Ref: Proposed Well and Waiver Request for Bernard Subdivision; Block 4, Lot HI -13.
Mr. Crewdson,
The existing 4 bedroom house is served by a community water system and a private septic
system. The community water system is scheduled to be decommissioned and all properties are
being required to have their own well drilled. It is our understanding that all the neighboring
properties past the referenced property, that are on the community water system, have drilled
their own wells and are no longer connected to the water system. It is also our understanding that
the owner originally submitted for a well permit and paid the MOA fee, but was directed to hire
an engineer to perform a site plan for the proposed well location due to the complexities
associated with his property. Per the request of the owner, we have prepared a site plan and
design drawing for a proposed well location.
As can be seen on the attached site plan and design drawings, we are proposing that the well be
drilled near the north property line and that we are requesting an 88 foot waiver from the
proposed well to his own drainfield and the proposed neighboring drainfield. Once the well is
drilled and water service is connected to the house, we are proposing that the community water
service line be decommissioned and abandoned in place by shutting off the water line with the
key box located in the northwest corner of the property. It is our understanding that the two
properties past the referenced property (north/northeast) have drilled their own wells and are no
longer connected to this water system. The well location was chosen after much discussion with
the owner and the well driller, Sullivan Water Wells, about existing site and soils conditions. The
area on the south side of the driveway was deemed to have very poor soils conditions in regards
to drilling the well and installing a water line/pitless. In short, this area was deemed to be
unsuitable for a well due to multiple factors. There are multiple undocumented foundations,
french, and curtain drains in this area which may be damaged by drilling in this area, potentially
causing surfacing of groundwater. Based upon this information, we have chose a well site to
minimize the risk of disturbing underground drains, undermining the garage foundation, and
potentially causing surfacing of groundwater. We have made a requirement for the owner and/or
well driller to have the well location staked to insure the well is drilled on property and meet the
88 feet from his own drainfield and the proposed neighboring drainfield (waiver requested).

We request you grant an 88 feet separation distance waiver from the proposed well to the
drainfield on the referenced property and the proposed drainfield on Lot H1 -A, Block 4; Bernard
S/D. The following items are justification for the waivers:
• The lot generally slopes from south to north. There is heavily vegetated between the well on
and the drainfield. If the septic systems were to overflow, it effluent would not travel toward
the well head.
• The location of the septic systems are or will be in a very visible area so that if any effluent
was to surface, it would be noticed and the problem corrected.
The other path of contamination is subsurface migration wastewater. As can be seen on the
attached well logs from neighboring wells, the well depth range from 240 to 340 feet deep and
there is approximately 40 feet of silty gravel and hardpan soils above the bedrock that have
served to inhibit the migration of untreated wastewater into the aquifer. Attached is a water
sample results from a neighboring well that is downhill from the septic system, which indicates
that there are no issues regarding nitrates or coliform bacteria. Based upon the aforementioned
facts, it appears that there is minimal risk associated with granting the request of a 88 feet
separation distance waiver.

Septic System As -Built
Dear Jim:
The owner of Lot H1B, Block 4, Bernard Subdivision recently replaced the
monitor tube on the west end of the absorption bed. The perforations in
the monitor tube now extend only through the drainfield rock in the bed. I
verified the acceptability of the monitor tube by visiting the site on
October 19, 1995. The bed is now in conformance with Municipal
requirements and can be certified.

completed by Gary Bock of Bock Engineers. His design was based on soils
tests conducted by himself and David Dayton. Mr. Dayton's soil test
indicated poorly graded gravels with a rapid percolation rate. He
therefore recommended a 2' sand filter be placed to slow the percolation
of septic effluent into the surrounding soils. Mr. Bock's percolation tests
indicated soils with slower percolation characteristics, but he chose to
include the filter sand anyway.
The original design was based on an absorption rate of .8 GPD/SF. This is
apparently an error because when a sand filter is placed the design
absorption rate should be .7 GPD/SF. The design, however, was approved
by the Municipality. The system was not constructed within one year of
issuance of the permit and the permit expired.
The owner of the property asked that I renew the permit and extend the
size of the absorption bed to accomodate a four bedroom home. I then
modified the original design extending the bed an additional 10' and
increased the septic tank capacity to 1,250 gallons. All other features of
the original design remained in tact. The Municipality then reissued the
permit for a four bedroom home.
During the construction of the absorption bed we found the underlying
soils to be very similar to those found by David Dayton with percolation
rates under 5 minutes per inch. We then recalculated the size of the bed

