Per your request I have redoubled my efforts to locate a driller's log for the well serving the Rein
residence on Lot 2, Block 1, Beede S/D, located at 14361 Fairkytes Avenue. At my request Ms. Rein
called Walter Jacobi (the property owner at the time the house was built) in Hawaii, and Mr. Jacobi
recollected that the well had been drilled by Dotten Well Drilling. Although that company is no longer
actively drilling wells in Anchorage, I was able to locate Earl Dotten and requested that he research their
records for a driller's log. After searching through his records, Mr. Dotten told me he was unable to
locate a log, or even f'md any documentation that he ever drilled this well.
Unless you have other suggestions as to how I should proceed, I feel that I have exhausted every
avenue in searching for this well log. I hope that you will be able to continue processing our waiver
request utilizing the driller's logs for two nearby lots which I supplied along with my original waiver
request.

Dear Sirs:
By means of this letter we are requesting issuance of a waiver of the required 100 foot separation
distance down to 80 feet between the private well and septic system components on Lot 2, Block 1, Beede
S/D. A site plan and other relevant backup documents from DHHS files are enclosed for your review.
Since these distances were noted and approved by the DHEP inspector at the time of installation, I
understand that your office will not require payment of the normal waiver processing fee.
There is no driller's log on file at DHHS, however the septic system inspection report indicates that the
well was completed prior to 7/31/80. The closest lots for which we have been able to locate driller's logs
are Lots 4 & 5, Block 1, Beede S/D (see enclosed). On April 2, 1993 we conducted an adequacy test of
the well on the subject lot. At the start of our test the static water level stood at 11 ! feet below the top of
the casing. Steady pumping of 65 gallons of water from the well at the maximum pump output of 6.5
gpm caused the water level to be drawn down to the level of the pump intake at 119 feet below the top of
the casing. We pumped a total of 667 gallons of water from the well over a 5 hour period and determined
that the sustainable yield of the well is 1.7 gpm, which is adequate for Municipal approval of a three
bedroom residence. Water samples collected on April 5 were satisfactory, showing 0 coliform or other
bacteria per 100 rnl., and 1.94 mg/1 of nitrate-N.
According to the as-built inspection report the wastewater disposal system was installed in July of
1980 and consists of a 1000 gallon, 2 compartment septic tank followed by 26 lineal feet of soil absorption
trench with 8 feet of sewer gravel and 4 feet of cover. The permit specifically okays a waiver to 80'
between tank and well and 95' between trench and well. On April 2,' 1993 we measured the distance from
the well to the septic tank cleanout pipe to be 81 feet, and the distance from the well to the standpipes at
both the beginni.ng and the end of the soil absorption trench to be 95 feet. Thus it appears that the system
was installed in accordance with the permit.
The house was occupied at the time of our test. To assess the adequacy of the system, on April 2 we
added 667 gallons of water through the cleanout at the beginning of the trench. No fluid ever built up in
this standpipe at any time during the test, nor did the system ever back up into the septic tank. At the start
of our test the fluid depth in the sump at the end of the trench was approximately 75 inches. This level
rose 13.5 inches within the first couple of minutes and then remained at the same level until the water flow
was stopped. The fluid level had receded to 6 inches below the original starting level within 20 minutes
after the flow of water was stopped. This indicates that the fluid level in the sump is higher than in the
rest of the trench (a frequently encountered situation) and that the trench has a surge capacity in excess of
667 gallons, which is approximately 1.5 times the design daily usage of a three bedroom residence.
Based on our test data we concluded that the system continues to function adequately for a three bedroom
residence.
Since no driller's log for the well is available, the following assignment of points in accordance with
DEC SCRO's separation distance waiver guidelines is based on shallow soil types reported as being ML
and SP below 13 feet. An assumed depth of 50 feet to impermeable bedrock is extrapolated from the
enclosed nearby well logs. The subject well is approximately 700 feet west of the wells represented by
these logs where the bedrock depth can be expected to be slightly deeper, as it is further from Flattop
Mountain. As is indicated on the site plan, the topography of the subject lot slopes fairly steeply down
from east to west, but is generally level between the well and the septic system. As measured in the well,
the depth to the aquifer is 110 feet below ground level. It is assumed that this aquifer slopes parallel to the
ground surface, so there is essentially 0 gradient to the aquifer between the well and septic system.

Based on these relatively high total points, it appears that the requested waiver can be safely granted
without concern as to contamination of the well by virtue of proximity of the septic system. Please give
