DEPTH LENGTH GRAVEL DEPTH WIDTH
The length dimension is the length(in feet) of the trench or drainfield. The
depth of a trench or pit is the distance between the surface of the ground and
the bottom of the excavation(in feet). There is no set width for trenches.
The gravel depth is the minimum depth of gravel between the outfall pipe and
the bottom of the excavation(in feet).

Permit applicant has the responsibility to inform this department during the
installation inspections of any wells adjacent to this property and the number
of residences that the well will serve.
* * * TWO(2) INSPECTIONS ARE REQUIRED * *
Backfilling of any system without final inspection and approval by this department
will be subject to prosecution.
Minimum distance between a well and any on-site sewage disposal system is 100 feet
for a private well or 150 to 200 feet from a public well depending upon the type
of public well. Minimum distance from a private well to a private sewer line
is 25 feet and to a community sewer line is 75 feet. Well logs are required
and must be returned to this department within 30 days of the well completion.
Other requirements may apply. Specifications and construction diagrams are
available to insure proper installation.

Dear Mr. Eggener:
Your request for waivers of the required 75 foot separation to a public sewer line and a
100 foot separation to a public sewer manhole have been approved. The approved
separation distances are the private well to the public sewer line of 19 feet and the private
well to the public sewer manhole of 96 feet.
This waiver approval applies to the existing well and public sewer line and manhole only.
Any future upgrade will require all separation distances be met or another approval from
this department.
If there are any concerns or questions regarding this matter, please call our office at 343-

Miller Well - Lot 8, Block 22, Alyeska 3rd Addition
Dear Jim:
In accordance with our telephone conversation of June 4, 1996 we have now completed
the sleeving of the Miller Well.
The work was performed by Dave Harper of Alpine Drilling on June 8, 1996.
The asbuilt drawing and driller's certification of compliance with the project specifications
are attached. The asbuilt drawing bears my original stamp.
All required fees have been paid.
We believe we have now complied with all of the conditions stipulated by your department
for grant of the well radius waiver. Those conditions were given to me by you on March 14,
1996 and restated in my letter to you on March 20, 1996. A copy of that letter is also attached.
Please review the enclosed materials and issue the waiver or call me if additional
information is required.

Dear Jim:
In accordance with conditions stipulated by your Department for grant of a well radius waiver
for the Doyle Miller well (Lot 8, Block 22, Alyeska 3rd Addition), which were recapped in our letter
to you dated March 20, 1996 (copy attached) - AWWU performed a video inspection of the sewer
main adjacent to the well on May 14, 1996. A copy of the video tape from that inspection is enclosed
along with the written inspection record prepared by the AWWU personnel who performed the work.
I have reviewed the video tape and the inspection report and concur with AWWU Manager
Bob Fuge's letter to Doyle Miller dated December 14, 1995 (copy attached) wherein he stated that
based on a similar TV inspection made on November 30, 1995 the sewer main was found to be "in
excellent condition" with "no anticipated problems in the foreseeable future".
We are assuming that the transmittal of this evaluation along with the accompanying video
constitutes compliance with condition #2 for grant of the waiver as stated in our March 20, 1996
letter. If after review of the enclosed information you have additional concerns about the integrity
of the sewer please contact us prior to May 31, 1996. Otherwise we will assume that sewer main
integrity has been satisfactorily confirmed and we will proceed with sleeving the well in accordance
with the drawing previously approved by your department.

Dear Jim:
Thank you for your phone call of March 14, 1996 advising me that after your review of the
technical information I provided (during our meeting on January 5, 1996 and by fax on February 14,
1996) your department will grant a waiver request for the Miller Well subject to certain conditions.
We understand those conditions to be as follows:
1. The well will be sleeved in accordance with the attached drawing.
2. The structural integrity of the reach of sewer line adjacent to the Miller Well will be
confirmed by television inspection with a copy of the videotape to be provided to the
Department. The subject segment of sewer is the 8" ductile iron main between
manhole 619 and manhole 618 on Higher Terrace - 1980 Sanitary Sewer
Improvements Girdwood/Alyeska LID 60-1 Schedule 1.
Please consider this letter, supported by the information we presented earlier and other
information you already have in your file, to be our formal request for the well radius waiver subject
to the conditions stated above. Attached find our check in the amount of $920.00 for the waiver fee.
Based on our March 14, 1996 conversation the Millers are proceeding with the work required
to clear the waiver conditions. Please advise if you would like to have a representative present during
either the sewer main television inspection or the well sleeving operation.
I appreciate your extra effort in working with us to find an acceptable solution to this
problem.

