We are requesting a change order to the existing open permit to reduce the bedrooms size from 4 -bedrooms to 3 -bedrooms as requested by the owner. Our review of available documentation and field investigation show that this project will not adversely impact any nearby Wells, Wastewater disposal systems, replacement disposal sites, or drainage flowing onto and off of the subject property. As shown on the plan, the tank and field will be greater than 10' from the house foundation. The tank will be of MOA approved construction. The tank shall be covered with a minimum of 2" moa approved insulation and 3' of cover or a minimum of 4' of cover without insulation. The bed will be located adjacent to a testhole performed by PES. The testhole showed groundwater at 4' and free draining soils. The bed will be an elevated mounded system with a minimum of 2' of filter sand below the sewer rock. The bed will be installed on a slope of approximately 3%. A width of 12' was selected to reduce the amount of the bed that has to be elevated above existing grade. A profile view drawing is provided to show the bed and extents of fill relative to the slope. Upon completion of the installation, a record drawing will be submitted showing the location of the new tank, leach field, well, and other applicable features.
