Septic System Documentation & Additional Information
The purpose of this letter is to provide the Department with additional information regarding the supplied record drawing to
document an existing drain field which was installed without a permit.
The conventional on-site wastewater treatment system on this lot serves an existing three (3) bedroom house. The treatment
system consists of a steel 1250-gallon septic tank (installed in 2004 under permit no. SW030246) and a deep trench absorption
field. The subject absorption field was installed as an upgrade to the previously existing drain field without a permit –by the
owner of the property.
PES was contracted by the owner to document the drain field and provide record drawings for MOA documentation. The
attached record drawing reflects data collected in the field by PES and owner/contractor provided documentation. The owner
was informed by the Onsite Department that a test hole would be required to document the system. The test hole was
performed by Ground Breakers, LLC. without PES present. The soils log for this test hole is attached. No groundwater was
observed in the test hole monitor tube o f the test hole conducted by Ground Breakers, LLC. on 06/2524 and 07/02/2024, with
PES verification on 07/09/2024. A test hole was previously performed on this lot by Jim Mack on 10/13/1976. Groundwater and
bedrock were not encountered during the excavation of this test hole. No groundwater was observed in this test hole monitor
tube after 7-days. PES performed an additional test hole on 08/07/2024. Groundwater and bedrock were not encountered
during the excavation of this test hole. The test hole performed by PES and corresponding sieve analysis results appear to
support the test hole and percolation test performed by Jim Mack on 10/13/1976.
Disclaimer:
PES verified the above noted information to the maximum extent feasible. PES did not conduct percolation tests on this
property. The test hole conducted by Ground Breakers, LLC. may verify separation distances to groundwater and bedrock. The
application rates used for the design of the wastewater treatment system were based on Jim Mack’s 10/13/1976 percolation
test and verified by surrounding soils. The system has been operating for some time with no reported issues. The owner of the
property hired PES to document the wastewater treatment system installed on the property in order to obtain an MOA
approval to operate. PES shall not be held liable by this or any other subsequent owner of the property, the Municipality of
Anchorage, the State of Alaska, or the Environmental Protection Agency for any components of the wastewater treatment
system on this property which might violate State or Federal laws.

To whom it may concern:
The existing 2 bedroom house is currently being upgraded to a 3 bedroom house. The existing 3
bedroom septic system contains a 1250 gallon septic tank and is currently in the area of the
proposed addition. We are proposing that the existing 1250 gallon septic tank be abandoned per
UPC code and be replaced with a 1000 gallon septic tank.
1. SURFACE WATERS: There are no surface waters within 100 feet of the proposed
upgrade.
2. TOPOGRAPHY: There are no slope concerns.

Comments: This investigation was completed in compliance with MOA guidelines, regulations,
and best industry practices / methods. The assessment of the condition of the well and septic
applies only to the conditions as of the day tested. The flow and absorption rates may change
due to subsurface conditions that may not be observed from the surface, changes in land use,
local soil characteristics, groundwater levels that may fluctuate during the year, quality of
construction (workmanship & materials), the water usage of the family being served by the
system and maintenance. The operational life of all well and septic systems are subject to

Please submit this letter to the Municipality of Anchorage
Department of Health and Environmental Protection as evidence
that we concur with a previous waiver that: was granted by the
Municipality for the on-site disposal system at the above
mentioned address.
The previous waiver which the Department has a copy of, allowed
the septic tank to be less than the required distance from the
single ;Family well. The actual distance is about 88 feet. We
offer the following justification for the waiver.
1. The septic tank is in good condition and is operating
properly.
2. The ground water table is not near 'the surface, as shown by
the well static water level, thus there is less danger of
contamination.
3. The soile in the area will provide for a medium rate of
percolation. There is no evidence of poorly graded gravels
with little or no fines that would allow rapid percolation
of 'the waste water.