Dear Mr. Henderson:
The On -Site Services Section has reviewed your request for a separation distance
waiver for the private well on the above mentioned lot. This request was for a
waiver to 21.1 ft. of the required 75 ft. separation between the well and the public
sewer line located in Higher Terrace. The submittal also requested a waiver to 96
ft. of the required 100 ft. separation distance between the well and a public sewer
manhole located in Higher Terrace.
The determination for the issuance of waivers is made according to guidelines
established by the Alaska Department of Environmental Conservation. Following
these guidelines, the On -Site Services Section has determined that this property
does not qualify for the waiver to 21.1 ft. from the well to the public sewer line.
As I mentioned previously, the fee for this waiver will be refunded at your request.

Road in the area you requested. These waivers were found from the records in the office only,
and a brief summary of each is given below:

Addition was cleaned in June of 1995 as part of the Utility's preventive maintenance
program. At your request, it was re -inspected November 30, 1995. This sewer main was
found to be in excellent condition on both occasions and we don't anticipate any problems
in the foreseeable future.

1. Basic Information.
This waiver was requested for a horizontal separation distance of 21.1 feet from a
private well to a public sewer line. The well was installed in 1984. The sewer line
was constructed in 1979. The horizontal separation distance required at the time of
the well installation was 75 feet.
2. Well Data.
The well was drilled on 5/7/84 to a total depth of 106 feet. The static water level in
the well casing on 1/31/95 was 12 ft.
3. ADEC Point Calculation.
Top of bedrock at 18 ft.
Total depth of 106 ft.
Static water level on 1/31/95 was 12 ft.
a. Water Table (Static Water Level in Well at 12 ft.) 1.5 pt.
b. Soil Type (Sand and Gravel - use sand value for no clay present) 1.5 pt.
c. Permeability (Sand and Gravel - use value for fine sand, for won't
qualify for next highest value of soil with perc rate > 50 min./inch.) 1.5 pt.
d. Gradient (use +5%) 4.5 pt.
e. Horizontal separation (use 21 feet.) 0.6 pt
Total points awarded 9.6 pt.
Points required to award waiver per guidelines is 16. Deficiency is 6.4 points, or

Waiver Guidelines
for SCRO
Since many of the engineering firms operate in two or more districts
the following guidelines for waiver requests are offered to provide
consistency and continuity during the.review process within the Region.
A. All waiver requests should be submitted by a registered engineer
in accordance with 18 AAC 80.020(e) and 18 AAC 72.021(a). Con-
sideration of exception should be referred to me.
B. The district office engineer should evaluate and sign all waiver
approvals and denials.
C. To comply with 18 AAC 80.020 and 18 AAC 72.021(a), the following
format is offered to use as a review guide in reaching a decision
as to whether a waiver will be granted in a specific instance.
It should be noted that separation waivers are specific and
unique to a site and specific set of conditions. The waiver
should in no way be alluded to as being precedent setting or
that the waiver represents a new standard for specific geographic
area or geological conditions. This guide is offered to assist
in determining if a lesser separation distance will meet the
intent of the regulation and protect public health. It is also
recommended that a short written evaluation be made in each of
the categories. If the engineering report does not address a
specific category, the worst case condition should be assumed.
1. Geological.
a) Where green or non -fractured bedrock is between the system
bottom and the water table one must consider the slope of
the rock formation with respect to the water source (i.e. does
it slope to or away from the source).
b) The attached charts may be used to assign number values to
the following geological conditions.
Distance from the sewage system bottom to groundwater
Soil sorbtion below the sewage system
Soil permeability below the'system
Water table gradiant

The sum of the above number values could equate to tne following
conditions:
0-8 Pollution is imminent from both chemical and bacteria
8-12 Contamination is unlikely from bacteria but could be
possible from chemicals or virus.
12-16 No bacteriological pollution possible but chemical
pollution possible but unlikely from household
chemicals.
Almost sure to be free from any form of contamination
from household sewage.
No contamination possible.