required using an application rate of .7 GPD/SF due to the requirement for
the filter sand. The length of the bed was then extended to 50 feet for a
total absorption area of 900 SF. A four bedroom home requires 857 SF of
absorption area with an application rate of .7 GPD/SF. The bed is
therefore acceptable for a four bedroom home.
The excavator on this project, Alaska General Industries, is scheduled to
attend the certification class on Monday, August 14. Once his attendance
is verified he will become a certified installer.
I have enclosed a gradation and certification letter concerning the filter
sand used on this project. It was delivered to the site by Gate Creek Sand
and Gravel and acquired from Quality Sand and Gravel. It appears to
conform to specifications required by A.D.E.C. I have also enclosed a
testhole log performed during the construction of the absorption bed.

Dear Mr. Waters:
This letter is in response to your letter, received in this office by facsimile on May 22, 1995,
which enclosed the results of a sieve analysis. The analysis was done by Mr. Mark Hansen, P.E..
Based upon your letter, the analysis is for sands from your pit, that are to be used for sand filters
for soil absorption systems (SAS).
After review of the required specification for filter sand, issued by the Department and the
results of your sieve analysis, 1 find that the sample of sand meets the requirements of the
Department.
Therefore, the Department hus no objection to the use of this sand as material for SAS. In order
to retain the Department's non -objection, a sieve analysis on a representative sample, taken frorn
your stockpile will need to be submitted to this office for every 1000 cubic yards stockpiled.
A similar submittal will need to be made next year, by May 1, 1996, if you intend to obtain the
Department's non -objection on your sand. This non -objection is for use in sand liners, which are
constructed in the Mat -Su Borough. It is emphasized that this letter of non -objection is for sand
used in wastewater disposal systems, which are less than or equal to 1000 square feet in size.
Systems larger than 1000 square feet may require A separate sieve analysis, every 1000 ecluarc
feet; however, this is the concern of the owner/developer, not the supplier/source of the material.

ON-SITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: Lot H-1 B, Block 4, Bernard Subdivision
GENERAL:
1. The scope of this project includes the procurement and
installation of a 1,250 gallon septic tank and the
construction of a shallow seepage bed to serve the proposed
four bedroom home.
2. Construction shall be in accordance with the approved site
plan, design drawings, Municipal Permit with any special
provisions or conditions, and all applicable State and
Municipal Wastewater Disposal Regulations.
3. The Contractor shall be responsible for obtaining all
underground utility locates.
4. Unless specifically agreed otherwise, the property owner
shall be responsible for final grading areas subsequently
depressed from soil settling. Property owner shall also be
responsible for revegetation of affected areas unless
specifically agreed otherwise.
5. Contractors installing wastewater disposal systems must
be certified by the Municipal Department of Health and
Human Services for system installations. Owners installing
their own systems must receive prior approval from D.H.H.S.
before beginning system installation.
SEPTIC TANK INSTALLATION
1. A septic tank is to be constructed by a certified septic tank
manufacturer. Construction shall include two 4" cleanouts
for pumping access.
2. The septic tank shall be sufficiently bedded to prevent
settling or shifting of the tank.

3. All standpipes on the septic tank shall extend a minimum of
12 inches above final grade.
4. Septic tanks installed without 4' of cover shall have a
minimum of 2" of direct burial insulation.
5. A foundation cleanout shall be installed one to four feet
from the building foundation. Two cleanouts shall be
installed between the tank and the drainfield. The first
cleanout shall be located on undisturbed soil not more than
10' from the tank. The first cleanout shall be to clean
toward the drainfield. The second cleanout shall be to clean
toward the tank.
6. Final grading over the septic tank shall be such that a
positive slope exists away from the septic tank.
DRAINFIELD CONSTRUCTION:
1. The seepage bed shall be constructed to the dimensions
shown on the design. The bottom of the bed shall be within
2" of level. The bottom of the bed must be raked before
gravel placement.
2. Distribution piping must be placed level with perforations
down atop a level bed of drainfield rock. Rock should then
be placed over the pipe to provide a minimum of 2" of cover.
3. A silt barrier or geotextile fabric must be placed between
the drainfield rock and the natural soil backfill.
4. Monitor tubes must be 4" in diameter and installed at the
locations shown on the design. The portion below ground
must be perforated.

