If you have drilled the well, a well log must be sent to this office for documentation of the installation and to close the permit. If a licensed Professional Engineer has inspected the installation of the on-site wastewater system, the original as-built inspection report must be sent to this office for review, approval and documentation. All inspection reports must be submitted within 30 days of construction completion. A new permit must be obtained from this office for a well and/or on-site wastewater system NOT installed by the expiration date. However a new permit can be issued free of charge for a second year if the application for the renewal is received on or before the date of expiration of the original permit for which a fee was paid. When applying for a new permit after the original permit has expired or for more than a second year, the fees are: $320.00 for an on-site wastewater permit and $120.00 for a well permit. Gentlemen: Attached are soil logs and a site plan for a new sewer system and well on the above lot. A three bedroom .single family dwelling that is planned for this lot. We found that there are two potential areas for development; the original soil tests were performed for a previous owner for purposes of sale. However, the new buyer has elected to developme~t on the northern portion of the lot instead, so two new testholes were dug. As you can see from the logs, there is shallow water present. In addition to these holes, the builder dug a third hole between testholes//3 and #4; this hole showed that the ML layer logged in testhole #4 literally "ran out" at the southern wall of the pit. There is a low area which is wet near the mid-point of the lot. In order to maintai~ a minimum 100 foot separation to this area, the system and reserve area have been moved as far north as possible. They have also been moved to the east as tar as possible to ensure no encroachment on potential water in the ditch along Evergreen. At the present time there is no water in this ditch directly opposite the proposed system. The developme, nt of this lot should have no adverse impact on neighboring lots. Michael N. Anderson, P.E.