It should be remembered that this is only a guide but that numbers of
16 or les for a continuous source of contamination such as a leach
field sho ld not be waived, unless other 75ti4 measures are
noted or special construction techniques which can mitagate some of
the geolo ical concerns are taken. Also, where the well penetrates
bedrock c lculate the values of the soil and the vertical separation
above the bedrock only.
2. Secial Construction:
a
u
Weigh any unusual materials or constructicn techniques
sed to (medigate)geological concerns.
b) Weigh material changes, design changes or special con-
struction techniques, which allow the waiver and sill meet
the intent of the regulations. Two examples are; sealing of
septic tanks to allow waivers to holding tank separation
distances, or to harden manholes (IAW the detail shown in
attachment B) to allow manhole separation to be waived to a
sewer line separation.

a) Consider how much of a waiver is requested (what % of the

Dear Mr. Cross:
Thank you for again agreeing to review the punitive impacts both in
financial and environmental consequences of your March 1, 1995 turn -down
of our request for a separation distance waiver for our private well.
To review the facts briefly: our 2/1/95 request was for a waiver of the
required 75' separation between the well and the public sewer line (our well
is 21.1' on the 20+% uphill slope side of the sewer); and waiver to 96' of the
required 100' separation distance between the well and the public sewer
manhole located down the hill from our well.
Our well has produced potable water for 12 years with no contamination
We understand there are basically two issues of concern to the MOA, DH&H5:

This alternative, with the exception of complying with standard code
practice for separation distances, does not provide additional assurances
for your concerns for adequate/safe drinking water, or protection of the
aquifer.
We propose to install a Bentonite Grout Seal for our existing well (similar
to those used in HazMat monitoring wells) to further minimize the remote
possibility of contamination of the aquifer via the existing well. Cost
estimates for this Grout and Seal proposal are as follows:
$1,500 Engineering development, review and approvals
$3,000 Alpine Grilling - Grouting and Sealing
$800-$1.000 MOA Variance Fees
$5,300 to $4,100
This alternative will need waiver from the standard code practice for
separation distances but will provide PE certified assurances for your
concerns for adequate or safe drinking water, and protection of the aquifer
We do not make this request for reconsideration of the separation distance
waiver by putting our families health, or the health of the aquifer at risk,
We believe this is the most equitable solution, providing safeguards to the
hillside environment and that of the groundwater aquifer. Your immediate
response to this request would be appreciated.

I have performed a site investigation and well flow test along with a health authority approval for the above
reference property for the current owner Mr. Doyle Miller on 1/31/95. Please consider this a formal request
for waiver of the separation distances from the public sewer line adjacent to the property in Higher Terrace
Street right of way. The subject well is currently located 21.1' from the main 8" ductile iron sewer line and
96' from the closest manhole #618 and 6.5' west and 11.1' south of the property lines. See location maps
attached for details.
The history of this property is as follows. The sewer main was constructed in 1979 and as -built in 1980 and
the well permit was issued in 1984 and the well drilled on 05/07/84 in its present location by Hefty
Drilling. This property is located on the most southerly portion of the Alyeska subdivision on the upper
hillside of a very steep mountain side with slopes of 35-40% from the street to the rear south side of the lot.
All of the lots along this street have a very similar problem with very steep slopes which make it very
difficult if not impossible to construct a home with normal setbacks from the street and still maintain an
accessible driveway to the property and feasible utility (sewer service) to the buildings. Almost all of the
existing homes have well encroachment problems relative the sewer line for the same reason. Lots 15 and
16 of block 21 on the lower side of the street currently have approved waivers that I am aware of.
I have enclosed the location maps showing the physical location to the subject well in relation to the
surrounding lots as well as the soils logs available from the adjacent sewer line construction and a copy of
the logs from the adjacent wells along the same street on both sides. Copies of the sewer service line for the
subject installed in 1984 along with the sewer main line plan and profile have also been included for your
review. The information provide on the initial well log by the driller in 1984 is somewhat incomplete, but
my recent flow test indicates that the well draws water at a continuous rate of 3.3 gpm at a level ranging
from 28' to 34', which is well within the solid bedrock level and above the fractured rock strata. Since the
well is uphill from the sewer line with the surface ground slope and slope of the subsurface bedrock
generally very steep in this area, the possibility of contamination of the well from any possible sewer line
leaks is reduced. The steep grade of the ductile iron sewer main along the street in this area is also a
positive factor allowing little time for the effluent to stand in the pipe and thus minimize the chance of any
possible leakage with the manhole #618 being significantly lower in elevation. The water analysis tests for
the subjects well in 1989 provided by the owner and recently test results show no sign of excess
contamination, indicating that the well has been performing adequately at least since 1989

