Ref: Proposed Septic Tank Upgrade for Fire Lake #2; Block 2, Lot 7

To whom it may concern:
The existing3-bedroom house is served b iae p i
y private water and septic system. The existing septic
tank and drainfield were installed in 1983 and consists of a 1000 -gallon steel septic tank and a
deep trench type drainfield. The existing drainfield has recently passed an adequacy test. Since
the septic tank is over 39 -years old, the septic tank must be upgraded. We are proposing that a
new 1000 -gallon Greer plastic septic tank be installed as shown on the attached design drawing.
Comments regarding the design are summarized as follows:
WELL TO TANK WAIVER AMENDMENT: The existing septic tank was granted a 75 -foot waiver
per the 1983 Inspection Report and the 2009 COSA. We are proposing that the new septic tank
be placed outside the 75 -foot well radius and we are requesting the existing 75 -foot waiver be
amended to include the new septic tank. Justification for this waiver amendment is that the new
tank will be made of plastic, the house location is in-between the tank and the well, and that if the
tank were to overflow, it would not flow towards the well. Recent water samples have been taken
and the nitrate level was found to be 1.87 mg/L.
WELL 1N MANHOLE: Based upon our site visit, the well was found in manhole with the well head
below existing grade. There was no water found around the casing inside the manhole (see
attached photos). Per the Paul Pinard, P.E. 2009 COSA, the manhole over the well was
"watertight" with no drain and your department approved the COSA. Based upon our findings the
manhole lid is not gasketed and in our opinion cannot be considered watertight. It does appear
that there is positive drainage around the manhole lid. We have submitted photos from our recent
site visit to Tim Eklund with your department and it is our understanding that the wellhead needs
to be extend higher in elevation. It is not clear if your department will require a drain be installed.
Please provide direction regarding this matter.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic

ordinances, and regulations in effect at the time of installation, unless noted otherwise.

In conducting this evaluation, GEG provided an engineering evaluation of the well and/or septic system in
accordance with the guidelines and regulations established by the Municipality of Anchorage and industry
practices. The reported results describe the condition of the system/s on the date/s of the evaluation.
Separation distances were measured to readily identifiable features. Hidden defects or encroachments may
exist that were not identified during the evaluation. The operational life of all wells and septic systems depend
upon a variety of variables, including (but not limited to) soil conditions, groundwater levels (that may fluctuate
during the year), quality of construction (materials and workmanship), and the water usage of the family utilizing
the system/s. These conditions can vary, and are outside the control of GEG. Satisfactory test results do not
guarantee future performance of the system/s; therefore, GEG makes no warranty (express or implied) regarding
the future performance of the well or septic system. GEG makes no representation whether an alternative well
or septic system can be installed on the property in the event either of the current systems fail to perform
adequately in the future. The content of this report is for the sole benefit of the person/party that retained GEG
to perform the evaluation. Reliance upon the information provided in this report by any other person or party
(including subsequent property purchasers) is not authorized, nor will it confer any legal right whatsoever.

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of On -Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

satisfactory, meeting ADEC drinking water standards for coliform bacteria, nitrates and arsenic.
NOTES: A 4 hour flow test was conducted during which the well produced an average of 31 gam
