We propose to install one deep trench with and Advantex system to serve the existing 6-bedroom residence. The design is based on the recent test hole conducted on June 16, 2021. The location of the existing crib and trenches will be determined at construction and the necessary, required ROW permits and/or waivers will be obtained to work in the ROW for this determination, complete removal decommission or continuation of use. Required separations between the proposed trench and these existing fields / crib will also be determined at this time with proper action (silt plug, waiver request if needed, ). The slopes are moderate at 0-5% at the proposed upgrade location. The lot and area are served by private water. The design will not impact any of the neighboring properties. Please contact us if you have any questions. Sincerely, Curtis Huffman, P.E. The waived distance is 5 feet. ~' This approval applies to the existing septic system lot line separation only. Any future upgrade to the septic system will require all separations be met or another approval from this department. Sinc ty, Civil Engineer On-site Services RWR/ljw:97 EAGL RIVER ENGIN VIC .$ Lou Butera, P.E. lC, '- 1 1989 August 1, 1989 Mr. John Smith, P.E. M.O.A. - D.H.H.S. 825 L Street Anchorage, AK 99501 P.O. Box 773294 Eagle River. Alaska 99577 RECEIVED Telephone (907, 694-5195 RE: Baronik Sub., Lot i Dear Mr. Smith, On behalf of our client, National Benevolent Association, we are applying for a septic construction permit for the above referenced property. The existing septic system was permitted and approved by M.O.A. in 1972 as a 4 bedroom duplex dwelling. In 1974 a permit to upgrade to 8 bedroom capacity was issued by O.A.A.B. with subsequent completion of the upgrade. The system has failed and the owner wishes to downgrade the home to a 5 B.R. single family dwelling. The submitted plan calls for a bed system with 5' lot line waiver adjacent to a roadway. Due to the waiver location there is no conflict with neighbor sewers or utilities. The existing septic tank will be inspected and if found deficient in any way will be replaced. The existing well will be abandoned to A.W.W.A. standards with a new well to be placed in location as shown. At this time the owner would like to obtain the septic permit with necessary waiver. We will apply for a well permit prior to drilling. If there are any questions regarding this matter, please contact me at 694-5195. Sincerely, Louis A. Butera, P.E. 7O SPECIFICATIONS FOR ON-SITE SEPTIC SYSTEM LEGAL: BARONIK SUB., LOT t GENERAL 1. The well and septic plan ere for- a single family residence only. 2.
