Readings were taken at the septic tank cleanout and at the pipe at the end of the trench. No water appeared in the end of trench pipe and water immediately backed up in the tank cleanout. After 120 gallons water level in the tank cleanout was at grade and the test was stopped. Since there was no distribution line cleanout and no monitor tube in the trench it was recommended that some be installed. In addition, since the line from the tank to the trench appeared to be plugged and perhaps the distribution line was plugged it was recommended that the lines be unplugged using a large "jetter". Work to locate the trench and install cleanouts and monitor was completed on Monday, May 5, 08 and a reading taken from the new monitor tube indicated the trench was dry. Denali Pumping arrived the next day at 9AM on May 6, 08 at the residence with a pump truck and "jetter" van pulling a trailer with 350 gallons of water with a high pressure pump. The septic tank was pumped and about 900 gallons was removed. Line cleaning was begun at the same time. About 300 gallons of clear water were applied to the trench distribution line under 4,000 PSI at a flow rate of 18GPM. Line was cleaned from both the two new after tank cleanouts and was cleaned to the end of the trench and to the tank. When the jetter hose was removed the hose was clean indicating that the clogs had been liquefied, diluted and blown from the distribution line into the trench. Readings were taken in the new trench monitor tube after the cleaning and a liquid level of 10" was noted. A reading was taken in the end of trench pipe and a liquid level of 14" was noted. Septic Test A septic test was begun at 11:55 AM immediately after the septic tank pumping and line cleaning. Flow was added from the outside faucet on the west side of the residence at a flow rate of 4GPM to one of the two after tank cleanouts. Liquid levels were monitored from the new monitor tube and from the existing pipe at the end of the trench. A total of 1,040 gallons were added during the 4 hour and 31 minutes of pumping. Level in the new monitor at the beginning of the test was 10". The level rose to 18" after 1 hour and 28 minutes of pumping and after 340 gallons. Level in the end of trench pipe was 14" at the beginning of the test and remained at 14" after 340 gallons were pumped. Level in the new monitor dropped from 19 %" to 17 after 30 minutes and to 15 %" after 1 hour. Level in the end of trench pipe rose from 20 % " to 21 % " after 30 minutes and remained at 21 '/2" after 1 hour. Additional recovery readings were taken the next day 24 hours after the end of pumping. The new monitor was 4" (10" at beginning of the test) and the end of trench pipe was 4" (14" at the beginning of the test) indicating recovery of 100%+. PASS — 3 BR Well Test Due to the lack of a well log the well information was taken from the 1987 inspection report. Total depth is 173'. Static was 112' and the yield was 3.3 GPM. At time of inspection the static was 116.6 below top of casing and the yield was 4 GPM. During the 4 hour and 31 minute pumping period level dropped from the static of 116.6 to 117.1. The septic system absorption trench currently in operation on the subject lot was originally constructed in December of 1981. The existing septic tank was placed in 1969 and originally connected to 2 concrete ring absorption pits. The absorption trench was connected to the tank in 1981 and the rings were abandoned. Evidence recently found on the lot indicates a log crib may have been utilized prior to the placement of the septic tank. The trench placed in 1981 was constructed 5' wide with 2' of effective depth. No cleanouts or monitor tubes were placed in the trench. The inspection report completed at the time states the existing septic tank was located 83' from the well with the absorption trench 100' from the well. Since the tank was constructed in 1969 it was only required to be 50' from the well. A recent adequacy test required the placement of a monitor tube in the absorption trench. Two post tank cleanouts were also placed allow access to the distribution line. Excavation for placement of the tubes indicates the trench was only 92' from the well. A waiver allowing the trench to remain and operate in its current location is therefore required. We hereby request a waiver be issued allowing the trench to be 92' from the well on the property. No information is available relating to the well on the lot. It was probably constructed in the late 1970s to replace another well on the lot. Evidence of the first well is found in the Municipal files, but no documentation exists as to why it was decommissioned and the new well drilled. The new well is 173' deep with 6" casing the entire depth. The static water level was recently found at 116.6' below the surface. Very few records exist for wells on adjacent lots. A well approximately 300' to the northeast was found to have nearly identical characteristics to this well with a similar depth and static level. A well approximately 300' to the southwest is 161' deep with a static water level of 105'. We therefore assume the hydraulic gradient is to the northeast at a slope of approximately 5% which is the same as the ground surface. T1 2N, R3W, Section 27, BLM Lot 30 May 7, 2008 Page Two The well log on the property to the southwest indicates hardpan and cobbles from 35' to 150' below the surface. Hardpan is thought to be nearly impermeable silt in the parlance of a well driller. This soil layer would act to restrict the permeability of any surface water or septic effluent and protect the underlying aquifer. Water samples recently taken from the water system on the subject lot indicate a nitrate content of 2.54 milligrams per liter. The well is obviously not influenced by the absorption trench on the property. SUBJECT: T12N, R3W, Section 27, BLM Lot 30 COSA An extensive effort has been made to improve the well and septic system on the subject lot to allow completion of a Certificate of On Site System Approval. This work includes the abandonment and decommissioning of an old well on the property. It also included minor repairs to the existing septic tank and absorption trench to improve their efficiency. Both the well and septic system are now functioning adequately to allow certification as indicated on the attached documentation. An old log crib was found on the property and is located just east of the absorption trench on the property. The top of the crib has been caved in, but a 6' x 6' by nearly 5' deep hole is still in place. The new owner of the property intends to landscape the entire area near the septic system and has requested he be allowed to fill the hole during this process. The hole is lined with logs which appear to be in good condition. There is no evidence of septic effluent or other biological material. The sidewalls are stable and in no danger of sloughing. A small fence has been placed around the perimeter of the hole as a further safety measure.
