notice provided for or concerning this Easement shall
be in writing and be deemed sufficiently given when sent by
certified or registered mail if sent to the respective address of
each party as set forth at the beginning of this Easement.
SECTION EIGHT
GOVERNING LAW
It is agreed that this Easement shall be governed by,
construed, and enforced in accordance with the laws of the State of
Alaska.

SECTION NINE
ENTIRE AGREEMENT
This Easement shall constitute the entire agreement
between the parties, and any prior understanding or representation
of any kind preceding the date of this Easement shall not be
binding upon either party except to the extent incorporated in this
Easement.
SECTION TEN
MODIFICATIONS OF EASEMENT
Any modification of this Easement or additional
obligations assumed by either party in connection with this
Easement shall be binding only if evidenced in writing and signed
by each party or an authorized representative of each party.

Narrative
Dear Mr. Smith,
The proposed septic upgrade will have very limited impact on adjacent properties for the
following reasons:
1. The area has large lots allowing sufficient room for septic sites.
2. Immediate neighboring lots are vacant with no well or septic systems.
3. Reserve space is adequate, due to absorption capacity.
4. Drainage will not be affected and is not a major consideration in our design.

--well and septic plan are for a single family residence only. The drawing and or site plan shall be a part of this specification.
All materials and workmanship shall meet the Anchorage Department of Health
and State Department of Environmental Conservation requirements.
All soil tests are advisory to the design and are to be verified or modified in the
field by the engineer.
All excavations and depths are advisory and are to be verified or modified in the
field by the contractor to meet Municipality of Anchorage, Department of
Environmental Conservation requirements.
It is the responsibility of the owner to obtain all necessary permits or easements
and to locate any adjacent multi-family wells.
The excavation is to be exactly in the area shown on the site plan, any deviation
requires engineer approval.
It is always recommended that a surveyor locate the nearest lot line position and
the location of any easements.

TRENCH
1. The trench is to follow the natural land contour to maintain uniform total depth
of the trench bottom.
2. The bottom of the trench shall be level, plus or minus 1.5".
3. The total depth of the trench excavation is not to exceed 9' at any point.
4. The trench gravel is to be covered with typar fabric material.
5. Soil or combination of soil and extruded board insulation to a depth of 3' or
equivalent is to be placed over the leachfield.
6. The area over the trench is to be finish graded to prevent ponding of surface
water runoff.
7. The septic tank and leachfield must not be closer than 100' to any existing private
well, 150' to any Class "C" well, or 200 feet to any community well.

During a recent COSA on-site inspection and test of the potable water
supply well on Block 3, Lot 8 of Chugach Park. Estates subdivision, the
well's productivity was determined to be .79 gallons per minute. The
minimum well productivity required by this Department (AMC 15.55) for a
4 -bedroom residence is .41 gallons per minute. Although the subject well
currently exceeds this minimum requirement, all parties concerned are
advised that the production capacity of the well may fluctuate. Restriction
of non-critical water uses such as washing cars and watering lawns and
gardens may be required.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.
