1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska Wastewater
Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80).
3. The wastewater code requires inspections during the installation. The engineer must notify the Development Services
Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
Special Provisions:
The Engineer needs to do an additional test hole prior to the construction of the septic field.
Construction may proceed at your own risk before the 7 day water monitoring is complete. Please
submit stamped and signed results with the As -built Inspection Report. If the results require a
design change, construction of the system will stop pending On -Site review and approval.

Septic System Upgrade Permit Request
Ladies and Gentlemen:
I am writing to request a permit to install an upgrade septic system be issued for this property. The
existing drain field is in failure and will be re -used as a reserve. The integrity of the existing 1250g
concrete septic tank will be verified and will be replaced if found to be leaking. This lot and the
surrounding lots are served by private wells. There are no wells within 100' of this system.
1. Soils. One test hole was excavated in the area of this drain field by Pannone Engineering Services
in October of 2013, and neither groundwater nor bedrock was encountered in the test hole at the time of
excavation. Water was monitored at a depth of fourteen feet (14.0') below ground level in the test hole
monitor tube after the seven day monitoring period. It is my opinion, based on the results of the
percolation tests and overall soils appearance; an application rate of 1.0 gallons/day/square feet should
be used for a conventional wastewater system. The test hole found the soil stratum that the system will
be installed in to be sand.
2. Soil Absorption System Design.
a. See Sheet 2 of the design package.
3. Surface Water: There is no surface water within 100 feet of the proposed drain field. The
proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage ditches.
4. Topography: The existing topography slopes approximately from the north to south at a grade of
less than 5% in the area of the proposed drain field. There are no steep slopes in the vicinity of the septic
system.
5. Waiver Request: I request a waiver to the minimum distance from the drain field to the lot line
from 10 feet to one foot (1'). The lot line will be staked by a licensed land surveyor before construction

begins.
The proposed installation will not affect the future development of the surrounding or existing lots. There
are no surrounding wells within 100 feet of the proposed septic location.

5. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures
outlined in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or
wastewater disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated
herein. I further verify that based on the information obtained from the Municipality of Anchorage files and from my investigation and
inspection, the on-site water supply and/or wastewater disposal system is (are) in compliance with all applicable Municipal and State
codes, ordinances, and regulations in effect at the time of installation.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA
COSA guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time
of the test, and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on
the local soil condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not
guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot
provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole
benefit of the owner listed above. Reliance on this report by another person is at their own risk. Pannone Engineering Services LLC highly
recommends buyers hire their own engineer to evaluate this report.

A water sample revealed an arsenic concentration of 69.3 micrograms per liter
(ug/L). The Environmental Protection Agency (EPA) has established a maximum
contaminant level (MCL) of 10.0 ug/L for public drinking water systems. While
private wells are not subject to this regulation, EPA standards are based on existing
health information and can therefore be used to gauge the relative quality of water
from private wells. information on arsenic is available from the On -Site Water and
Wastewater Program website (www.muni.org/onsite) or at 343-7904.
This advisory must be attached to all copies of the subject Certificate of On -Site
Systems Approval.

Conditional COSA Request
Ladies and Gentlemen:
1 am writing to request a conditional COSA be issued for this property to accommodate its sale in the near future.
Adequate protection is required for the tank and field to prevent vehicles from driving over them. The current
weather conditions are not conducive for optimal installation at this time. Repairs will be less than $2,000.
1. COSA. The Owner is in the process of selling this property. The existing system was located and inspected.
The system was tested for operation and was found passing. All standpipes were located, exposed, and visually
verified. The proposed resolution is to strategically place a form of protection that will be installed in such a way as
to make the remediation permanent and meet the intent of the MOA code.
The proposed installation will not affect the future development of the surrounding or existing lots