5. Direct bury insulation must be placed over the distribution
system when less than 3' of backfill depth is available.
Finish grade over the bed must be mounded to prevent
settlement or depressions.
MATERIAL SPECIFICATIONS:
1. Septic tanks must be constructed by a Municipally approved
septic tank manufacturer.
2. The following pipe materials are approved for use in septic
system installations in the Municipality of Anchorage:
Cast Iron (perforated and solid), ASTM D3034 or P.V.C.
(perforated and solid), ASTM F810 or H.D.P.E. (perforated, but
not solid) and ASTM D2662 or A.B.S. (perforated and solid).
3. Insulation shall be at least 2" thick extruded direct burial
polystyrene (Dow Chemical Co. Styrofoam HI or equalL.
4. Septic tank inlets and outlets shall be fitted with
watertight couplings (Caulder, Fernco, or equal).
5. A permeable geotextile fabric (Typar, Mirafi or equal) must
be installed between the final drain rock layer and the
native soil layer.
6. All drain rock shall be .5" to 2.5" in diameter with less than
3% passing the #200 seive.
INSPECTIONS:
A minimum of two inspections are required by Municipal
Ordinance. These inspections must be conducted under the
supervision of a professional engineer registered in the State
of Alaska. The first inspection must be conducted after the
excavation of trenches, beds or pits and before the installation
of any gravel. A septic tank may be set in place, but may not
be backfilled.

The second inspection must be conducted after the placement
of the geotextile fabric, gravel, distribution piping,
standpipes, cleanouts and insulation. No backfill should be in
place at the time of inspection.

Dear Mr. Smith:
I have tested and investigated the above lot for installation of a
septic system and offer the following findings regarding its
adequacy to support the proposed system:
The immediate area is served by the private Bernard Water
System rather than individual wells.
The area has large lots and generally good absorption
capacities thus allowing sufficient room for septic sites
and reserve areas.
Immediate neighboring septic systems are all +50'
distant.
Drainage will not be affected. I have set the system
back 100 ft. from the drainage easement on the lot. The
easement does not contain any evidence of being an active
watercourse even during spring runoff.

A. General
1. The septic plan is for a single family residence only.
2. The site plan and design shall be a part of this
specification.
3. All materials and workmanship shall meet the requirements
of the Anchorage Department of Health and the state D.E.C.
4. All soil tests are advisory to the design and are to be
verified or modified in the field by the engineer.
5. All excavations and depths are advisory and are to be
verified or modified in the field to meet Municipality of
Anchorage and state D.E.C. requirements.
6. It is the responsibility of the owner to obtain all
necessary permits and or easements.
7. The excavation is to be exactly in the area shown on the
site plan, any deviation requires engineer approval.
8. It is always recommended that a surveyor locate the
nearest lot line position and the location of any
easements.
B. BED
1. The bed is to follow the natural land contour to maintain
uniform total depth of the bed bottom.
2. The bottom of the bed shall be level, plus or minus 2.0"
3. The total depth of the bed excavation shall not exceed 4
feet at any point.
4. The bed gravel is to be covered with a geotextile filter
fabric material.
5. Soil or a combination of soil and extruded board
insulation to an equivalent depth of 4 feet is to be
placed over the absorption bed.
6. The area over the bed is to be finish graded to prevent
ponding of surface water runoff.

conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the

REF: Additional Non -Functional Monitoring Tube in Bed for Bernard; Block 4, Lot 111-B
To whom it may concern:
We have submitted a Certificate of On -Site Systems Package requesting approval from your department. It was noted
in the package that there are two (2) monitoring tubes in the drainfield and one (I) was functional and the other was
non-functional (did not extend down into the effective). During the review process a "comment sheet" was provided
to stating that the non-functional monitoring tube must be functional. GEG appealed this requirement citing MOA
regulation 15.65.060 Subsurface Disposal Fields, E. "General Construction Standards", item 13. It states, "A
subsurface disposal field must have vertical monitoring pipes installed. At least one monitoring pipe with a minimum
diameter of four inches shall be installed in each disposal field if the field is not continuous. The monitor pipe is to be
perforated to allow liquid level measurement to the bottom of the trench excavation. Non -perforated monitor pipe
shall extend from the distribution pipe invert to above ground level." Even though one (1) of the monitoring tubes in
the drainfield is non-functional, the other is functional and meets the above stated regulation. In short, the drainfield
meets the intent of the code and we request approval of the COSA without being required to do any repairs to the
second monitoring tube.