SUBJECT: VACATION RFJJEST - Alyeska Subdivision Third Addition, Block 22, Lot 8
The Anchorage Water & Wastewater Utility has reviewed your request for vacation
of the 10' utility easements paralleling the northwest and northeast property
lines of referenced property.
The existing house, cantilever overhangs, eaves and well are shown on the
asbuilt drawing dated January 27, 1995, and are encroaching into the referenced
easements. The well which is located on the northern corner of the lot is
within 20' of the AWWU sanitary sewer mainline. Petitioner will need to resolve
the location of the well with Jim Cross at the Department of Health and Human
Services. Mr. Cross can be reached at 343-4708.
AWWU water mainlines are not available to the referenced lot. AWWU sanitary
sewer mainline is located within the Higher Terrace right-of-way, 10' from and
parallel to the northwest property line. AWWU requires a 30' easement or
right-of-way centered over mainlines. Under this requirement, for the
northwestern utility easement, AWWU must retain the northwest 5' while allowing
the vacation of southeast 5' of the subject easement. For the utility easement
located on the northeast 10' of the referenced lot, AWWU is not using this
easement and will not object to the proposed vacation of this easement.
This letter of non -objection is being issued to supplement your application to
the MOA Department of Economic Development and Planning per Title 21.15.130.d
of the Anchorage Municipal Code regarding vacation and replatting of the
subject property.

Mr. Millers house, well, cantilever overhangs, eaves, and well were inadvertently
constructed into the 10' platted utility easements along the Northwest and Northeast
property lines as shown on the attached asbuilt. Since these easements appear to be
surplus to present and future needs, we are proposing to eliminate those easements
through the platting process. We would leave the 10' easements along the Southwest and
Southeast property lines.
We are requesting a written statement from each utility approving our proposed vacation.
If we receive approval for this vacation from all utility companies, we will then initiate a
preliminary plat.

Per your request I have reviewed the situation with respect to the well serving the residence
you own on Lot 8, Block 22, Alyeska S/D, located on Higher Terrace Road in Girdwood.
According to records on file at the Municipal Health Department a well permit was issued on
1/17/84. The driller's log indicates that the well was drilled on 5/7/84 and has a total depth of 106
feet, of which all but the uppermost 18 feet are drilled through bedrock. The well production is
reported to be 6 gpm coming in through fractures in the bedrock below a depth of 65 feet.
Because the lot is characterized by extreme slopes in the vicinity of 35%, the well was
drilled near the front of the lot, at a location only 21 feet away from a public sanitary sewer which
runs along Higher Terrace Road. This is much less than the 75 feet separation which was
specified on the permit for a private well serving a single family residence. The well is also only
96 feet from a sewer manhole, instead of the normally required 100 feet. I understand that your
recent request for a separation distance waiver has been denied by the Municipal Health Dep't.
On September 1, 1995 I collected water samples from the water supply system serving this
lot. The enclosed lab test reports show that the water quality was satisfactory, with 0 coliform or
other bacteria per 100 ml., and non-detectable nitrates. I also spoke with a well driller with regard
to providing additional source protection by sealing the well with grout, in order to justify issuance
of a separation distance waiver allowing the well to be formally approved in its present location.
Basically, this would involve installing a 4" diameter PVC liner inside the well with a shale packer
set ata depth of 65 feet between the outside of the liner and the inside of the bore hole. The driller
„would then use a Tremie tube to completely fill the annular space between the PVC liner and the
bedrock with a slurry_of expansive bentonite grout -This bentonite seal would extend up from the
shale packer -at 65 feet to just below the level of the pitless adapter. Once installed, this seal should
eliminate the possibility of contaminants from a leaky sewer line from making their way into the
bore hole through fissures in the bedrock. The well is partially artesian, indicating that the aquifer
is confined and not susceptible to contamination originating directly above. •
In my opinion, the proposed grouting remedy should be sufficient to justify issuance of a
r separation distance waiver allowing the well to be approved. In the event additional source
protection were felt to be justified, a IJV filter could also be installed inside the residence which
would guarantee that any pathogens which somehow made their way into the water system were
