The proposed change to update the 2 bedroom permit to a 3 bedroom permit has been approved. A provision of the
approval is that a perc test is done in the upper GP stratum before construction. Please submit stamped and signed
results with the As-built Inspection Report. If the results require a design change, construction of the system will stop
pending On-Site review and approval.
Please correct either page 4 of 5 or page 4 of 5 (?), the distance from top of trench to OG do not match. There were other
items that we discussed that will be submitted with the record drawing.
A change order fee of $130 will be charged.

Septic System Revision Request
Ladies and Gentlemen:
I am writing to request a revision to Permit number SW09017 and Waiver W090036. The original permit
was for a two bedroom system. I was informed that the house is a three bedroom house. I researched
the record and found the original septic system was indeed installed for a three bedroom house. I am
requesting your approval to extend the existing field eleven feet to meet the absorption area for a three
bedroom house.
I am also requesting a change in Waiver 090036. The field ended up being 30 feet from the steep slope.
The revise section A-A shows the effluent will not day-light out of the toe of the slope.
I also request written approval be granted to install the system with a step in it. it was approved in the
field by your representative. The eleven foot extension of the field will not encroach into the five foot
separation to ground water. See the attached section B-B.
Soils. A single test hole was excavated in June of 2009. Ground water was encountered in the
excavation in the forms of seeps at 10 feet and stabilized at a depth of 9.9 feet after the monitoring
period. Bedrock was not encountered in the test hole. Based on the percolation test results and the
overall soils appearance, the soils are suited for the use of a conventional wastewater septic system with
an application rate of 1.2 gpdpsf.

Surface Water: there is no surface water within 100 feet of the proposed upgrade.
4. Topography: The average topography in the area of the proposed septic system is
approximately 2 percent based on contour information. There is a two horizontal to one vertical (2H:iV)
slope 30 feet from the west end of the field. The attached cross section (Section A-A) shows that the
effluent will lot day-light along this slope. I request a WAIVER be granted allowing this installation to be
installed to within 30 feet of a 2H:IV slope. I also request a variance to the requirement to construct the
field parallel to the contours. The drain-field was approved in the field to have a 1.5 foot step to prevent
the field from day-lighting out of the ground. The proposed extension of 11 feet will not encroach on the
separation distance to ground water. See attached section B-B.
5. Drawing Markings: The Drawings are marked ~For MoA Review Only". When written
notification that the review is complete, and that there are no further comments, is received from MoA
On-Site Department, the note will be removed and #Issued for Construction" drawings will be issued.
The proposed installation will not affect the future development of the surrounding or existing lots.
There are no wells or septic systems within 100 feet of the proposed septic location.

Septic System Upgrade Permit Request
Ladies and Gentlemen:
I am writing to request an upgrade septic permit be issued for the above referenced lot. The proposed
system will serve an existing two-bedroom house; the lot is currently developed. The lot is served by a
private water supply system located on the eastern portion of the lot. The surrounding developed lots
are served by private water systems; the private wells are located over :100 feet from the proposed soil
absorption system.
Soils. A single test hole was excavated in June of 2009. Ground water was encountered in the
excavation in the forms of seeps at :10 feet and stabilized at a depth of 9.9 feet after the monitoring
period. Bedrock was not encountered in the test hole. Based on the percolation test results and the
overall soils appearance, the soils are suited for the use of a conventional wastewater septic system with
