James Sizemore and Associates 6410 Switzerland Drive Anchorage, Alaska 99516 Subject: Lot 8B Goldenview Heights Subdivision, PID #020-042-93 NOTICE OF VIOLATION Dear Mr. Sizemore: We have requested numerous times during the past four months that you submit the as-built drawings of the on-site septic system installed on the subject lot under Permit Number SW91-181. You are required by Municipal Ordinance AMC 15.65.150.E to submit an inspection report (as-built drawing) within thirty (30) working days of the date of the final inspection of the on-site wastewater disposal system. This letter will serve as an official Notice of Violation to the above referenced section of AMC 15.65.150.E. If the subject inspection report (as-built drawing) is not received by not later than January 29, 1993, a citation will be issued accordingly. Additionally, on September 2, 1992 this office received an application for a well permit on the captioned lot. We have not issued the well permit fo~ two reasons: (1) a well permit will not be issued unless there is an approved on-site wastewater disposal system on the lot, and (2) the well permit application was for a five bedroom residence whereas the wastewater disposal system was designed and approved for a four bedroom residence. If there are indeed five (5) bedrooms, then the recently installed wastewater disposal system is undersized and must be upgraded. Natural vegetation on the lot includes large spruce and birch trees. Homes on lots in the general vicinity of this lot have been on On-Site systems for many years. Followingis a summary of reasons that this proposed system should not adversly affect adjoining wells, wastewater systems, reserved space, or drainage. 1. WELLS The wells on adjoining lots that are currently occupied are properly separated from the proposed and replacement disposal systems by at least 100 ft. 2.WASTEWATER SYSTEMS The disposal system proposed for this lot complies with provisions of the latest MOA Regulations. All setback distances are complied with. The adjacent undeveloped lots are large enough that there should be adequate room for them to develope their lots adequately. 3. RESERVED SPACE. There is adequate reserve space on this lot for a replacement system and there should be adequate space on the adjoining large lots for systems and replacement systems. 4. SURFACE AND SUBSURFACE DRAINAGE There are no areas of standing water on the lot, and there is no visible surface drainage. Water was not encountered in any of the three test holes dug and in which monitering tubes were installed.
